Tutkimus RSVpreF-rokotteen saamiseksi raskaana oleville HIV-tartunnan saaneille ja heidän vauvoilleen (MORISOT)
Kolmannen vaiheen, satunnaistettu, kaksoissokkoutettu, lumekontrolloitu koe hengitysteiden syntikaalisen viruksen (RSV) prefuusio F-alayksikkörokotteen turvallisuuden, siedettävyyden ja immunogeenisyyden arvioimiseksi raskaana oleville osallistujille, joilla on HIV ja heidän vauvoillaan
Tutkimuksen tarkoituksena on saada tietoa RSVpreF-rokotteen turvallisuudesta ja immuuniaktiivisuudesta. Sitä tutkitaan vauvoilla, jotka ovat syntyneet HIV-tartunnan saaneiden äitien kanssa. Näillä vauvoilla voi olla suurempi mahdollisuus sairastua tai kuolla RSV-infektion vuoksi. Respiratory Syncytial Virus (RSV) on yleinen virustyyppi (bakteeri), joka voi aiheuttaa vakavia sairauksia (hengitysteiden sairauksia), joissa tarvitaan lääkärin apua. Rokotteet auttavat kehoasi tuottamaan vasta-aineita, jotka auttavat taistelemaan sairauksia vastaan. Vasta-aineet ovat aineita, joita kehosi käyttää taistelemaan infektioita vastaan. Vasta-aineet voivat siirtyä lapseen äidin istukan kautta.
Tutkimuksessa tarkastellaan äitien ja heidän vauvojensa turvallisuutta, siedettävyyttä ja immuunitoimintaa.
Tämä tutkimus etsii raskaana olevia naisia, jotka ovat:
- Alle 49-vuotias ja sinulla on HIV (ihmisen immuunikatovirus -
- Normaalin lääketieteellisen hoidon saaminen raskauden aikana
- Sinulla ei ole kuppaa (bakteeriperäinen sukupuolitauti), hepatiitti B -virusta ((HBV) maksatulehdus), tuberkuloosia ((TB) bakteeriperäinen keuhkosairaus).
- olet saanut vakaata (antiretroviraalista) HIV-hoitoa vähintään 90 päivää.
- suostuvat olemaan läsnä kaikilla opintokäynneillä, toimenpiteillä ja verikokeilla.
Osallistujat saavat joko:
- RSVpreF-rokote
- Plasebo. Plasebossa ei ole lääkettä, mutta se näyttää aivan tutkimusrokotteelta.
Raskaana olevat osallistujat ovat mukana tutkimuksessa:
- suostumus nykyisen raskautensa aikana ja
- 6 kuukauden ajan vauvan synnytyksen jälkeen (yhteensä noin 10 kuukautta). Raskaana oleville osallistujille on suunniteltu vähintään 5 käyntiä tässä tutkimuksessa. Osallistujat pikkulapsille: Kaikkia ilmoittautuneille äideille syntyneitä vauvoja seurataan syntymästä alkaen enintään 6 kuukauden ajan. Pikkulapsille osallistujat saavat vähintään 3 opintokäyntiä, joista osa saa tapahtua kotikäynneillä tai puhelimitse.

Keskeiset osallistumiskriteerit - Äiti osallistujat
- ≤49-vuotiaat naiset, jotka ovat 24 0/7 - 36 0/7 raskausviikkoa suunnitellun rokotuspäivänä ja joilla on komplisoitumaton yksittäisraskaus ja joilla ei tiedetä olevan lisääntynyttä komplikaatioriskiä.
- Vahvistettu vakaa HIV-tauti.
- Nykyinen ja vakaa antiretroviraalisen hoidon (ART) käyttö vähintään 90 päivää ennen ilmoittautumista.
- Sinulle tehtiin sikiön poikkeavuuden ultraäänitutkimus ≥18 raskausviikolla, eikä merkittäviä sikiön poikkeavuuksia havaittu.
- Aikomus synnyttää sairaalassa tai synnytyslaitoksessa, josta voi saada tutkimusmenetelmiä.
- Osallistuja on valmis antamaan tietoisen suostumuksen osallistujan vauvan osallistumiselle tutkimukseen.
- Pystyy antamaan allekirjoitetun tietoon perustuvan suostumuksen, joka sisältää tietoisen suostumuksen asiakirjassa (ICD) ja tässä pöytäkirjassa lueteltujen vaatimusten ja rajoitusten noudattamisen.

Tärkeimmät poissulkemiskriteerit - Äiti osallistujat
- Raskausajan painoindeksi (BMI) > 40 kg/m2. Jos raskaana olevaa BMI:tä ei ole saatavilla, voidaan käyttää nykyisen raskauden ensimmäisen synnytyskäynnin BMI:tä.
- Osallistuja, jolla on opportunistisia infektioita tai pahanlaatuisia kasvaimia.
- Aiemmin aktiivinen krooninen virushepatiitti, jossa on biokemiallisia todisteita aspartaattiaminotransferaasin (AST) tai alaniiniaminotransferaasin (ALT) arvoista > 5 kertaa normaalin yläraja 6 kuukauden sisällä ennen ilmoittautumista.
- Aiemmin rokotteeseen liittyvä vakava haittavaikutus ja/tai vakava allerginen reaktio (esim. anafylaksia) jollekin tutkimustoimenpiteen osalle tai mihin tahansa siihen liittyvään rokotteeseen.
- Nykyinen raskaus koeputkihedelmöityksestä. Osallistujat, joiden tiedetään käyttäneen klomifeenisitraattia ja/tai letrotsolia kohdunsisäisen keinohedelmöityksen (IUI) kanssa tai ilman, ovat sallittuja.
- Nykyiset raskauden komplikaatiot tai poikkeavuudet suostumushetkellä, jotka lisäävät tutkimukseen osallistumiseen ja tutkimukseen osallistumiseen liittyvää riskiä, mukaan lukien, mutta ei rajoittuen, seuraavat:
- Preeklampsia, eklampsia tai hallitsematon raskausajan hypertensio.
- Istukan poikkeavuus.
- Polyhydramnion tai oligohydramnion.
- Merkittävä verenvuoto tai veren hyytymishäiriö.
- Endokriiniset häiriöt, mukaan lukien hoitamaton kilpirauhasen liikatoiminta tai hoitamaton kilpirauhasen vajaatoiminta. - Aiemmat raskauden komplikaatiot tai poikkeavuudet suostumuksen myöntämishetkellä, tutkijan arvion perusteella, jotka lisäävät tutkimukseen osallistumiseen ja sen loppuun saattamiseen liittyvää riskiä, mukaan lukien, mutta ei rajoittuen, seuraavat:
- Ennenaikainen synnytys ≤34 raskausviikolla
- Aikaisempi kuolleena syntymä tai vastasyntyneen kuolema
- Edellinen vauva, jolla on tunnettu geneettinen häiriö tai merkittävä synnynnäinen poikkeavuus
- Ei-HIV-infektioon liittyvä synnynnäinen tai hankittu immuunipuutoshäiriö tai reumatologinen häiriö tai muu sairaus, joka vaatii kroonista hoitoa tunnetuilla immunosuppressiivisilla lääkkeillä.
- Tuberkuloosihoidon käyttö tällä hetkellä tai milloin tahansa tämän raskauden aikana.

Percentage of Maternal Participants With Prespecified Local Reactions Within 7 Days After Vaccination
Aikaikkuna: From Day 1 to Day 7 after vaccination
|
Local reactions included redness, swelling, and pain at injection site and were recorded in electronic diary (e-diary).
Redness and swelling were measured by the maternal participant and recorded in measuring device units (mdu) in the e-diary (range: 1 to 21). 1 mdu = 0.5 centimetre (cm).
Redness and swelling were graded as mild (>2.0 to 5.0 cm), moderate (>5.0 to 10.0 cm), severe (>10.0 cm) and grade 4 (necrosis or exfoliative dermatitis for redness and necrosis for swelling).
Pain at injection site was graded as mild (did not interfere with activity), moderate (interfered with activity), severe (prevented daily activity) and grade 4 (emergency room visit or hospitalization for severe pain at the injection site).
Grade 4 reactions were classified by the investigator or medically qualified designee.

Systemic events included fever, fatigue, headache, nausea, muscle pain, joint pain, vomiting and diarrhea and were recorded by participants in the e-diary.
Fever was defined as an oral temperature greater than or equal to (>=) 38.0 degree Celsius (C) and classified as mild (38.0 to 38.4 degree C), moderate (38.5 to 38.9 degree C), severe (39.0 to 40.0 degree C) and grade 4 (>40.0
degree C).
Headache, nausea, fatigue, muscle pain and joint pain were graded as mild (did not interfere with activity), moderate (some interference with activity), severe (prevented daily activity).
Vomiting: mild (1-2 times in 24 hours [H]), moderate (>2 times in 24 H), severe (required intravenous [IV] hydration).
Diarrhea: mild (2-3 loose stools in 24 H), moderate (4-5 loose stools in 24 H), severe (>=6 loose stools in 24 H).
For all systemic events except fever, Grade 4= emergency room visit or hospitalization.
Grade 4 events were classified by investigator or medically qualified designee.

AESIs were based on targeted medical events associated with pregnant maternal participants prior to/during delivery and post delivery.
For maternal participants, the following were considered as protocol defined AESIs: diagnosis of Guillain-Barré syndrome; diagnosis of acute polyneuropathy without an underlying etiology; hypertensive disorders of pregnancy; preterm delivery (delivery at <37 0/7 weeks' gestation) and atrial fibrillation.
AESIs was to be recorded as an AE or serious adverse event (SAE) on the case report form (CRF).

Postnatal standard of care procedures in South Africa were as follows: healthy newborns and their mothers were typically discharged within 24 hours after an uncomplicated vaginal delivery, or within 72 hours following an uncomplicated Caesarean section.
These practices were supported by the Guidelines for Maternity Care in South Africa.
