Celem pracy jest ocena, czy dodanie seleksypagu do standardowego leczenia opóźnia progresję choroby u dzieci z tętniczym nadciśnieniem płucnym (TNP) w porównaniu z placebo.

PAH u dzieci jest rzadką i postępującą chorobą związaną ze znaczną chorobowością i śmiertelnością.
Biorąc pod uwagę istotną medyczną potrzebę opracowania terapii u dzieci z PAH, potrzebne są dalsze badania kliniczne w populacji pediatrycznej, aby uzyskać więcej danych dotyczących leczenia PAH u dzieci.
Selexipag (JNJ-67896049) jest dostępnym doustnie, selektywnym i długo działającym nieprostanoidowym agonistą receptora prostacykliny, zatwierdzonym i dostępnym w handlu do leczenia dorosłych uczestników z PAH.
Seleksypag i jego metabolit mają właściwości przeciwzwłóknieniowe, przeciwproliferacyjne i przeciwzakrzepowe.
Obecnie żaden lek ukierunkowany na szlak prostacyklin nie jest dopuszczony do stosowania u dzieci i młodzieży w PAH.
Skuteczna i dostępna doustnie terapia działająca na receptor prostacykliny, taka jak seleksypag, wprowadzona w medycznie odpowiednim stadium choroby PAH, a przede wszystkim w połączeniu z aktualnymi doustnymi lekami swoistymi dla PAH pierwszego rzutu u uczestników wymagających dodatkowej terapii z powodu niewystarczającej kontroli choroby, byłaby stanowi duży postęp w leczeniu pacjentów pediatrycznych z PAH.
Badanie to składa się z okresu przesiewowego trwającego do 6 tygodni i okresu leczenia metodą podwójnie ślepej próby, w tym zwiększania dawki i okresów leczenia podtrzymującego, po którym następuje 3-letni okres otwartej próby (OLEP) i 30-dniowa obserwacja bezpieczeństwa w górę, który występuje po ostatniej dawce interwencji badawczej (podwójnie zaślepiona lub otwarta próba).
Podczas badania zostaną przeprowadzone oceny bezpieczeństwa, farmakokinetyki i skuteczności.
Powołany zostanie Niezależny Komitet Monitorowania Danych (IDMC) w celu bieżącego monitorowania danych, przeglądu danych pośrednich oraz zapewnienia stałego bezpieczeństwa uczestników biorących udział w tym badaniu.
Przybliżony czas trwania badania wynosi 8 lat.

Kryteria przyjęcia:
- Uczestnicy w wieku od (>=) 2 do mniej niż (<) 18 lat, ważący >=9 kilogramów (kg) w momencie randomizacji
- Rozpoznanie tętniczego nadciśnienia płucnego (TNP) potwierdzone udokumentowanym historycznym cewnikowaniem prawego serca (RHC) wykonanym w dowolnym momencie przed badaniem przesiewowym uczestnika
- PAH (Grupa 1 Światowej Organizacji Zdrowia [WHO]), w tym uczestnicy z zespołem Downa, o następującej etiologii: idiopatyczne PAH (IPAH); dziedziczne PAH (HPAH); TNP związane z wrodzoną wadą serca (TNP związane z wrodzoną wadą serca [aCHD]) (TNP z współistniejącą CHD [tj. małym ubytkiem przegrody międzyprzedsionkowej, ubytkiem przegrody międzykomorowej lub przetrwałym przewodem tętniczym, które samo w sobie nie odpowiada za rozwój podwyższony PVR] i jeśli został zatwierdzony przez BCAC) i pooperacyjne PAH (utrzymujące się/nawracające/rozwijające się >=6 miesięcy po naprawie CHD); Wywołane lekami lub toksynami; WWA związane z ludzkim wirusem niedoboru odporności (HIV)

- Uczestnicy leczeni co najmniej 1 lekiem specyficznym dla PAH, na przykład antagonistą receptora endoteliny (ERA) i/lub inhibitorem fosfodiesterazy typu 5 (PDE-5)/stymulatorem rozpuszczalnej cyklazy guanylowej, pod warunkiem, że dawka lecznicza została(-y) stabilny przez co najmniej 3 miesiące przed podaniem pierwszej dawki badanej interwencji
Kryteria wyłączenia:
- TNP spowodowane nadciśnieniem wrotnym, schistosomatozą, chorobą zarostową żył płucnych i/lub naczyniakowatością naczyń włosowatych płuc
- TNP związane z zespołem Eisenmengera
- Wcześniejsza ekspozycja na Uptravi (seleksypag)
- Znana współistniejąca choroba zagrażająca życiu z oczekiwaną długością życia <12 miesięcy
- Ciąża, planowanie ciąży lub karmienie piersią
- Znane alergie, nadwrażliwość lub nietolerancja na seleksypag lub jego substancje pomocnicze

Eksperymentalny: Seleksypag
Uczestnicy otrzymają selexipag na podstawie masy ciała w Dniu 1 i będą kontynuować podawanie dwa razy dziennie.
Sekselipag będzie zwiększany w ciągu pierwszych 12 tygodni, aż uczestnik osiągnie indywidualną maksymalną tolerowaną dawkę (iMTD) lub maksymalną dawkę odpowiadającą kategorii masy ciała wyjściowej.
Po zwiększaniu dawki następuje okres utrzymania po 12. tygodniu aż do zakończenia leczenia (EOT), przy maksymalnej tolerowanej dawce.
Uczestnicy będą nadal otrzymywać specyficzne terapie towarzyszące w leczeniu nadciśnienia płucnego (PAH), takie jak ERA, inhibitory PDE-5 i stymulator rozpuszczalnej cyklazy guanylanowej, zgodnie z lokalnym standardem opieki.

Uczestnicy otrzymają placebo dopasowane do masy ciała w Dniu 1 i będą kontynuować z dawką podawaną dwa razy dziennie.
Uczestnicy będą nadal otrzymywać specyficzne dla PAH terapie towarzyszące, takie jak ERA, inhibitory PDE-5 i stymulatory rozpuszczalnej cyklazy guanylowej, zgodnie z lokalnym standardem opieki.

Uczestnicy z pozytywnym współczynnikiem korzyści/ryzyka dla selekcypagu w PAH otrzymają selekcypag w okresie otwartej obserwacji.
Uczestnicy przyjmujący selekcypag w okresie podwójnie ślepej próby będą kontynuować leczenie w ich iMTD podczas OLEP, a u tych wcześniej przyjmujących placebo, iMTD będzie zwiększany w ciągu pierwszych 12 tygodni, aż uczestnik osiągnie iMTD. Uczestnicy będą nadal otrzymywać specyficzne terapie towarzyszące dla PAH, takie jak ERA, inhibitory PDE-5 i stymulatory rozpuszczalnej cyklazy guanylanowej, zgodnie z lokalnymi standardami opieki. |

A serious AE was defined as any untoward medical occurrence that at any dose resulted in death, was life-threatening, required inpatient hospitalization or prolongation of existing hospitalization, resulted in persistent or significant disability/incapacity, was a congenital anomaly/birth defect, was a suspected transmission of any infectious agent via a medicinal product, or was medically important.

Tanner stage was assessed in F >=8 years; M >=9 years.
Pubic hair growth (M and F) stages: S1: No hair, S2: Downy hair, S3: More coarse and curly hair, S4: Adult-like hair quality; S5: Hair extends to medial surface of thighs.
BD in females: S1: Nipple raised a little, rest of breast still flat, S2: Breast bud forms, S3: More elevated, outside areola, S4: Increased breast size, S5: Final adult-size breasts.
