Esta Investigación Clínica es un ensayo multicéntrico comparativo aleatorizado, controlado, enmascarado (paciente y evaluador primario de criterios), de superioridad, con 2 grupos paralelos (Grupo de Intervención: brazo guiado por algoritmo; Grupo de Control: brazo de atención estándar).
Los pacientes con EP, tratados con DA en la inclusión, serán aleatorizados (proporción 1:1) al brazo de atención estándar (SoC) o al brazo guiado por algoritmo. Serán reclutados en centros expertos en EP de la red NSPARK/FCRIN. El objetivo principal es evaluar la eficacia de la aplicación ICD SHIELD, un software con un algoritmo informático que asiste a los clínicos en la prescripción de medicamentos dopaminérgicos, en comparación con la atención estándar, en el criterio de valoración principal. Después de la inclusión (V0 en M0), se realizarán cuatro visitas de seguimiento: V1 en M6, V2 en M12, V3 en M18 y V4 en M24 durante las consultas externas donde los participantes suelen ser seguidos. En cada visita de seguimiento (M6 a M24), el neurólogo registrará la historia médica y de tratamiento, realizará el examen neurológico de las secciones III a IV de la MDS-UPDRS y la escala CGI-I, y revisará las secciones I y II de la MDS-UPDRS para una posible reevaluación/aclaración. La escala PGI, el cuestionario PDQ39, las secciones I y II de la MDS-UPDRS y las escalas HAD serán completados por el paciente. La escala MoCA, la ASBPD y la QUIP-RS serán realizadas por un neuropsicólogo, o un investigador entrenado para cada escala, según las instrucciones de puntuación y ciego a la asignación del brazo de tratamiento. Luego, el tratamiento será adaptado por el neurólogo, según la recomendación del resultado del algoritmo o la recomendación única del clínico, dependiendo del brazo en el que el paciente esté aleatorizado.
Se realizará una llamada telefónica adicional 3 meses después de la visita anterior para un chequeo remoto, para confirmar que el cambio de prescripción (disminución o suspensión de DA) ha sido seguido adecuadamente por el paciente, si se aplicó el ajuste para detener completamente o disminuir la DA a una dosis equivalente a 40 mg de Levodopa.
Se tomará una muestra de sangre opcional en el basal o en una visita de seguimiento y se enviará para extracción de ADN y biobanco en el ICM, Hospital Pitié-Salpêtrière, París. La duración del reclutamiento será de 2 años. La duración de la participación para cada participante será de 2 años, la duración total del estudio será de 4 años.
El análisis principal será por intención de tratar e involucrará un modelo lineal generalizado mixto (GLMM) con un enlace logit ajustado por factores de estratificación de minimización (centro, sexo, edad, Dosis Diaria Equivalente de Levodopa (LEDD) en la inclusión).

Criterios de inclusión:
- Hombres y mujeres ≥ 18 años
- Diagnóstico de EP según los criterios de la Sociedad de Trastornos del Movimiento de 2015 (Postuma et al., Mov Disord. 2015), con bradicinesia Y al menos UNO de los siguientes: rigidez muscular o temblor en reposo; sin otra causa sospechosa de parkinsonismo
- Duración de la enfermedad inferior a 6 años incluida
- Sin ICDRBs clínicamente significativos (leves o superiores) en curso (puntuaciones parciales de ASBPD parte IV cada una <2)
- Pacientes tratados actualmente con DA durante al menos 2 meses y sin motivo planificado o conocido para interrumpir el DA en los próximos 2 años
Criterios de exclusión:
Parkinsonismo atípico o secundario como parálisis supranuclear progresiva, atrofia multisistémica o parkinsonismo inducido por fármacos, etc.
- Pacientes con un trastorno cognitivo o psiquiátrico que impida la participación del paciente según el criterio del investigador
- No dispuesto a participar en la investigación clínica o a firmar el consentimiento
- Mujer embarazada o en período de lactancia, o WOCBP con prueba de embarazo en <suero u orina> positiva
- Participación en ensayos con fármacos en investigación dentro de los 30 días previos al cribado o dentro de las 5 vidas medias del producto en investigación, lo que sea más largo
- Participante no afiliado o beneficiario de un sistema de seguridad social francés

En el (brazo guiado por algoritmo) el tratamiento del paciente será adaptado por el neurólogo en función de la salida del algoritmo.

Tras la evaluación del paciente y los datos clínicos introducidos en la aplicación ICD SHIELD, incluida la elección planificada de prescripción por parte del neurólogo para el próximo periodo, el clínico recibirá el enfoque terapéutico recomendado por la aplicación ICD SHIELD según el resultado proporcionado por el algoritmo.
El clínico puede repetir el uso de la aplicación si planea probar varias opciones de prescripción en la aplicación si se considera necesario, pero se recomienda un uso único en cada visita.
El neurólogo deberá seguir la recomendación de la aplicación ICD SHIELD en la medida de lo posible, a menos que se considere inapropiada.
El neurólogo toma la decisión final.
