The Viability-Guided Angioplasty After Acute Myocardial Infarction-Trial (The VIAMI-Trial)
Resultados precoces e de longo prazo da colocação de stent eletivo da artéria relacionada ao infarto em pacientes com viabilidade na área do infarto logo após o infarto agudo do miocárdio. O VIAMI-Julgamento.

Estudos anteriores mostraram que a angioplastia de rotina da artéria coronária relacionada ao infarto logo após a trombólise para infarto agudo do miocárdio não resultou em benefício clínico. Esse achado enfatiza a importância de selecionar um subgrupo de pacientes com infarto do miocárdio recente, que realmente se beneficiarão da angioplastia da artéria relacionada ao infarto.
Vários estudos demonstraram que a presença de viabilidade na área do infarto logo após o infarto agudo do miocárdio impõe um risco aumentado de novos eventos cardíacos como infarto recorrente, angina pectoris e necessidade de intervenções coronarianas. Portanto, postula-se que apenas os pacientes com viabilidade na área do infarto se beneficiarão da angioplastia precocemente após o infarto agudo do miocárdio.
No estudo VIAMI, os pacientes admitidos no hospital com infarto agudo do miocárdio e que não foram submetidos à angioplastia coronária imediata serão submetidos a testes de viabilidade por ecocardiografia com dobutamina dentro de 3 dias após a admissão. Os pacientes com sinais inequívocos de viabilidade na área do infarto serão randomizados para uma estratégia de tratamento invasivo ou conservador. Na estratégia invasiva os pacientes serão submetidos à coronariografia com o intuito de realizar angioplastia com balão com stent da artéria coronária relacionada ao infarto, com uso concomitante do inibidor intravenoso de plaquetas abciximab. No grupo conservador, os pacientes serão submetidos à angiografia coronária e à angioplastia somente quando um novo infarto iminente ou isquemia recorrente estiver presente. No grupo invasivo, a angioplastia com balão será realizada o mais rápido possível após a randomização. Pacientes sem viabilidade não serão randomizados, mas participarão como grupo controle.

Critério de inclusão:
- Admissão dentro de 6 horas após o início da dor e ECG com supradesnivelamento do segmento ST ≥ 1 mm em duas ou mais derivações padrão ou supradesnivelamento do segmento ST ≥ 2 mm em duas derivações torácicas contíguas, indicativo de isquemia transmural, com ou sem desenvolvimento de novo Q- ondas, com necrose miocárdica comprovada pela elevação enzimática (CPK total ≥ 2x o limite superior da normalidade).
- História do paciente compatível com infarto do miocárdio subagudo (≥ 6 horas), com pelo menos sinais de necrose miocárdica no eletrocardiograma (ondas Q) e nível sérico significativamente aumentado de enzimas cardíacas específicas.
- Idade entre 18 e 80 anos.
- Teste de viabilidade realizado antes da angiografia coronária.
- Nenhuma indicação clínica para um procedimento invasivo a ser realizado antes do teste de viabilidade

- Acompanhamento não confiável
- Teste de viabilidade tecnicamente não é possível
- Contra-indicações para ecocardiografia com dobutamina (insuficiência cardíaca, arritmia)
- Contra-indicações para angiografia coronária, como nefropatia diabética grave ou alergia conhecida ao contraste
- Hipersensibilidade conhecida ao abciximabe
- Doença não cardíaca grave, com risco de vida
