Essai sur l'angioplastie guidée par la viabilité après un infarctus aigu du myocarde (essai VIAMI)

Des études antérieures ont montré que l'angioplastie de routine de l'artère coronaire liée à l'infarctus peu de temps après la thrombolyse pour un infarctus aigu du myocarde n'a pas entraîné de bénéfice clinique. Cette constatation souligne l'importance de sélectionner un sous-groupe de patients ayant récemment subi un infarctus du myocarde, qui bénéficieront réellement de l'angioplastie de l'artère liée à l'infarctus.
Plusieurs études ont démontré que la présence d'une viabilité dans la zone de l'infarctus tôt après un infarctus aigu du myocarde impose un risque accru de nouveaux événements cardiaques comme l'infarctus récurrent, l'angine de poitrine et la nécessité d'interventions coronariennes. Par conséquent, il est postulé que seuls les patients viables dans la zone de l'infarctus bénéficieront d'une angioplastie tôt après un infarctus aigu du myocarde.
Dans l'essai VIAMI, les patients admis à l'hôpital avec un infarctus aigu du myocarde et qui n'ont pas subi d'angioplastie coronarienne immédiate subiront un test de viabilité par échocardiographie à la dobutamine dans les 3 jours suivant leur admission. Les patients présentant des signes non équivoques de viabilité dans la zone de l'infarctus seront ensuite randomisés vers une stratégie de traitement invasive ou conservatrice. Dans la stratégie invasive, les patients subiront une angiographie coronarienne avec l'intention d'effectuer une angioplastie par ballonnet avec stenting de l'artère coronaire liée à l'infarctus, avec l'utilisation concomitante de l'abciximab, un inhibiteur plaquettaire intraveineux. Dans le groupe conservateur, les patients subiront une angiographie coronarienne et une angioplastie uniquement en présence d'un nouvel infarctus imminent ou d'une ischémie récurrente. Dans le groupe invasif, l'angioplastie par ballonnet sera réalisée dès que possible après la randomisation. Les patients sans viabilité ne seront pas randomisés, mais participeront en tant que groupe témoin.

Critère d'intégration:
- Admission dans les 6 heures suivant l'apparition de la douleur et ECG avec ≥ 1 mm de sus-décalage du segment ST dans deux dérivations standard ou plus ou ≥ 2 mm de sus-décalage du segment ST dans deux dérivations thoraciques contiguës, indiquant une ischémie transmurale, avec ou sans développement de nouveaux Q- vagues, avec nécrose myocardique prouvée par élévation enzymatique (CPK totale ≥ 2x la limite supérieure de la normale).
- Antécédents du patient compatibles avec un infarctus subaigu du myocarde (≥ 6 heures), avec au moins des signes de nécrose myocardique à l'ECG (ondes Q) et une augmentation significative du taux sérique d'enzymes spécifiques cardiaques.
- Âge compris entre 18 et 80 ans.
- Test de viabilité effectué avant la coronarographie.
- Aucune indication clinique pour une procédure invasive à effectuer avant le test de viabilité
Critère d'exclusion:
- Aucun consentement éclairé obtenu
- Suivi peu fiable
- Test de viabilité techniquement impossible
- Contre-indications à l'échocardiographie à la dobutamine (insuffisance cardiaque, arythmie)
- Contre-indications à la coronarographie, telles que néphropathie diabétique sévère ou allergie au contraste connue
- Hypersensibilité connue à l'abciximab
- Maladie non cardiaque grave, potentiellement mortelle
