Malgré les dépenses annuelles de 12 milliards de dollars pour l'évaluation d'urgence des douleurs thoraciques aux États-Unis, seuls 15% des patients admis ont une cause cardiaque de leurs symptômes présentés. Les unités de décision clinique (CDU) améliorent l'utilisation des ressources et sont une option de soins recommandée par l'American College of Cardiology / American Heart Association, mais sont sous-utilisées chez les patients souffrant de douleurs thoraciques à risque non faible en raison des faiblesses des tests cardiaques traditionnels. L'imagerie par résonance magnétique cardiaque (IRM) est sensible et spécifique de l'ischémie, peut évaluer simultanément la fonction cardiaque et la perfusion myocardique, et pourrait révolutionner le processus de diagnostic chez les patients à risque intermédiaire souffrant de douleurs thoraciques. La précision supérieure de la CMR pourrait réduire les tests et les procédures invasives. La sensibilité élevée pour l'ischémie en cours pourrait permettre l'imagerie en parallèle avec les marqueurs cardiaques. Par conséquent, la RMC pourrait améliorer la prise en charge des patients des services d'urgence (SU) souffrant de douleurs thoraciques à risque intermédiaire. Cependant, l'efficacité et la sécurité de la CMR n'ont pas été largement testées dans le cadre de la CDU.
Hypothèse principale : Une stratégie CDU-CMR réduira l'occurrence du composite de revascularisation, de réhospitalisation et de tests cardiaques récurrents à 90 jours par rapport à une stratégie de soins hospitaliers.
Méthodes : Les participants (n = 146) à risque intermédiaire de syndrome coronarien aigu (SCA) seront recrutés dans un essai clinique du Wake Forest University Baptist Medical Center (WFUBMC) ED. Les participants seront également randomisés pour CDU-CMR ou soins hospitaliers. Les participants au CDU-CMR subiront une imagerie CMR au repos et à l'effort en parallèle avec des marqueurs cardiaques en série. Les participants aux soins hospitaliers subiront des marqueurs cardiaques en série suivis de tests cardiaques existants, tels que déterminés par leurs prestataires de soins. Le résultat principal est le résultat composite de la revascularisation à 90 jours, de la réhospitalisation et des tests cardiaques récurrents. Le critère de jugement secondaire est la durée d'hospitalisation indexée. Les événements d'innocuité comprennent les SCA après la sortie, la mortalité et les événements indésirables liés aux tests d'effort.

Critère d'intégration:
- Âge supérieur ou égal à 21 ans au moment de l'inscription
- Gêne thoracique ou autres symptômes compatibles avec un SCA possible, comme indiqué par le médecin traitant après avoir obtenu un ECG et des biomarqueurs cardiaques pour l'évaluation du patient
- Le score de risque de thrombolyse dans l'infarctus du myocarde (TIMI) >/= 2 ou l'impression du médecin de symptômes à probabilité intermédiaire ou élevée représentent un SCA
- Le patient nécessite une évaluation en milieu hospitalier ou CDU pour sa douleur thoracique
- Le médecin traitant estime que le patient pourrait être renvoyé chez lui si la maladie cardiaque était exclue
- Le médecin traitant estime que le patient est en sécurité pour les soins de l'UCD
Évaluation de la probabilité avant le test L'évaluation du risque intermédiaire de développer un SCA sera basée sur un score de risque TIMI >/= 2 et/ou une impression clinique d'un médecin urgentiste certifié/éligible par le conseil de probabilité intermédiaire ou élevée que les symptômes représentent un SCA. Les médecins sont encouragés à utiliser les directives 2007 de l'American College of Cardiology (ACC)/American Heart Association (AHA) comme cadre pour cette évaluation.
Critère d'exclusion:
- Biomarqueurs cardiaques élevés
- Nouveau sus-décalage du segment ST sur n'importe quel électrocardiogramme (>/= 1 mV)
- Nouvelle dépression du segment ST sur n'importe quel électrocardiogramme (>/= 2 mV)
- Ischémie cardiaque inductible connue sans revascularisation ultérieure
- Impossible de rester à plat
- Hypotension symptomatique au moment de l'inscription (systolique < 90 mm Hg)
- Contre-indications à l'IRM (exemples : Pacemaker, défibrillateur, clips d'anévrisme cérébral, corps étranger oculaire métallique, dispositifs implantés, claustrophobie sévère)
- Refus du patient ou incapacité de se conformer à l'examen et au suivi du dossier médical
- Diagnostic terminal avec espérance de vie inférieure à 3 mois
- Actuellement enceinte
- Clairance de la créatinine < 45 ml/min au moment de l'inscription ou préoccupation clinique pour une lésion rénale aiguë
- Maladie hépatique chronique avec une clairance de la créatinine < 60 ml/min au moment de l'inscription
- Syndrome hépato-rénal
- Antécédents de greffe de foie, de cœur ou de rein
- Angioplastie confirmée, mise en place d'un stent ou pontage aortocoronarien (CABG) au cours des 6 derniers mois

Les patients sont admis à l'hôpital et reçoivent les soins habituels.

Expérimental: Protocole CDU-CMR
Les patients seront transférés à l'unité de décision clinique et subiront une évaluation par IRM cardiaque à l'effort.
|
Après l'évaluation au service d'urgence, les patients sont randomisés vers les soins de l'unité de décision clinique ou les soins aux patients hospitalisés.
Les patients de l'unité de décision clinique subiront également une IRM cardiaque d'effort.
Les patients du groupe de soins hospitaliers peuvent subir tous les tests souhaités, y compris l'IRM cardiaque, tel que déterminé par leur médecin traitant.
