Ondanks dat jaarlijks 12 miljard dollar wordt uitgegeven aan de noodevaluatie van pijn op de borst in de VS, heeft slechts 15% van de opgenomen patiënten een cardiale oorzaak van hun symptomen. Klinische beslissingseenheden (CDU's) verbeteren het gebruik van middelen en zijn een aanbevolen zorgoptie door het American College of Cardiology / American Heart Association, maar worden onderbenut bij niet-laagrisicopatiënten met pijn op de borst vanwege de zwakke punten van traditionele harttesten. Cardiale magnetische resonantiebeeldvorming (CMR) is gevoelig en specifiek voor ischemie, kan tegelijkertijd de hartfunctie en myocardiale perfusie beoordelen en kan een revolutie teweegbrengen in het diagnostische proces voor patiënten met een gemiddeld risico met pijn op de borst. De superieure nauwkeurigheid van CMR kan testen en invasieve procedures verminderen. De hoge gevoeligheid voor aanhoudende ischemie zou beeldvorming parallel met cardiale markers mogelijk kunnen maken. Als gevolg hiervan zou CMR de zorg kunnen verbeteren voor patiënten op de afdeling spoedeisende hulp (SEH) met pijn op de borst met middelmatig risico. De efficiëntie en veiligheid van CMR is echter niet uitgebreid getest in de CDU-setting.
Primaire hypothese: Een CDU-CMR-strategie zal het optreden van de combinatie van revascularisatie, heropname in het ziekenhuis en terugkerende harttesten na 90 dagen verminderen in vergelijking met een intramurale zorgstrategie.
Methoden: Deelnemers (n=146) met een middelmatig risico op acuut coronair syndroom (ACS) zullen worden gerekruteerd voor een klinische proef van Wake Forest University Baptist Medical Center (WFUBMC) ED. Deelnemers worden in gelijke mate gerandomiseerd naar CDU-CMR of intramurale zorg. CDU-CMR-deelnemers ondergaan rust- en stress-CMR-beeldvorming parallel met seriële cardiale markeringen. Deelnemers aan intramurale zorg zullen seriële cardiale markers ondergaan, gevolgd door bestaande cardiale testen zoals bepaald door hun zorgverleners. Het primaire resultaat is de samenstelling van 90 dagen revascularisatie, heropname in het ziekenhuis en terugkerende harttesten. Het secundaire resultaat is de duur van het verblijf in het ziekenhuis. Veiligheidsgebeurtenissen omvatten ACS na ontslag, mortaliteit en aan stresstests gerelateerde bijwerkingen.

Beschrijving
Inclusiecriteria:
- Leeftijd groter dan of gelijk aan 21 jaar op het moment van inschrijving
- Ongemak op de borst of andere symptomen die passen bij mogelijke ACS zoals aangegeven door de behandelend arts na het verkrijgen van een ECG en cardiale biomarkers voor evaluatie van de patiënt
- Trombolyse bij myocardinfarct (TIMI) risicoscore >/= 2 of de indruk van de arts dat de symptomen middelmatig of zeer waarschijnlijk zijn, vertegenwoordigen ACS
- Patiënt heeft een intramurale of CDU-evaluatie nodig voor hun pijn op de borst
- De behandelend arts is van mening dat de patiënt naar huis kan worden ontslagen als hartaandoeningen worden uitgesloten
- De behandelend arts voelt dat de patiënt veilig is voor CDU-zorg
Kansbepaling voorafgaand aan de test De beoordeling van het middelmatige risico voor het ontwikkelen van ACS zal worden gebaseerd op een TIMI-risicoscore >/= 2 en/of een door de raad gecertificeerde / door de raad in aanmerking komende klinische indruk van een spoedeisende hulparts met een middelmatige of hoge waarschijnlijkheid dat de symptomen ACS vertegenwoordigen. Artsen worden aangemoedigd om de richtlijnen van het American College of Cardiology (ACC)/American Heart Association (AHA) uit 2007 te gebruiken als raamwerk voor deze beoordeling.
Uitsluitingscriteria:
- Verhoogde cardiale biomarkers
- Nieuwe ST-segmentelevatie op elk elektrocardiogram (>/= 1 mV)
- Nieuwe ST-segmentdepressie op elk elektrocardiogram (>/= 2 mV)
- Bekende induceerbare cardiale ischemie zonder daaropvolgende revascularisatie
- Kan niet plat liggen
- Symptomatische hypotensie op het moment van inschrijving (systolisch < 90 mm Hg)
- Contra-indicaties voor MRI (voorbeelden: pacemaker, defibrillator, cerebrale aneurysmaclips, metalen oculair vreemd lichaam, geïmplanteerde apparaten, ernstige claustrofobie)
- Weigering of onvermogen van de patiënt om te voldoen aan de beoordeling en follow-up van het medisch dossier
- Terminale diagnose met een levensverwachting van minder dan 3 maanden
- Momenteel zwanger
- Creatinineklaring < 45 ml/min op het moment van inschrijving of klinische zorg voor acuut nierletsel
- Chronische leverziekte met een creatinineklaring van <60 ml/min op het moment van inschrijving
- Hepato-renaal syndroom
- Geschiedenis van lever-, hart- of niertransplantatie
- Bevestigde angioplastiek, plaatsing van een stent of coronaire bypassoperatie (CABG) in de afgelopen 6 maanden

Dit is de vergelijkingsarm.
Patiënten worden opgenomen in het ziekenhuis en ondergaan de gebruikelijke zorg.

Experimenteel: CDU-CMR-protocol
Patiënten worden overgebracht naar de klinische beslissingseenheid en ondergaan een stresscardiale MRI-evaluatie.
|
Na ED-evaluatie worden patiënten gerandomiseerd naar zorg op de klinische beslissingseenheid of intramurale zorg.
Patiënten op de klinische besliseenheid zullen ook een stresscardiale MRI ondergaan.
Patiënten in de intramurale zorgarm kunnen elke gewenste test ondergaan, inclusief cardiale MRI, zoals bepaald door hun behandelend arts.
