Satunnaistettu, kontrolloitu, ei-plasebotutkimus, jonka tarkoituksena oli ensisijaisesti arvioida suun kautta otettavien avohoitoantibioottien (eli atsitromysiinin ja amoksisilliinin) vaikutusta latenssiin (ts. niiden potilaiden osuuteen, jotka synnyttävät 28 päivän sisällä kalvon repeämisestä) ehkäistävissä olevan, synnytystä edeltävän kalvojen repeämisen jälkeen raskausviikkojen 18 0/7 ja 22 6/7 välillä.

Sisällyttämiskriteerit:

- Elävä, yksittäinen raskaus
- Potilas pystyy antamaan tietoisen suostumuksen
- Raskausaika 18 viikon ja 0 päivän välillä ja 22 viikon ja 6 päivän välillä kalvon repeämisen aikaan
- Kalvojen ennenaikaisen, synnytystä edeltävän repeämän diagnoosi kliinisen tutkimuksen löydöksillä joko 1) kohdunkaulan kanavasta virtaavan lapsivesien ja/tai emättimeen kerääntyvän nesteen visualisoimalla steriilillä tähystystutkimuksella, 2) emäksisellä pH-testillä (eli positiivinen nitratsiini) emätinneste, 3) mikroskooppisella tutkimuksella tunnistetun kuivatun emätinnesteen arborisaatio (eli ferning) ja/tai 4) lapsivesiindeksi (AFI) on alle 4 cm
Poissulkemiskriteerit:
- Raskausajan ajoitus suoritettu tai vahvistettu ultraäänellä ≥ 18 viikon ja 0 päivän raskausiässä
- Potilas haluaa raskauden keskeyttämisen tai synnytyksen käynnistämisen
- Tunnettu suuri sikiön anomalia tai aneuploidia
- Amniocenteesi ≤ 7 päivää kalvon repeämän diagnoosista
- Kohdunkaulan hammaskiven asettaminen ≤ 7 päivää kalvon repeämän diagnoosista
- Tunnettu lääkeaineallergia tai merkittävät haittavaikutukset makrolidi- tai penisilliiniantibiooteille
- Nykyinen antibioottien käyttö kalvon repeämän diagnoosin aikana
- Emättimen verenvuoto kalvon repeämän diagnoosin yhteydessä tai ensimmäisten 24 tunnin sisällä diagnoosista
- Kuumeinen kalvon repeämän diagnoosin yhteydessä (eli lämpötila ≥ 38 celsiusastetta) ja/tai ensimmäisten 24 tunnin sisällä diagnoosista
- Aktiivinen ennenaikainen synnytys kalvon repeämän diagnoosin yhteydessä (eli kohdunkaulan muutokseen liittyvä johdonmukainen supistumiskuvio) ja/tai ensimmäisen 24 tunnin sisällä diagnoosista
- Kohdunkaulan laajeneminen ≥ 4 cm
- Sikiön osien esiinluiskahdus sisäisen kohdunkaulan osien tason yläpuolelle
- Täydellisen, 7 päivän avohoitoseurantaa ei tarvitse suorittaa ennen sairaalaan ottamista uudelleen, jos repeämä tapahtuu 22. raskausviikon aikana

Kokeellinen: Antibiootit
Potilaita, jotka on satunnaistettu tutkimuksen hoitoryhmään (eli antibioottihoitoon), hoidetaan seitsemän päivän suun kautta otettavalla atsitromysiinillä ja amoksisilliinilla.
Atsitromysiiniä annostellaan 500 mg:n kerta-annoksena (2-250 mg tabletteja suun kautta) välittömästi satunnaistamisen jälkeen, mutta ennen kotiinlähtöä, minkä jälkeen 1-250 mg tabletti päivässä 4 lisäpäivän ajan (yhteensä 5 päivää).
Amoksisilliinia annostellaan yhtenä 500 mg:n tablettina suun kautta kolme kertaa päivässä 7 päivän ajan, ja ensimmäinen annos annetaan myös ennen kotiinlähtöä.

The proportion of patient's "positive" for severe neonatal morbidity composite.
A neonate will be termed "positive" for severe neonatal morbidity composite if any one of the following is diagnosis: bronchopulmonary dysplasia (BPD), pulmonary hypoplasia, intraventricular hemorrhage (IVH) grade III/IV, necrotizing enterocolitis (NEC) Bell's Stage II or greater, neonatal sepsis with positive blood cultures, neonatal pneumonia with positive blood cultures, neonatal death.
