Badanie oceniające skuteczność i bezpieczeństwo CDR132L u pacjentów ze zmniejszoną frakcją wyrzutową lewej komory po zawale mięśnia sercowego (HF-REVERT)
Faza 2, wieloośrodkowe, randomizowane, równoległe, 3-ramienne, kontrolowane placebo badanie mające na celu ocenę skuteczności i bezpieczeństwa CDR132L u pacjentów ze zmniejszoną frakcją wyrzutową lewej komory (≤ 45%) po zawale mięśnia sercowego
Jest to wieloośrodkowe, randomizowane, równoległe, 3-ramienne, kontrolowane placebo badanie fazy 2, mające na celu ocenę skuteczności i bezpieczeństwa CDR132L u pacjentów ze zmniejszoną frakcją wyrzutową lewej komory (LVEF) (≤ 45%) po zawale mięśnia sercowego (MI). Badanie to składa się z okresu przesiewowego (który ma miejsce co najmniej 3 dni po rozpoznaniu zawału mięśnia sercowego), 6-miesięcznego okresu podwójnie ślepej próby i 6-miesięcznego okresu przedłużenia z wizytą końcową badania (EOS) w dniu 360/miesiąc 12 .
Dwie dawki CDR132L zostaną przetestowane w porównaniu z placebo pod kątem ich wpływu na pacjentów, którzy właśnie przebyli zawał serca oprócz standardowej opieki. Celem badania jest wykazanie, że CDR132L jest bezpieczny i skuteczny w leczeniu niewydolności serca u takich pacjentów.

Opis
Główne kryteria włączenia:
- Pacjenci płci męskiej lub żeńskiej w wieku od ≥ 30 do ≤ 80 lat w dniu podpisania świadomej zgody, który jest określany jako początek okresu przesiewowego.
- Spontaniczny ostry zawał mięśnia sercowego (AMI) (typu I) w oparciu o uniwersalną definicję MI z randomizacją, która ma nastąpić nie później niż 14 dni po rozpoznaniu zdarzenia indeksowego.
- Pacjent z LVEF ≤ 45% mierzoną metodą ECHO po rozpoznaniu MI (STEMI lub NSTEMI).
- Można uwzględnić pacjenta z wcześniejszymi incydentami MI w historii.
- Pacjent o masie ciała ≤ 120 kg.
- Poziom N-końcowego pro peptydu natriuretycznego typu B ≥ 125 pg/ml i < 8000 pg/ml w badaniu przesiewowym.
- Pacjent ze STEMI/NSTEMI, który przeszedł przezskórną interwencję wieńcową z powodu tego zdarzenia.
Kryteria wyłączenia:
- Kobieta w wieku rozrodczym (WOCBP).
- Pacjent z HF pochodzenia innego niż niedokrwienny; np. zapalenie mięśnia sercowego, kardiomiopatia alkoholowa.
- Pacjent z IV klasą według New York Heart Association (NYHA) podczas badania przesiewowego lub randomizacji.
- Pacjent ma zaplanowaną interwencję kardiologiczną (dopuszczalny jest angiografia bez angioplastyki) lub inną planowaną operację po okresie przesiewowym.
- Pacjent ma ciężką wadę zastawkową serca.
- Pacjent ma ciśnienie skurczowe < 90 mmHg lub > 180 mmHg, ciśnienie rozkurczowe < 50 mmHg lub > 110 mmHg i/lub tętno < 50 lub > 100 uderzeń na minutę podczas badania przesiewowego lub randomizacji.
- Pacjent z oszacowaną szybkością filtracji kłębuszkowej < 30 ml/min/1,73 m2 lub na dializie.
- Pacjent z niewydolnością wątroby sklasyfikowaną w skali Child-Pugh B lub C.
- Pacjent ma historię choroby (chorób) wpływających na barierę krew-mózg, np. udar w ciągu 6 miesięcy lub stwardnienie rozsiane.
- U pacjenta występowały zaburzenia krzepnięcia w wywiadzie lub małopłytkowość (płytki krwi < 100 000/μl).
- Pacjent ma źle kontrolowaną cukrzycę, jak ustalił badacz.
- U pacjenta występowały lub występowały jakiekolwiek z następujących schorzeń serca: znane strukturalne nieprawidłowości serca wykraczające poza HF, zespół wydłużonego odstępu QT w wywiadzie rodzinnym, omdlenia sercowe lub nawracające omdlenia idiopatyczne.
- Wszelkie istotne klinicznie nieprawidłowości, według uznania badacza, rytmu, przewodzenia lub morfologii spoczynkowego EKG, które stanowią dodatkowe zagrożenie dla bezpieczeństwa pacjentów.
- Pacjent z aktywną infekcją „zespołu ciężkiej ostrej niewydolności oddechowej koronawirusem 2 (SARS-CoV-2)” potwierdzoną zgodnie z lokalnymi wytycznymi dotyczącymi badań podczas badań przesiewowych.
- Pacjentów nie należy włączać do badania, jeśli otrzymali jakąkolwiek zabronioną terapię w ciągu 3 miesięcy od badania przesiewowego.

CDR132L jest syntetycznym antysensownym oligonukleotydem (ASO) i selektywnym inhibitorem mikroRNA-132-3p (miR-132).
miR-132 w kardiomiocytach jest centralnym przełącznikiem wpływającym na ekspresję genów, które są zasadniczo zaangażowane w nieprzystosowaną przebudowę serca, transformację i patologiczny wzrost serca (hipertrofię), przyczyniając się do niekorzystnej przebudowy serca i niewydolności serca (HF).1-5
Nieprawidłowa ekspresja miR-132 w komórkach serca jest przyczynowo związana z przebudową serca i progresją HF.

The calculation is following the KCCQ scoring manual that states: "Overall Summary Score is defined as mean of the following available summary scores: Physical Limitation Score, Total Symptom Score, Quality of Life Score and Social Limitation Score".
Because the manual states that the Overall Sum-mary Score should be calculated on available scores, then if physical limitation score is missing, but other sub scores are available the Overall Summary Score will be calculated anyhow based on other sub scores.
