Étude de phase 2, multicentrique, randomisée, parallèle, à 3 bras, contrôlée par placebo pour évaluer l'efficacité et l'innocuité du CDR132L chez les patients présentant une fraction d'éjection ventriculaire gauche réduite (≤ 45 %) après un infarctus du myocarde

Il s'agit d'une étude de phase 2, multicentrique, randomisée, parallèle, à 3 bras et contrôlée par placebo visant à évaluer l'efficacité et l'innocuité du CDR132L chez les patients présentant une fraction d'éjection ventriculaire gauche (FEVG) réduite (≤ 45 %) après un infarctus du myocarde (IM). Cette étude consiste en une période de dépistage (qui doit avoir lieu au moins 3 jours après le diagnostic d'IM), une période en double aveugle de 6 mois et une période de prolongation de 6 mois avec la visite de fin d'étude (EOS) au jour 360/mois 12 .

Deux dosages de CDR132L seront testés contre placebo sur leurs effets sur des patients, qui viennent de faire une crise cardiaque en plus des soins standards. L'objectif de l'étude est de montrer que le CDR132L est sûr et efficace pour améliorer l'insuffisance cardiaque chez ces patients.

La description

Principaux critères d'inclusion :

  1. Patients de sexe masculin ou féminin, âgés de ≥ 30 à ≤ 80 ans à la date de signature du consentement éclairé défini comme le début de la période de dépistage.
  2. Infarctus aigu du mycarde (IAM) spontané (type I) selon la définition universelle de l'IM avec randomisation devant survenir au plus tard 14 jours après le diagnostic de l'événement index.
  3. Patient avec une FEVG ≤ 45 % mesurée par ECHO après un diagnostic d'IDM (STEMI ou NSTEMI).
  4. Les patients avec des antécédents d'IM peuvent être inclus.
  5. Patient avec un poids corporel ≤ 120 kg.
  6. Niveau de peptide natriurétique N-terminal pro de type B ≥ 125 pg/ml et < 8000 pg/ml lors du dépistage.
  7. Patient avec STEMI/NSTEMI ayant subi une intervention coronarienne percutanée pour cet événement.

Critère d'exclusion:

  1. Une femme en âge de procréer (WOCBP).
  2. Patient atteint d'IC ​​d'origine non ischémique ; par exemple, myocardite, cardiomyopathie alcoolique.
  3. Patient avec la classe IV de la New York Heart Association (NYHA) lors de la sélection ou de la randomisation.
  4. Le patient a une intervention cardiaque planifiée (une angiographie sans angioplastie est acceptable) ou toute autre intervention chirurgicale planifiée après la période de sélection.
  5. Le patient a une cardiopathie valvulaire sévère.
  6. Le patient a une TA systolique < 90 mmHg ou > 180 mmHg, une TA diastolique < 50 mmHg ou > 110 mmHg et/ou une fréquence cardiaque < 50 ou > 100 battements/minute lors du dépistage ou de la randomisation.
  7. Patient avec un débit de filtration glomérulaire estimé < 30 mL/min/1,73 m2 ou sous dialyse.
  8. Patient présentant une insuffisance hépatique classée Child-Pugh B ou C.
  9. Le patient a des antécédents médicaux de maladie (s) affectant la barrière hémato-encéphalique, par exemple, accident vasculaire cérébral dans les 6 mois ou sclérose en plaques.
  10. Le patient a des antécédents médicaux de troubles hémorragiques ou de thrombocytopénie (plaquettes < 100 000/μL).
  11. Le patient a un diabète mal contrôlé tel que déterminé par l'investigateur.
  12. Le patient a des antécédents ou la présence de l'une des affections cardiaques suivantes : anomalies cardiaques structurelles connues au-delà de l'insuffisance cardiaque, antécédents familiaux de syndrome du QT long, syncope cardiaque ou syncope idiopathique récurrente.
  13. Toute anomalie cliniquement significative, à la discrétion de l'investigateur, dans le rythme, la conduction ou la morphologie de l'ECG au repos qui pose un risque supplémentaire pour la sécurité des patients.
  14. Patient avec une infection active au « coronavirus du syndrome respiratoire aigu sévère 2 (SRAS-CoV-2) » confirmée conformément aux directives de test locales lors du dépistage.
  15. Le patient ne doit pas être inscrit à l'étude s'il a reçu un traitement interdit dans les 3 mois suivant le dépistage.

CDR132L est un oligonucléotide antisens synthétique (ASO) et un inhibiteur sélectif du microARN-132-3p (miR-132). miR-132 dans les cardiomyocytes est un interrupteur central affectant l'expression des gènes qui sont impliqués de manière cruciale dans le remodelage cardiaque inadapté, la transformation et la croissance cardiaque pathologique (hypertrophie), contribuant au remodelage cardiaque indésirable et à l'insuffisance cardiaque (IC).1-5 L'expression aberrante de miR-132 dans les cellules cardiaques est causalement associée au remodelage cardiaque et à la progression de l'IC.
