Une étude randomisée, multicentrique, en double aveugle, contrôlée par placebo, en groupes parallèles, axée sur les événements, séquentielle en groupe avec période d'extension en ouvert pour évaluer l'efficacité et l'innocuité du sélexipag en tant que traitement complémentaire à la norme de soins chez les enfants Âgé >=2 à
Le but de cette étude est d'évaluer si l'ajout de sélexipag au traitement standard de soins retarde la progression de la maladie chez les enfants atteints d'hypertension artérielle pulmonaire (HTAP) par rapport au placebo.

Description détaillée
L'HTAP pédiatrique est une maladie rare et évolutive associée à une morbidité et une mortalité considérables.
Compte tenu du besoin médical important de développer des traitements chez les enfants atteints d'HTAP, des études cliniques complémentaires dans la population pédiatrique sont donc nécessaires pour fournir davantage de données pour la prise en charge de l'HTAP chez les enfants.
Selexipag (JNJ-67896049) est un agoniste non prostanoïde disponible par voie orale, sélectif et à longue durée d'action du récepteur de la prostacycline approuvé et disponible dans le commerce pour le traitement des participants adultes atteints d'HTAP.
Le sélexipag et son métabolite possèdent des propriétés anti-fibrotiques, anti-prolifératives et anti-thrombotiques.
Actuellement, aucun médicament ciblant la voie de la prostacycline n'est approuvé pour une utilisation pédiatrique dans l'HTAP.
Une thérapie efficace et disponible par voie orale agissant sur le récepteur de la prostacycline, telle que le sélexipag, introduite à un stade médicalement approprié de l'HTAP, et principalement en association avec des médicaments oraux spécifiques de première intention actuels contre l'HTAP chez les participants nécessitant un traitement supplémentaire en raison d'un contrôle insuffisant de la maladie, représente une avancée majeure dans la prise en charge thérapeutique des participants pédiatriques de l'HTAP.
Cette étude consiste en une période de dépistage pouvant aller jusqu'à 6 semaines et une période de traitement en double aveugle, comprenant des périodes de titration et d'entretien, suivies d'une période d'extension en ouvert de 3 ans (OLEP) et d'un suivi de sécurité de 30 jours. période de mise en place qui survient après la dernière dose de l'intervention à l'étude (soit en double aveugle, soit en ouvert).
Des évaluations de l'innocuité, de la pharmacocinétique et de l'efficacité seront effectuées au cours de l'étude.
Un comité indépendant de surveillance des données (IDMC) sera créé pour surveiller les données de manière continue, pour examiner les données intermédiaires et pour assurer la sécurité continue des participants inscrits à cette étude.
La durée approximative de l'étude est de 8 ans.

Critère d'intégration:
- Participants entre supérieur ou égal à (>=) 2 et inférieur à (<) 18 ans pesant >= 9 kilogrammes (kg) lors de la randomisation
- Diagnostic d'hypertension artérielle pulmonaire (HTAP) confirmé par un cathétérisme cardiaque droit (RHC) historique documenté effectué à tout moment avant le dépistage du participant
- HTAP (Groupe 1 de l'Organisation mondiale de la santé [OMS]), y compris les participants atteints du syndrome de Down, des étiologies suivantes : HTAP idiopathique (IPAH) ; HAP héréditaire (HPAH); HTAP associée à une cardiopathie congénitale (HTAP associée à une cardiopathie congénitale [aCHD]) (HTAP avec coronaropathie coïncidente [c'est-à-dire une petite communication interauriculaire, une communication interventriculaire ou une persistance du canal artériel qui n'est pas elle-même responsable du développement de PVR élevé] et si approuvé par le BCAC) et HTAP postopératoire (persistante/récidivante/en développement >= 6 mois après la réparation d'une maladie coronarienne) ; Induit par un médicament ou une toxine ; HTAP associée au virus de l'immunodéficience humaine (VIH)
- Classe fonctionnelle OMS (FC) II et III
- Participants traités avec au moins 1 traitement spécifique à l'HTAP, par exemple, un antagoniste des récepteurs de l'endothéline (ARE) et/ou un inhibiteur de la phosphodiestérase de type 5 (PDE-5)/stimulateur de la guanylate cyclase soluble, à condition que la ou les doses de traitement aient été stable pendant au moins 3 mois avant la première dose de l'intervention à l'étude
Critère d'exclusion:
- HTAP due à une hypertension portale, une schistosomiase, une maladie veino-occlusive pulmonaire et/ou une hémangiomatose capillaire pulmonaire
- HTAP associée au syndrome d'Eisenmenger
- Exposition antérieure à Uptravi (selexipag)
- Maladie concomitante connue menaçant le pronostic vital avec une espérance de vie inférieure à 12 mois
- Enceinte, prévoyant de devenir enceinte ou allaitante
- Allergies, hypersensibilité ou intolérance connues au sélexipag ou à ses excipients

Expérimental: Selexipag
Les participants recevront du sélexipag en fonction du poids corporel au jour 1 et poursuivront ensuite avec une administration deux fois par jour.
Le sélexipag sera augmenté progressivement au cours des 12 premières semaines jusqu'à ce que les participants atteignent la dose maximale tolérée individuelle (iMTD) ou jusqu'à ce qu'une dose maximale correspondant à leur catégorie de poids corporel initial soit atteinte.
L'augmentation progressive est suivie d'une période de maintenance après la semaine 12 jusqu'à la fin du traitement (EOT), à la dose maximale tolérée.
Les participants continueront à recevoir des thérapies concomitantes spécifiques à l'hypertension artérielle pulmonaire (HTAP) telles que les ERA, les inhibiteurs de la PDE-5 et le stimulateur de la guanylate cyclase soluble conformément aux normes de soins locales.

Comparateur placebo: Placebo
Les participants recevront un placebo correspondant en fonction du poids corporel au jour 1 et continueront ensuite avec une posologie deux fois par jour.
Les participants continueront à recevoir des thérapies concomitantes spécifiques à l'HTAP telles que les ERA, les inhibiteurs de la PDE-5 et le stimulateur de la guanylate cyclase soluble conformément aux normes de soins locales.

Les ERA seront administrés en tant que traitement standard.

Expérimental: Période d'extension en ouvert : Selexipag
Les participants présentant un rapport bénéfice/risque positif du sélexipag pour l'hypertension artérielle pulmonaire (HTAP) se verront proposer du sélexipag durant la période d'extension en ouvert.
Les participants sous sélexipag durant la période de traitement en double aveugle poursuivront leur traitement à leur dose maximale tolérée individualisée (DMTi) durant la période d'extension en ouvert (PEO). Pour ceux précédemment sous placebo, la DMTi sera augmentée progressivement au cours des 12 premières semaines jusqu'à ce que le participant atteigne sa DMTi.
Les participants continueront à recevoir les thérapies concomitantes spécifiques à l'HTAP, telles que les antagonistes des récepteurs de l'endothéline (ARE), les inhibiteurs de la phosphodiestérase de type 5 (PDE-5) et les stimulateurs de la guanylate cyclase soluble, conformément aux standards de soins locaux.

Disease progression event as adjudicated by CEC, defined as any of the following: death (all causes); atrial septostomy or Potts' anastomosis, or registration on lung transplant list; hospitalization due to worsening pulmonary arterial hypertension (PAH); clinical worsening of PAH.
Clinical worsening of PAH was defined as initiation of new PAH-specific therapy or intravenous diuretics or continuous oxygen use and at least 1 of the following: worsening in World Health Organization (WHO) functional class (FC); new occurrence or worsening: of syncope, of at least 2 PAH symptoms (shortness of breath/dyspnea, chest pain, cyanosis, dizziness/near syncope, or fatigue), of signs of right heart failure not responding to oral diuretics.

If a participant had reached Tanner stage 5, no further tanner pubertal stage assessment were to be completed.
Tanner stage was assessed in F >=8 years; M >=9 years.
Pubic hair growth (M and F) stages: S1: No hair, S2: Downy hair, S3: More coarse and curly hair, S4: Adult-like hair quality; S5: Hair extends to medial surface of thighs.
BD in females: S1: Nipple raised a little, rest of breast still flat, S2: Breast bud forms, S3: More elevated, outside areola, S4: Increased breast size, S5: Final adult-size breasts.
