Die lebensfähigkeitsgesteuerte Angioplastie nach akutem Myokardinfarkt-Versuch (der VIAMI-Versuch)

Detaillierte Beschreibung
Frühere Studien haben gezeigt, dass eine routinemäßige Angioplastie der infarktbedingten Koronararterie kurz nach der Thrombolyse bei akutem Myokardinfarkt keinen klinischen Nutzen brachte. Dieser Befund unterstreicht, wie wichtig es ist, eine Untergruppe von Patienten mit einem kürzlich erlittenen Myokardinfarkt auszuwählen, die wirklich von einer Angioplastie der infarktbezogenen Arterie profitieren.
Mehrere Studien haben gezeigt, dass das Vorhandensein von Lebensfähigkeit im Infarktbereich früh nach einem akuten Myokardinfarkt ein erhöhtes Risiko für neue kardiale Ereignisse wie wiederkehrende Infarkte, Angina pectoris und die Notwendigkeit von Koronarinterventionen mit sich bringt. Daher wird postuliert, dass nur Patienten mit einer Lebensfähigkeit im Infarktbereich von einer Angioplastie früh nach einem akuten Myokardinfarkt profitieren.
In der VIAMI-Studie werden Patienten, die mit einem akuten Myokardinfarkt ins Krankenhaus eingeliefert werden und sich keiner sofortigen Koronarangioplastie unterzogen haben, innerhalb von 3 Tagen nach der Aufnahme einem Lebensfähigkeitstest mittels Dobutamin-Echokardiographie unterzogen. Patienten mit eindeutigen Anzeichen einer Lebensfähigkeit im Infarktgebiet werden dann randomisiert einer invasiven oder einer konservativen Behandlungsstrategie zugeteilt. Bei der invasiven Strategie werden die Patienten einer Koronarangiographie unterzogen, mit der Absicht, eine Ballonangioplastie mit Stenting der infarktbedingten Koronararterie durchzuführen, bei gleichzeitiger Anwendung des intravenösen Thrombozytenaggregationshemmers Abciximab. In der konservativen Gruppe werden Patienten nur dann einer Koronarangiographie und Angioplastie unterzogen, wenn ein neuer drohender Infarkt oder eine wiederkehrende Ischämie vorliegt. In der invasiven Gruppe wird die Ballonangioplastie so schnell wie möglich nach der Randomisierung durchgeführt. Patienten ohne Lebensfähigkeit werden nicht randomisiert, sondern nehmen als Kontrollgruppe teil.

Beschreibung
Einschlusskriterien:
- Aufnahme innerhalb von 6 Stunden nach Schmerzbeginn und EKG mit ≥ 1 mm ST-Strecken-Hebung in zwei oder mehr Standardableitungen oder ≥ 2 mm ST-Strecken-Hebung in zwei benachbarten Brustableitungen, was auf eine transmurale Ischämie hinweist, mit oder ohne Entwicklung neuer Q- Wellen, mit Myokardnekrose, nachgewiesen durch Enzymanstieg (Gesamt-CPK ≥ 2x der oberen Normgrenze).
- Anamnese des Patienten, die mit einem subakuten Myokardinfarkt (≥ 6 Stunden) vereinbar ist, mit mindestens Anzeichen einer Myokardnekrose im EKG (Q-Wellen) und einem deutlich erhöhten Serumspiegel herzspezifischer Enzyme.
- Alter zwischen 18 und 80 Jahren.
- Vor der Koronarangiographie durchgeführte Lebensfähigkeitstests.
- Keine klinische Indikation für die Durchführung eines invasiven Eingriffs vor der Lebensfähigkeitsprüfung
Ausschlusskriterien:
- Keine Einverständniserklärung eingeholt
- Unzuverlässige Nachverfolgung
- Eine Prüfung der Lebensfähigkeit ist technisch nicht möglich
- Kontraindikationen für die Dobutamin-Echokardiographie (Herzinsuffizienz, Arrhythmie)
- Kontraindikationen für eine Koronarangiographie, wie z. B. schwere diabetische Nephropathie oder bekannte Kontrastmittelallergie
- Bekannte Überempfindlichkeit gegen Abciximab
- Schwere, lebensbedrohliche nichtkardiale Erkrankung
