Tutkimus CDR132L:n tehon ja turvallisuuden arvioimiseksi potilailla, joilla on pienentynyt vasemman kammion ejektiofraktio sydäninfarktin jälkeen (HF-REVERT)
Vaihe 2, monikeskus, satunnaistettu, rinnakkais, 3-haarainen, lumekontrolloitu tutkimus CDR132L:n tehon ja turvallisuuden arvioimiseksi potilailla, joilla on pienentynyt vasemman kammion ejektiofraktio (≤ 45 %) sydäninfarktin jälkeen
Tämä on vaiheen 2, monikeskus, satunnaistettu, rinnakkainen, 3-haarainen, lumekontrolloitu tutkimus, jolla arvioidaan CDR132L:n tehoa ja turvallisuutta potilailla, joilla on pienentynyt vasemman kammion ejektiofraktio (LVEF) (≤ 45 %) sydäninfarktin (MI) jälkeen. Tämä tutkimus koostuu seulontajaksosta (joka tapahtuu vähintään 3 päivää MI-diagnoosin jälkeen), 6 kuukauden kaksoissokkojaksosta ja 6 kuukauden jatkojaksosta tutkimuksen lopussa (EOS) päivänä 360/kuukausi 12 .
Kahta CDR132L-annosta testataan lumelääkettä vastaan niiden vaikutuksista potilaisiin, jotka ovat juuri saaneet sydänkohtauksen tavanomaisen hoidon lisäksi. Tutkimuksen tavoitteena on osoittaa, että CDR132L on turvallinen ja tehokas parantamaan tällaisten potilaiden sydämen vajaatoimintaa.

Kuvaus
Tärkeimmät osallistumiskriteerit:
- Mies- tai naispotilaat, jotka ovat ≥ 30 - ≤ 80-vuotiaita tietoisen suostumuksen allekirjoituspäivänä, joka määritellään seulontajakson alkajaksi.
- Spontaani akuutti sydäninfarkti (AMI) (tyyppi I), joka perustuu yleiseen sydäninfarktin määritelmään, ja satunnaistaminen tapahtuu viimeistään 14 päivää indeksitapahtuman diagnoosin jälkeen.
- Potilas, jonka LVEF ≤ 45 % mitattuna ECHO:lla MI-diagnoosin jälkeen (STEMI tai NSTEMI).
- Mukaan voidaan ottaa potilas, jolla on aiempia sydäninfarktin tapahtumia.
- Potilas, jonka paino on ≤ 120 kg.
- N-terminaalisen pro B-tyypin natriureettisen peptidin taso ≥ 125 pg/ml ja < 8000 pg/ml seulonnassa.
- STEMI/NSTEMI-potilas, jolle tehtiin perkutaaninen sepelvaltimointerventio tämän tapahtuman vuoksi.
Poissulkemiskriteerit:
- Hedelmällisessä iässä oleva nainen (WOCBP).
- Potilas, jolla on ei-iskeeminen alkuperä; esim. sydänlihastulehdus, alkoholinen kardiomyopatia.
- Potilas, jolla on New York Heart Associationin (NYHA) luokka IV seulonnassa tai satunnaistuksessa.
- Potilaalla on suunniteltu sydäninterventio (angiogrammi ilman angioplastiaa on hyväksyttävä) tai muu suunniteltu leikkaus seulontajakson jälkeen.
- Potilaalla on vaikea sydänläppäsairaus.
- Potilaan systolinen verenpaine < 90 mmHg tai > 180 mmHg, diastolinen verenpaine < 50 mmHg tai > 110 mmHg ja/tai syke < 50 tai > 100 lyöntiä minuutissa seulonnassa tai satunnaistuksessa.
- Potilas, jonka arvioitu glomerulussuodatusnopeus < 30 ml/min/1,73 m2 tai dialyysihoidossa.
- Potilas, jolla on Child-Pugh B tai C luokiteltu maksan vajaatoiminta.
- Potilaalla on ollut veri-aivoesteeseen vaikuttavia sairauksia, esim. aivohalvaus 6 kuukauden sisällä tai multippeliskleroosi.
- Potilaalla on ollut verenvuotohäiriöitä tai trombosytopenia (verihiutaleita < 100 000/μl).
- Potilaalla on tutkijan määrittämän huonosti hallinnassa oleva diabetes.
- Potilaalla on ollut jokin seuraavista sydänsairauksista: tunnetut sydämen rakenteelliset poikkeavuudet HF:n lisäksi, suvussa pitkä QT-oireyhtymä, sydämen pyörtyminen tai toistuva, idiopaattinen pyörtyminen.
- Kaikki kliinisesti merkittävät poikkeavuudet, tutkijan harkinnan mukaan lepo-EKG:n rytmissä, johtumisessa tai morfologiassa, jotka aiheuttavat ylimääräisen turvallisuusriskin potilaille.
- Potilaalla, jolla on aktiivinen "vakava akuutti hengitystieoireyhtymä koronavirus 2 (SARS-CoV-2)" -infektio, joka on vahvistettu paikallisten testausohjeiden mukaisesti seulonnassa.
- Potilasta ei saa ottaa mukaan tutkimukseen, jos hän on saanut kiellettyä hoitoa 3 kuukauden sisällä seulonnasta.

CDR132L on synteettinen antisense-oligonukleotidi (ASO) ja mikroRNA-132-3p:n (miR-132) selektiivinen estäjä.
miR-132 kardiomyosyyteissä on keskeinen kytkin, joka vaikuttaa sellaisten geenien ilmentymiseen, jotka ovat ratkaisevassa asemassa sydämen epämukavassa uudelleenmuodostumisessa, transformaatiossa ja patologisessa sydämen kasvussa (hypertrofia), mikä edistää sydämen haitallista uudelleenmuodostumista ja sydämen vajaatoimintaa (HF).1-5
MiR-132:n poikkeava ilmentyminen sydänsoluissa liittyy kausaalisesti sydämen uudelleenmuotoiluun ja HF:n etenemiseen.

The KCCQ is a 23-item, self-administered ques-tionnaire developed to independently measure the participant's perception of their health status.
Scores range from 0 to 100, with 0 as lowest score and 100 as the highest score.
Higher scores indi-cate better health status, fewer symptoms, and greater disease-specific health-related quality of life.
The calculation is following the KCCQ scoring manual that states: "Overall Summary Score is defined as mean of the following available summary scores: Physical Limitation Score, Total Symptom Score, Quality of Life Score and Social Limitation Score".
Because the manual states that the Overall Sum-mary Score should be calculated on available scores, then if physical limitation score is missing, but other sub scores are available the Overall Summary Score will be calculated anyhow based on other sub scores.

The KCCQ is a 23-item, self-administered ques-tionnaire developed to independently measure the participant's perception of their health status.
Scores range from 0 to 100, with 0 as lowest score and 100 as the highest score.
Higher scores indi-cate better health status, fewer symptoms, and greater disease-specific health-related quality of life.
The calculation is following the KCCQ scoring manual that states: "Overall Summary Score is defined as mean of the following available summary scores: Physical Limitation Score, Total Symptom Score, Quality of Life Score and Social Limitation Score".
Because the manual states that the Overall Summary Score should be calculated on available scores, then if physical limitation score is missing, but other sub scores are available the Overall Summary Score will be calculated anyhow based on other sub scores.
