Tämän tutkimuksen tarkoituksena on arvioida, hidastaako seleksipagin lisääminen tavanomaiseen hoitoon keuhkovaltimon hypertensiota (PAH) sairastavien lasten taudin etenemistä lumelääkkeeseen verrattuna.

Lasten PAH on harvinainen ja etenevä sairaus, johon liittyy huomattavaa sairastuvuutta ja kuolleisuutta.
Ottaen huomioon merkittävä lääketieteellinen tarve kehittää hoitoja PAH-lapsille, tarvitaan lisää kliinisiä tutkimuksia lapsiväestöllä, jotta saadaan lisää tietoa lasten PAH:n hoidosta.
Seleksipagi (JNJ-67896049) on suun kautta saatava, selektiivinen ja pitkävaikutteinen prostasykliinireseptorin ei-prostanoidiagonisti, joka on hyväksytty ja kaupallisesti saatavilla aikuisten PAH-potilaiden hoitoon.
Seleksipagilla ja sen metaboliitilla on antifibroottisia, antiproliferatiivisia ja antitromboottisia ominaisuuksia.
Tällä hetkellä prostatykliinireitille suunnattuja lääkkeitä ei ole hyväksytty lasten käyttöön PAH:n hoitoon.
Tehokas ja suun kautta saatavilla oleva prostasykliinireseptoriin vaikuttava hoito, kuten seleksipagi, joka otetaan käyttöön PAH-sairauden lääketieteellisesti sopivassa vaiheessa ja ensisijaisesti yhdessä nykyisten ensilinjan suun kautta otettavien PAH-spesifisten lääkkeiden kanssa osallistujille, jotka tarvitsevat lisähoitoa riittämättömän taudinhallinnan vuoksi. on merkittävä edistysaskel lasten PAH-potilaiden hoidossa.
Tämä tutkimus koostuu enintään 6 viikon mittaisesta seulontajaksosta ja kaksoissokkohoitojaksosta, mukaan lukien titraus- ja ylläpitojaksot, jota seuraa 3 vuoden avoin jatkojakso (OLEP) ja 30 päivän turvallisuusseurantajakso. tutkimustoimenpiteen viimeisen annoksen jälkeen (joko kaksoissokkoutettu tai avoin).
Turvallisuus-, farmakokineettiset ja tehon arvioinnit suoritetaan tutkimuksen aikana.
Riippumaton tiedonseurantakomitea (IDMC) perustetaan seuraamaan tietoja jatkuvasti, tarkastelemaan välitietoja ja varmistamaan tähän tutkimukseen osallistuneiden jatkuva turvallisuus.

Sisällyttämiskriteerit:
- Osallistujat, jotka ovat vähintään (>=) 2 ja alle (<) 18-vuotiaita ja painavat >=9 kilogrammaa (kg) satunnaistuksen yhteydessä
- Keuhkoverenpainetauti (PAH) -diagnoosi vahvistettu dokumentoidulla historiallisella oikean sydämen katetroinnilla (RHC), joka suoritettiin milloin tahansa ennen osallistujan seulontaa

Poissulkemiskriteerit:
- portaalihypertensiosta, skitosomiaasista, keuhkolaskimotukossairaudesta ja/tai keuhkokapillaarien hemangiomatoosista johtuva PAH
- Eisenmengerin oireyhtymään liittyvä PAH
- Aiempi altistuminen Uptraville (seleksipagi)
- Tunnettu samanaikainen henkeä uhkaava sairaus, jonka elinajanodote on alle 12 kuukautta
- Raskaana oleva, raskautta suunnitteleva tai imettävä
- Tunnetut allergiat, yliherkkyys tai intoleranssi seleksipagille tai sen apuaineille

Kokeellinen: Seleksipag
Osallistujat saavat selexipagia painonsa mukaisesti Päivänä 1 ja jatkavat sen jälkeen kahdesti päivässä annosteltuna.
Selexipagin annosta nostetaan ensimmäisten 12 viikon aikana, kunnes osallistuja saavuttaa yksilöllisen suurimman siedetyn annoksen (iMTD) tai kunnes saavutetaan heidän lähtöpainoryhmäänsä vastaava enimmäisannos.
Annoksen nostamista seuraa ylläpitokausi Viikon 12 jälkeen aina hoidon päättymiseen (EOT) saakka suurimmalla siedetyllä annoksella.
Osallistujat jatkavat keuhkovaltimoiden hypertensioon (PAH) erityisesti suunnattujen rinnakkaishoitojen, kuten ERA-lääkkeiden, PDE-5-estäjien ja liukoisen guanylaattisyklaasin stimulaattorin, saamista paikallisen hoitostandardin mukaisesti.

Osallistujille, joilla on positiivinen hyöty/riski-suhde selexipagilla PAH:ta varten, tarjotaan selexipagia avoimen etiketin jatkojakson aikana.
Osallistujat, jotka saavat selexipagia kaksoissokkohoitojakson aikana, jatkavat hoitoa iMTD:llään OLEP:n aikana; niille, jotka aiemmin saivat lumelääkettä, iMTD nostetaan selexipagilla ensimmäisten 12 viikon aikana, kunnes osallistuja saavuttaa iMTD:n.

Disease progression event as adjudicated by CEC, defined as any of the following: death (all causes); atrial septostomy or Potts' anastomosis, or registration on lung transplant list; hospitalization due to worsening pulmonary arterial hypertension (PAH); clinical worsening of PAH.

If a participant had reached Tanner stage 5, no further tanner pubertal stage assessment were to be completed.
Tanner stage was assessed in F >=8 years; M >=9 years.
Pubic hair growth (M and F) stages: S1: No hair, S2: Downy hair, S3: More coarse and curly hair, S4: Adult-like hair quality; S5: Hair extends to medial surface of thighs.
BD in females: S1: Nipple raised a little, rest of breast still flat, S2: Breast bud forms, S3: More elevated, outside areola, S4: Increased breast size, S5: Final adult-size breasts.
GD in males: S1: Testes, scrotum, and penis about same size, S2: Enlargement of scrotum, testes, and penis, S3: Enlargement of penis, S4: Penis and glands became larger, S5: Genitalia size and shape same an adult male.
