Dit klinisch onderzoek is een multicenter, vergelijkend, gerandomiseerd, gecontroleerd, gemaskeerd (patiënt en primaire criteria-beoordelaar), superioriteitsonderzoek met 2 parallelle groepen (interventiegroep: algoritme-gestuurde arm; controlegroep: standaardzorg-arm).
PD-patiënten, behandeld met DA bij inclusie, worden gerandomiseerd (1:1-ratio) naar de standaardzorg-arm (SoC) of de algoritme-gestuurde arm. Zij worden geworven in PD-expertcentra van het NSPARK/FCRIN-netwerk. Het primaire doel is om de werkzaamheid te beoordelen van de ICD SHIELD-app, een software met een computergebaseerd algoritme dat clinici ondersteunt bij het voorschrijven van dopaminerge medicatie, vergeleken met standaardzorg, op het primaire eindpunt. Na de inclusie (V0 bij M0) worden vier vervolgbezoeken uitgevoerd: V1 bij M6, V2 bij M12, V3 bij M18 en V4 bij M24 tijdens poliklinische bezoeken waar deelnemers gewoonlijk worden gevolgd. Bij elk vervolgbezoek (M6 tot M24) noteert de neuroloog de medische en behandelingsgeschiedenis, voert een neurologisch onderzoek uit (MDS-UPDRS secties III tot IV en CGI-I-schaal) en beoordeelt de MDS-UPDRS secties I en II voor mogelijke herbeoordeling/verduidelijking. De PGI-schaal, PDQ39-vragenlijst, MDS-UPDRS secties I en II, en de HAD-schalen worden door de patiënt ingevuld. De MoCA-schaal, ASBPD en QUIP-RS worden uitgevoerd door een neuropsycholoog, of een onderzoeker die voor elke schaal is getraind, volgens scoringsinstructies en geblindeerd voor behandelingstoewijzing. Vervolgens wordt de behandeling door de neuroloog aangepast, volgens de aanbeveling van de algoritme-uitkomst of de klinische aanbeveling alleen, afhankelijk van de arm waarin de patiënt is gerandomiseerd.
Een extra telefoongesprek wordt 3 maanden na het vorige bezoek uitgevoerd voor een controle op afstand om te bevestigen dat de voorschrijfwijziging (verminderen of stoppen van DA) correct is opgevolgd door de patiënt, indien de aanpassing om DA volledig te stoppen of te verminderen tot een dosis equivalent aan 40mg Levodopa is toegepast.
Een optionele bloedmonster wordt afgenomen bij baseline of tijdens een vervolgbezoek en verzonden voor DNA-extractie en biobanking bij het ICM, Pitié-Salpêtrière Ziekenhuis, Parijs. De wervingsduur bedraagt 2 jaar. De deelnameduur voor elke deelnemer bedraagt 2 jaar, de totale studieduur bedraagt 4 jaar.
De hoofdanalyse zal volgens intention-to-treat worden uitgevoerd en omvat een gemengd gegeneraliseerd lineair model (GLMM) met een logit-link, gecorrigeerd voor minimalisatiestratificatiefactoren (centrum, geslacht, leeftijd, Levodopa Equivalent Dagelijkse Dosis (LEDD) bij inclusie).

Inclusiecriteria:
- Mannen en vrouwen ≥ 18 jaar oud
- Diagnose van PD volgens de criteria van de Movement Disorders Society uit 2015 (Postuma et al., Mov Disord. 2015), met bradykinesie EN minstens ÉÉN van de volgende: spierstijfheid, of rusttremor; zonder andere vermoedelijke oorzaak van parkinsonisme
- Ziekteduur minder dan 6 jaar, inclusief
- Geen aanhoudende klinisch significante (Mild of hoger) ICDRBs (ASBPD deel IV subscores elk <2)
- Patiënten die momenteel minimaal 2 maanden met DA worden behandeld en zonder huidige geplande of bekende reden om DA te stoppen gedurende de komende 2 jaar
Uitsluitingscriteria:
Atypisch of secundair parkinsonisme zoals supranucleaire paralyse, multi-systeematrofie of geneesmiddel-geïnduceerd parkinsonisme, etc...
- Patiënten met een cognitieve of psychiatrische stoornis die deelname van de patiënt verhindert volgens het oordeel van de onderzoeker
- Niet bereid om deel te nemen aan het klinisch onderzoek of om de toestemmingsverklaring te ondertekenen
- Zwangere of zogende vrouw, of WOCBP met een positieve zwangerschapstest in <serum of urine>
- Deelname aan onderzoeken met experimentele geneesmiddelen binnen 30 dagen vóór screening of binnen 5 halfwaardetijden van het experimentele product, afhankelijk van wat het langst is
- Deelnemer niet aangesloten bij of begunstigde van een Frans sociaal zekerheidsstelsel

Experimenteel: Algoritme-gestuurde groep
In de (algoritme-gestuurde arm) zal de behandeling van de patiënt door de neuroloog worden aangepast op basis van de algoritme-uitvoer.

Na evaluatie van de patiënt en de klinische invoer in de ICD SHIELD app, inclusief de geplande keuze van voorschrift door de neuroloog voor de volgende periode, ontvangt de clinicus de therapeutische aanbeveling van de ICD SHIELD app, afhankelijk van de output van het algoritme.
De clinicus kan het gebruik van de app herhalen als hij/zij verschillende keuzes van voorschrift in de app wil proberen indien nodig geacht, maar een enkel gebruik wordt aanbevolen per bezoek.
De neuroloog zal de aanbeveling van de ICD SHIELD app zoveel mogelijk moeten volgen, tenzij deze als ongeschikt wordt beoordeeld.
De neuroloog neemt de uiteindelijke beslissing.
