Essai de phase I du topotécan à haute dose en association avec le carboplatine, avec support de cellules souches du sang périphérique chez des patientes atteintes d'un premier carcinome ovarien récidivant sans traitement au platine depuis 6 à 12 mois

Description détaillée
La combinaison Topotécan plus carboplatine à fortes doses a été publiée par Miles Prince et al en 2001. Dans une triple association, les auteurs ont pu définir la Dose Maximale Tolérée (DMT) de 3,5 mg/m²/jour x 5 jours pour le topotécan, 250 mg/m² pour le paclitaxel et l'AUC à 12 pour le carboplatine (46). La DMT du topotécan combiné au carboplatine (AUC 16) et VP 16 n'a pas pu être déterminée par Carroll et al (47). Or, dans l'étude ITOV01bis (ASCO abstract 2007 n°1661), la DMT du topotécan a été déterminée en association avec le cyclophosphamide à 120 mg/kg et a été fixée à 9 mg/m2/jx 5 jours, soit la même que la DMT utilisée dans monothérapie ITOV 01).
Des études rapportées ci-dessus, l'association à haute dose de topotécan et de carboplatine semble possible avec une dose limitée de carboplatine à ASC 20, une allocation de 5 jours pour les deux médicaments [avec une ASC quotidienne fixe à 4 pour le carboplatine, identique au programme TAXIF I en tumeurs germinales, publié par notre équipe (Annual Oncology 2004) ainsi que TAXIF II développé par l'hôpital Tenon] avec un temps d'administration de 30 minutes par jour pour le topotécan et de 2 heures pour le carboplatine.
ces données justifient le schéma de notre étude :
- un traitement établi de 5 jours consécutifs offre le meilleur indice thérapeutique,
- perfusion de 30 minutes, semble donner moins de toxicité non hématologique par rapport à la perfusion continue, qui a prévalu dans l'essai ITOV 01,
- Le sauvetage par les cellules souches sanguines (prélevées par chimiothérapie type mobilisation de cyclophosphamide à forte dose suivie de facteurs de croissance hématopoïétiques (G-CSF, Filgrastim) réinjection est prévue à H96 après la fin du traitement,
- six doses séquentielles établies en l'absence de toxicité limitante, comme suit : 7,5 - 8,0 - 8,5 - 9,0 - 9,5 - 10,0 mg/m2. Les paliers 9,5 mg/m² et 10 mg/m seront discutés après approbation par un comité indépendant en charge de l'étude. La poursuite du Topotécan à dose conventionnelle pourra se faire grâce à des données cliniques basées sur l'efficacité et la tolérance.

Critère d'intégration:
- Adénocarcinome primitif de l'ovaire ou des trompes, ou carcinome péritonéal prouvé histologiquement
- Âge entre 18 et 65 ans
- Critères ECOG £ 2
- Patientes atteintes d'un premier cancer de l'ovaire en rechute sans traitement à base de platine depuis 6 à 12 mois et après un traitement de première ligne avec du sel de platine et des taxanes ensemble ou successivement
- Sérologie virale négative (HbS, HbC et VIH)
- Consentement éclairé
- Patients avec la sécurité sociale
Critère d'exclusion:
- Carcinome ovarien en rechute réfractaire (rechute < 6 mois) ou sensible (rechute > 12 mois)
- Espérance de vie < 3 mois
- Traitement antérieur par radiographie pelvienne
- Traitement antérieur par le topotécan ou un autre inhibiteur du topoisomère I
- Occlusion intestinale non résolutive sous traitement symptomatique
- Créatinine > ou égale à 1,25N et/ou clairance de la créatinine < ou égale à 60 ml/mn
- Bilirubine > 1,25N ; transaminase et phosphatase alcaline > 2N (3N si métastases hépatiques étaient présentes)
- Cœur anormal (échographie uniquement) (FR < 30 % ; FEVG < 50 %)
- Globules blancs < ou égal à 4,0 x 109/L, Neutrophiles < ou égal à 1,5 x 109/L, plaquettes < ou égal à 100 x 109/L
- Neuropathie : grade > ou égal à 2
- Épilepsie
- Métastases cérébrales symptomatiques
- Pathologie psychiatrique grave
- Infection grave non maîtrisée
- Patient ayant déjà reçu un support de cellules souches du sang périphérique
- Allergie aux facteurs de croissance hématopoïétiques
- Plus d'une ligne de chimiothérapie
- Impossibilité d'utiliser un accès veineux central
- Hypersensibilité au carboplatine ou à d'autres produits contenant du platine
- Participation à un autre essai clinique
- Absence de contraception efficace
