ARRIÈRE-PLAN:
Malgré l'utilisation généralisée de la digitaline et sa disponibilité depuis près de deux siècles, une incertitude entoure la pertinence de son rôle et de sa valeur dans le traitement des patients insuffisants cardiaques congestifs en rythme sinusal. L'étude était un effort de collaboration multicentrique avec le programme d'études coopératives du ministère des Anciens Combattants qui a fourni un soutien à un centre de coordination des données et à un centre de coordination de la pharmacie.
NARRATIF DE CONCEPTION :
Essai international randomisé, en double aveugle, simple, multicentrique avec 186 centres aux États-Unis et 116 au Canada. Dans l'essai principal, les patients avec des fractions d'éjection ventriculaire gauche de 0,45 ou moins ont été randomisés pour recevoir de la digoxine (3 397 patients) ou un placebo (3 403 patients) en plus des diurétiques et des inhibiteurs de l'ECA. Dans un essai auxiliaire de patients avec des fractions d'éjection supérieures à 0,45, 492 patients ont été assignés au hasard à la digoxine et 496 au placebo. Les patients ont été recrutés sur une période de trois ans dans les 302 centres et suivis pendant au moins deux ans. Le recrutement des patients a commencé en février 1991 et s'est terminé en septembre 1993. Le suivi s'est terminé en décembre 1995. Le document principal sur les résultats a été publié en 1997.
Trois sous-études ont été menées. La sous-étude de qualité de vie/test de marche de 6 minutes a déterminé l'effet du traitement sur le bien-être, les activités quotidiennes et l'état fonctionnel d'un patient. La sous-étude d'électrocardiogramme Holter/signal moyen a examiné la physiopathologie de la mort cardiaque subite. La sous-étude neurohormonale a déterminé si l'administration à long terme de digoxine atténuait la réponse neuroendocrinienne chez les patients souffrant d'insuffisance cardiaque.
La date d'achèvement de l'étude indiquée dans ce dossier a été obtenue à partir de la « date d'achèvement » saisie dans le système de rapport de vue de requête (QVR).
