- Patiënt kan geïnformeerde toestemming geven
- Zwangerschapsduur tussen 18 weken en 0 dagen en 22 weken en 6 dagen op het moment van membraanruptuur
- Diagnose van vroeggeboorte, prelabor ruptuur van vliezen door klinische onderzoeksbevindingen van ofwel 1) visualisatie van vruchtwater dat uit het cervicale kanaal komt en/of zich in de vagina verzamelt via steriel speculumonderzoek, 2) een basische pH-test (d.w.z. positieve nitrazine) van vaginale vloeistof, 3) arborisatie (d.w.z. ferning) van gedroogde vaginale vloeistof geïdentificeerd via microscopisch onderzoek, en/of 4) een vruchtwaterindex (AFI) van minder dan 4 cm
Uitsluitingscriteria:
- Zwangerschapsdatering uitgevoerd of bevestigd door middel van echografie bij ≥ 18 weken en 0 dagen zwangerschapsduur
- Patiënt wenst zwangerschapsonderbreking of inleiding van de bevalling
- Bekende ernstige foetale anomalie of aneuploïdie
- Vruchtwaterpunctie ≤ 7 dagen na diagnose van gebroken vliezen
- Cervicale cerclage plaatsing ≤ 7 dagen na diagnose van gescheurde vliezen
- Bekende medicijnallergie of significante bijwerkingen van macrolide- of penicilline-antibiotica
- Huidig antibioticagebruik op het moment van diagnose van membraanbreuk
- Vaginale bloeding op het moment van diagnose van membraanbreuk of binnen de eerste 24 uur na diagnose
- Koorts op het moment van diagnose van membraanbreuk (d.w.z. temperatuur ≥ 38 graden Celsius) en/of binnen de eerste 24 uur na diagnose
- Actieve vroeggeboorte op het moment van diagnose van membraanbreuk (d.w.z. consistent samentrekkingspatroon geassocieerd met cervicale verandering) en/of binnen de eerste 24 uur na diagnose
- Cervicale dilatatie van ≥ 4 cm
- Verzakking van foetale delen voorbij het niveau van de interne cervicale os
- Weigering om volledige, 7-daagse poliklinische monitoring te voltooien voorafgaand aan heropname in het ziekenhuis, mocht er een ruptuur optreden tijdens de 22e week van de zwangerschap

Experimenteel: Antibiotica
Patiënten die gerandomiseerd zijn in de behandelingsarm (d.w.z. antibiotica) van het onderzoek zullen worden behandeld met een zevendaagse kuur met oraal azithromycine en amoxicilline.
Azitromycine zal worden gedoseerd als een enkele dosis van 500 mg (2-250 mg orale tabletten), toegediend onmiddellijk na randomisatie, maar vóór ontslag naar huis, gevolgd door 1-250 mg orale tablet per dag gedurende 4 extra dagen (voor een totaal van 5 dagen).
Amoxicilline wordt driemaal daags gedurende 7 dagen gedoseerd als een enkele orale tablet van 500 mg, waarbij de eerste dosis ook wordt gegeven voordat u naar huis gaat.

A neonate will be termed "positive" for severe neonatal morbidity composite if any one of the following is diagnosis: bronchopulmonary dysplasia (BPD), pulmonary hypoplasia, intraventricular hemorrhage (IVH) grade III/IV, necrotizing enterocolitis (NEC) Bell's Stage II or greater, neonatal sepsis with positive blood cultures, neonatal pneumonia with positive blood cultures, neonatal death.
