Yksityiskohtainen kuvaus
Tämä kliininen tutkimus on monikeskuksellinen, vertaileva, satunnaistettu, kontrolloitu, sokkoutettu (potilas ja ensisijainen kriteerien arvioija) paremmuustutkimus, jossa on 2 rinnakkaista ryhmää (Interventioryhmä: Algoritmi-ohjattu haara; Kontrolliryhmä: standardihoidon haara).
PD-potilaat, joita hoidetaan DA:lla osallistumishetkellä, satunnaistetaan (1:1-suhde) joko standardihoidon (SoC) haaraan tai algoritmi-ohjattuun haaraan. Heidät rekrytoidaan NSPARK/FCRIN-verkoston PD-asiantuntijakeskuksissa. Ensisijaisena tavoitteena on arvioida ICD SHIELD -sovelluksen, ohjelmiston, jossa on tietokonepohjainen algoritmi, joka auttaa kliinikkoja dopaminergisten lääkkeiden määräämisessä, tehokkuutta verrattuna standardihoitoon ensisijaisessa päätepisteessä. Osallistumisen jälkeen (V0 M0:ssa) suoritetaan neljä seurantakäyntiä: V1 M6:ssa, V2 M12:ssa, V3 M18:ssa ja V4 M24:ssä avohoidon aikana, jolloin osallistujia yleensä seurataan. Jokaisella seurantakäynnillä (M6:sta M24:ään) neurologi kirjaa lääketieteellisen ja hoidon historian, suorittaa neurologisen tutkimuksen MDS-UPDRS-osat III–IV ja CGI-I-asteikon, ja tarkistaa MDS-UPDRS-osat I ja II mahdollista uudelleenarviointia/täsmennystä varten. PGI-asteikko, PDQ39-kysely, MDS-UPDRS-osat I ja II ja HAD-asteikot täytetään potilaan toimesta. MoCA-asteikko, ASBPD ja QUIP-RS suoritetaan neuropsykologin tai koulutetun tutkijan toimesta kunkin asteikon mukaisesti pisteytysohjeiden mukaan ja sokkona hoitoryhmän allokaatiosta. Sitten neurologi mukauttaa hoitoa algoritmin tuloksen suosituksen tai kliinikon yksinomaiseen suositukseen perustuen siihen, mihin haaraan potilas on satunnaistettu.
Lisäpuhelu tehdään 3 kuukautta edellisen käynnin jälkeen etätarkistukseen varmistaakseen, että potilas on noudattanut asianmukaisesti reseptimuutosta (DA:n vähentäminen tai lopettaminen), jos säätöä käytettiin lopettamaan kokonaan tai vähentämään DA annokseksi, joka vastaa 40 mg levodopaa.
Valinnainen verinäyte otetaan perustasolla tai seurantakäynnillä ja lähetetään DNA-eriottamista ja biopankkointia varten ICM:lle, Pitié-Salpêtrière-sairaalaan, Pariisiin. Rekrytoinnin kesto on 2 vuotta. Jokaisen osallistujan osallistumisen kesto on 2 vuotta, ja tutkimuksen kokonaiskesto on 4 vuotta.
Pääanalyysi tehdään aikeiden mukaan (ITT) ja sisältää sekoitetun yleistetyn lineaarisen mallin (GLMM) logit-linkillä, joka on säädetty minimointikerrostratifikointitekijöillä (keskus, sukupuoli, ikä, levodopan päiväannosekvivalentti (LEDD) osallistumishetkellä).

- Miehet ja naiset ≥ 18 vuotta vanhoja
- PD-diagnoosi 2015 Movement Disorders Society -kriteerien (Postuma et al., Mov Disord. 2015) mukaan, bradykinesian JA vähintään YHDEN seuraavista: lihasjäykkyys tai lepotremori; ilman muita epäiltyjä parkinsonismin syitä
- Sairauden kesto alle 6 vuotta mukaan lukien
- Ei meneillään olevia kliinisesti merkittäviä (lievää tai korkeampaa) ICDRB:itä (ASBPD osa IV alaosuudet kukin <2)
- Potilaat, joilla on vähintään 2 kuukauden DA-hoito ja ilman suunniteltua tai tunnettua syytä DA-hoidon lopettamiseen seuraavien 2 vuoden aikana
Poissulkemiskriteerit:
Epätyypillinen tai sekundaarinen parkinsonismi, kuten supranukleaarinen halvaus, monijärjestelmäatrofia tai lääkeaineiden aiheuttama parkinsonismi jne...
- Potilaat, joilla on kognitiivinen tai psyykkinen häiriö, joka estää potilaan osallistumisen tutkijan arvion mukaan
- Ei halua osallistua kliiniseen tutkimukseen tai allekirjoittaa suostumusta
- Raskaana oleva tai imettävä nainen, tai WOCBP, jonka <seerumi- tai virtsa->raskaustesti on positiivinen

Algoritmiohjatuessa ryhmässä potilaan hoitoa sopeutetaan neurologin toimesta algoritmin tulosteen perusteella.

Potilaan arvioinnin ja ICD SHIELD -sovellukseen syötettyjen kliinisten tietojen, mukaan lukien neurologin suunnitteleman reseptivalinnan seuraavalle jaksolle, jälkeen kliinikko saa ICD SHIELD -sovelluksen suositteleman hoitomenetelmän algoritmin antaman tuloksen perusteella.
Kliinikko voi toistaa sovelluksen käytön, jos hän suunnittelee kokeilevansa erilaisia reseptivalintoja sovelluksessa tarvittaessa, mutta kertakäyttöä suositellaan jokaisella käynnillä.
Neurologin tulee noudattaa ICD SHIELD -sovelluksen suositusta mahdollisimman paljon, ellei sitä pidetä sopimattomana.
Neurologi tekee lopullisen päätöksen.
