De levensvatbaarheid-geleide angioplastiek na een acuut myocardinfarct-onderzoek (het VIAMI-onderzoek)
Vroege en langetermijnresultaten van electieve stenting van de infarctgerelateerde slagader bij patiënten met levensvatbaarheid in het infarctgebied vroeg na een acuut myocardinfarct. De VIAMI-proef.

Eerdere studies hebben aangetoond dat routinematige angioplastiek van de infarctgerelateerde kransslagader kort na trombolyse voor een acuut myocardinfarct geen klinisch voordeel opleverde. Deze bevinding benadrukt het belang om een subgroep van patiënten met een recent myocardinfarct te selecteren, die echt baat zullen hebben bij angioplastiek van de infarctgerelateerde slagader.
Verschillende onderzoeken hebben aangetoond dat de aanwezigheid van levensvatbaarheid in het infarctgebied vroeg na een acuut myocardinfarct een verhoogd risico op nieuwe cardiale gebeurtenissen met zich meebrengt, zoals een terugkerend infarct, angina pectoris en de noodzaak van coronaire interventies. Daarom wordt verondersteld dat alleen patiënten met levensvatbaarheid in het infarctgebied baat zullen hebben bij angioplastiek kort na een acuut myocardinfarct.
In de VIAMI-studie zullen patiënten die in het ziekenhuis worden opgenomen met een acuut myocardinfarct en die geen onmiddellijke coronaire angioplastiek hebben ondergaan, binnen 3 dagen na opname een levensvatbaarheidstest ondergaan door middel van dobutamine-echocardiografie. Patiënten met duidelijke tekenen van levensvatbaarheid in het infarctgebied worden vervolgens gerandomiseerd naar een invasieve of een conservatieve behandelstrategie. Bij de invasieve strategie ondergaan patiënten coronaire angiografie met de bedoeling ballonangioplastiek uit te voeren met stenting van de infarctgerelateerde kransslagader, met gelijktijdig gebruik van de intraveneuze bloedplaatjesremmer abciximab. In de conservatieve groep ondergaan patiënten alleen coronaire angiografie en angioplastiek als er een nieuw dreigend infarct of terugkerende ischemie aanwezig is. In de invasieve groep wordt ballonangioplastiek zo snel mogelijk na randomisatie uitgevoerd. Patiënten zonder levensvatbaarheid worden niet gerandomiseerd, maar nemen deel als controlegroep.

Beschrijving
Inclusiecriteria:
- Opname binnen 6 uur na aanvang van de pijn en ECG met ≥ 1 mm ST-segmentelevatie in twee of meer standaardleads of ≥ 2 mm ST-elevatie in twee aaneengesloten thoraxleads, indicatief voor transmurale ischemie, met of zonder de ontwikkeling van nieuwe Q- golven, met myocardiale necrose bewezen door enzymstijging (totale CPK ≥ 2x de bovengrens van normaal).
- Patiëntgeschiedenis compatibel met subacuut myocardinfarct (≥ 6 uur), met ten minste tekenen van myocardnecrose op het ECG (Q-golven) en een significant verhoogde serumspiegel van hartspecifieke enzymen.
- Leeftijd tussen 18 en 80 jaar.
- Levensvatbaarheidstest uitgevoerd voorafgaand aan coronaire angiografie.
- Er is geen klinische indicatie voor een invasieve procedure die moet worden uitgevoerd voordat de levensvatbaarheid wordt getest

- Geen geïnformeerde toestemming verkregen
- Onbetrouwbare opvolging
- Testen op levensvatbaarheid technisch niet mogelijk
- Contra-indicaties voor dobutamine-echocardiografie (hartfalen, aritmie)
- Contra-indicaties voor coronaire angiografie, zoals ernstige diabetische nefropathie of bekende contrastallergie
- Bekende overgevoeligheid voor abciximab
- Ernstige, levensbedreigende niet-cardiale ziekte
