Eficácia de um Algoritmo baseado em Modelo de Previsão para PREVENIR Perturbações de Controlo de Impulsos Induzidas por Medicamentos na Doença de Parkinson (PREVENT-ICD)

Esta Investigação Clínica é um ensaio multicêntrico, comparativo, randomizado, controlado, mascarado (paciente e avaliador dos critérios primários), de superioridade, com 2 grupos paralelos (Grupo de Intervenção: braço guiado por algoritmo; Grupo de Controlo: braço de tratamento padrão).
Os doentes com DP, tratados com DA na inclusão, serão randomizados (proporção 1:1) para o braço de tratamento padrão (SoC) ou para o braço guiado por algoritmo. Serão recrutados em centros especializados em DP da rede NSPARK/FCRIN. O objetivo principal é avaliar a eficácia da aplicação ICD SHIELD, um software com um algoritmo baseado em computador que auxilia os clínicos na prescrição de medicamentos dopaminérgicos, em comparação com o tratamento padrão, no endpoint primário. Após a inclusão (V0 em M0), serão realizadas quatro visitas de seguimento: V1 em M6, V2 em M12, V3 em M18 e V4 em M24, durante consultas externas onde os participantes são habitualmente acompanhados. Em cada visita de seguimento (M6 a M24), o neurologista registará o historial médico e de tratamento, realizará o exame neurológico das secções III a IV da MDS-UPDRS e a escala CGI-I, e reverá as secções I e II da MDS-UPDRS para possível reavaliação/esclarecimento. A escala PGI, o questionário PDQ39, as secções I e II da MDS-UPDRS e as escalas HAD serão preenchidos pelo paciente. A escala MoCA, a ASBPD e a QUIP-RS serão realizadas por um neuropsicólogo, ou por um investigador treinado para cada escala, de acordo com as instruções de pontuação e cego à atribuição do braço de tratamento. Em seguida, o tratamento será adaptado pelo neurologista, de acordo com a recomendação do resultado do algoritmo ou com a recomendação exclusiva do clínico, dependendo do braço em que o paciente foi randomizado.
Será realizada uma chamada telefónica adicional 3 meses após a visita anterior para um checkup remoto, para confirmar que a alteração da prescrição (redução ou interrupção do DA) foi corretamente seguida pelo paciente, se o ajuste para parar completamente ou reduzir o DA para uma dose equivalente a 40 mg de Levodopa foi aplicado.
Uma amostra de sangue opcional será colhida na linha de base ou numa visita de seguimento e enviada para extração de ADN e biobanco no ICM, Hospital Pitié-Salpêtrière, Paris. A duração do recrutamento será de 2 anos. A duração da participação de cada participante será de 2 anos, e a duração total do estudo será de 4 anos.
A análise principal será em Intenção de Tratar e envolverá um modelo linear generalizado misto (GLMM) com ligação logit ajustado para fatores de estratificação por minimização (centro, sexo, idade, Dose Diária Equivalente de Levodopa (LEDD) na inclusão).

Critérios de Inclusão:
- Homens e mulheres ≥ 18 anos
- Diagnóstico de DP de acordo com os critérios da Sociedade de Distúrbios do Movimento de 2015 (Postuma et al., Mov Disord. 2015), com bradicinesia E pelo menos UM dos seguintes: rigidez muscular ou tremor em repouso; sem outra causa suspeita de parkinsonismo
- Duração da doença inferior a 6 anos incluída
- Sem ICDRBs clinicamente significativos em curso (Leves ou superiores) (pontuações ASBPD parte IV cada <2)
- Doentes atualmente tratados com DA há pelo menos 2 meses e sem razão planeada ou conhecida para interromper o DA nos próximos 2 anos
Critérios de Exclusão:
Parkinsonismo atípico ou secundário, como paralisia supranuclear progressiva, atrofia de múltiplos sistemas ou parkinsonismo induzido por medicamentos, etc...
- Doentes com um distúrbio cognitivo ou psiquiátrico que impeça a participação do doente de acordo com o julgamento do investigador
- Não disposto a participar na Investigação Clínica ou a assinar o consentimento
- Mulher grávida ou a amamentar, ou MCFE com teste de gravidez <soro ou urina> positivo
- Participação em ensaios de medicamentos experimentais nos 30 dias anteriores ao rastreio ou dentro de 5 meias-vidas do produto experimental, o que for mais longo
- Participante não afiliado ou beneficiário de um sistema de segurança social francês

Experimental: Grupo orientado por algoritmo

Após a avaliação do doente e dos dados clínicos inseridos na aplicação ICD SHIELD, incluindo a escolha planeada da prescrição pelo neurologista para o próximo período, o clínico receberá a abordagem terapêutica recomendada pela aplicação ICD SHIELD, dependendo do resultado fornecido pelo algoritmo.
O clínico pode repetir a utilização da aplicação se planear experimentar várias escolhas de prescrição na aplicação, se considerar necessário, mas recomenda-se uma única utilização em cada consulta.
O neurologista terá de seguir a recomendação da aplicação ICD SHIELD tanto quanto possível, a menos que seja considerada inadequada.
O neurologista toma a decisão final.
