PAH bij kinderen is een zeldzame en progressieve aandoening die gepaard gaat met aanzienlijke morbiditeit en mortaliteit. Gezien de grote medische noodzaak om behandelingen voor kinderen met PAH te ontwikkelen, zijn daarom verdere klinische studies bij pediatrische patiënten nodig om meer gegevens te verzamelen voor de behandeling van PAH bij kinderen. Selexipag (JNJ-67896049) is een oraal verkrijgbare, selectieve en langwerkende niet-prostanoïde agonist van de prostacyclinereceptor die is goedgekeurd en in de handel verkrijgbaar is voor de behandeling van volwassen deelnemers met PAH. Selexipag en zijn metaboliet hebben antifibrotische, antiproliferatieve en antitrombotische eigenschappen. Momenteel zijn er geen geneesmiddelen die zich richten op de prostacyclineroute goedgekeurd voor pediatrisch gebruik bij PAH. Een effectieve en oraal beschikbare therapie die inwerkt op de prostacyclinereceptor, zoals selexipag, geïntroduceerd in een medisch geschikt stadium van de PAH-ziekte, en voornamelijk in combinatie met huidige eerstelijns orale PAH-specifieke geneesmiddelen bij deelnemers die aanvullende therapie nodig hebben vanwege onvoldoende ziektecontrole, zou vertegenwoordigt een belangrijke vooruitgang in de therapeutische behandeling van pediatrische PAH-deelnemers. Deze studie bestaat uit een screeningperiode van maximaal 6 weken en een dubbelblinde behandelingsperiode, inclusief optitratie- en onderhoudsperioden, gevolgd door een open-label extensieperiode van 3 jaar (OLEP) en een veiligheidscontrole van 30 dagen. up-periode die optreedt na de laatste dosis studie-interventie (dubbelblind of open-label). Inclusiecriteria:

  • Deelnemers tussen groter dan of gelijk aan (>=) 2 en jonger dan (<) 18 jaar met een gewicht >=9 kilogram (kg) bij randomisatie
  • Diagnose van pulmonale arteriële hypertensie (PAH) bevestigd door gedocumenteerde historische rechterhartkatheterisatie (RHC) uitgevoerd op enig moment voorafgaand aan de screening van de deelnemer
  • PAH (Wereldgezondheidsorganisatie [WHO] groep 1), inclusief deelnemers met het syndroom van Down, met de volgende etiologieën: idiopathische PAH (IPAH); Erfelijke PAH (HPAH); PAH geassocieerd met congenitale hartziekte (PAH-geassocieerd met congenitale hartziekte [aCHD]) (PAH met toevallige CHD [d.w.z. een klein atriumseptumdefect, ventrikelseptumdefect of open ductus arteriosus die zelf niet verantwoordelijk is voor de ontwikkeling van verhoogde PVR] en indien goedgekeurd door de BCAC) en postoperatieve PAH (aanhoudend / terugkerend / zich ontwikkelend >=6 maanden na herstel van CHD); Geneesmiddel- of toxine-geïnduceerd; PAH geassocieerd met humaan immunodeficiëntievirus (HIV)

Uitsluitingscriteria:

  • PAH als gevolg van portale hypertensie, schistosomiasis, pulmonale veno-occlusieve ziekte en/of pulmonale capillaire hemangiomatose
  • PAH geassocieerd met Eisenmenger-syndroom
  • Eerdere blootstelling aan Uptravi (selexipag)
  • Bekende bijkomende levensbedreigende ziekte met een levensverwachting <12 maanden
  • Zwanger, van plan zwanger te worden of borstvoeding te geven
  • Bekende allergieën, overgevoeligheid of intolerantie voor selexipag of zijn hulpstoffen

Experimenteel: Selexipag
Deelnemers zullen selexipag ontvangen op basis van het lichaamsgewicht op Dag 1 en zullen daarna doorgaan met tweemaal daagse dosering. Selexipag zal tijdens de eerste 12 weken worden opgetitreerd totdat de deelnemers de individuele maximaal getolereerde dosis (iMTD) bereiken of totdat een maximale dosis die overeenkomt met hun baseline lichaamsgewichtscategorie wordt bereikt. Optitratie wordt gevolgd door een onderhoudsperiode na Week 12 tot het einde van de behandeling (EOT), op de maximaal getolereerde dosis. Deelnemers zullen doorgaan met het ontvangen van pulmonale arteriële hypertensie (PAH)-specifieke gelijktijdige therapieën zoals ERA's, PDE-5-remmers en oplosbare guanylaatcyclase-stimulator volgens de lokale standaardzorg.
De Selexipag-tablet wordt oraal toegediend.

Deelnemers zullen op dag 1 een overeenkomstig placebo ontvangen op basis van het lichaamsgewicht en zullen daarna doorgaan met tweemaal daagse dosering. Deelnemers zullen blijven deelnemen aan PAH-specifieke gelijktijdige therapieën zoals ERA's, PDE-5-remmers en oplosbare guanylaatcyclase-stimulatoren volgens de lokale standaardzorg.
Bijpassende placebotabletten zullen oraal worden toegediend.
ERAs worden toegediend als SOC-therapie.
PDE-5-remmer zal als SOC-therapie worden toegediend.

Experimenteel: Open-Label Extension Periode: Selexipag
Deelnemers met een positieve baten/risicoverhouding van selexipag voor PAH krijgen selexipag aangeboden in de open-label extensieperiode. Deelnemers die tijdens de dubbelblinde behandelingsperiode selexipag gebruiken, blijven tijdens de OLEP behandelen op hun iMTD; voor degenen die eerder placebo kregen, zal de iMTD gedurende de eerste 12 weken selexipag opgehoogd worden totdat de deelnemer de iMTD bereikt. Deelnemers blijven PAH-specifieke bijkomende therapieën ontvangen, zoals ERA's, PDE-5-remmers en oplosbare guanylaatcyclase-stimulatoren, volgens de lokale standaardzorg.

If a participant had reached Tanner stage 5, no further tanner pubertal stage assessment were to be completed. Tanner stage was assessed in F >=8 years; M >=9 years. Pubic hair growth (M and F) stages: S1: No hair, S2: Downy hair, S3: More coarse and curly hair, S4: Adult-like hair quality; S5: Hair extends to medial surface of thighs. BD in females: S1: Nipple raised a little, rest of breast still flat, S2: Breast bud forms, S3: More elevated, outside areola, S4: Increased breast size, S5: Final adult-size breasts. GD in males: S1: Testes, scrotum, and penis about same size, S2: Enlargement of scrotum, testes, and penis, S3: Enlargement of penis, S4: Penis and glands became larger, S5: Genitalia size and shape same an adult male.
