Un essai de fenêtre d'opportunité sur l'afatinib dans le cancer du poumon non à petites cellules (NSCLC) à un stade précoce
Un essai thérapeutique de l'afatinib dans le cadre néoadjuvant. Effets moléculaires de l'afatinib : un essai de fenêtre d'opportunité dans le NSCLC à un stade précoce
Cette étude s'adresse aux patients atteints d'un cancer du poumon non à petites cellules (NSCLC) et il ou elle subira une intervention chirurgicale pour retirer la tumeur. L'objectif principal de cette étude est d'étudier la sécurité et l'adéquation du traitement par l'afatinib chez les participants avant l'intervention chirurgicale pour retirer la tumeur. Un autre objectif de l'étude est de voir si le traitement par Afatinib affecte la croissance et l'activité de la tumeur. L'afatinib n'a pas été approuvé pour une utilisation par la FDA (Food and Drug Administration) et est expérimental.
Chez certaines personnes, on pense que la croissance des cellules cancéreuses est stimulée par le facteur de croissance épidermique (EGF). Il s'agit d'une protéine présente dans le corps qui se lie à la paroi cellulaire à des sites appelés récepteurs et stimule la croissance cellulaire. Certaines cellules ont trop de récepteurs et une croissance incontrôlée et à cause de cela, se développent en une tumeur cancéreuse. Il existe plusieurs autres médicaments anticancéreux qui se lient à ces récepteurs pour ralentir ou arrêter la croissance du cancer, tels que le géfitinib et l'erlotinib, qui sont approuvés par la FDA et utilisés pour le traitement du NSCLC.
Il a été démontré que le médicament Afatinib inhibe les récepteurs EGF mutés lors d'essais cliniques et peut être un candidat pour le traitement du NSCLC et de divers cancers à un stade précoce. Les participants à cette étude prendront l'Afatinib par voie orale, sous forme de comprimés, pendant au moins 14 jours avant de subir leur intervention chirurgicale prévue pour retirer leur tumeur cancéreuse. Les participants seront observés pour les effets secondaires et suivis pendant 30 jours après la chirurgie pour évaluer les résultats après la chirurgie.

Critère d'intégration:
- NSCLC histologiquement confirmé, qui sont considérés comme des candidats chirurgicaux selon les critères standard. Les patients avec toutes les histologies seront autorisés à s'inscrire.
Patients atteints d'une maladie de stade IA à IIIA
- Statut de performance ECOG 0-1
- Maladie mesurable selon les critères RECIST 1.1
- Médiastinoscopie et/ou échographie bronchique endoscopique (EBUS) et/ou échographie endoscopique (EUS) pour une stadification chirurgicale complète lorsque cela est cliniquement indiqué
- Bilirubine totale ≤ 1,5 mg/dl, SGOT (AST) et SGPT (ALT) ≥ 3 x LSN
- Créatinine sérique ≤ 1,5 mg/dl
- Les infections graves et actives doivent être contrôlées. Les patients peuvent être recrutés alors qu'ils sont encore sous antibiotiques tant que les signes cliniques d'infection active ont disparu.
- Un document de consentement éclairé (ICD) signé
- Patients de 18 ans ou plus
- Capable et disposé à prendre des médicaments par voie orale
Critère d'exclusion:
- Maladie pulmonaire préexistante connue.
- Antécédents ou présence d'anomalies cardiovasculaires cliniquement pertinentes telles qu'hypertension non contrôlée, insuffisance cardiaque congestive de classification NYHA 3, angor instable ou arythmie mal contrôlée. Infarctus du myocarde dans les 6 mois précédant la randomisation.
- Troubles gastro-intestinaux aigus importants ou récents avec diarrhée comme symptôme majeur, par ex. Maladie de Crohn, malabsorption ou diarrhée de grade CTC ≥2 de toute étiologie.
- Fonction ventriculaire gauche cardiaque de base (< 1 mois avant le traitement) avec une fraction d'éjection au repos inférieure à 50 % mesurée par imagerie du pool sanguin multigated du cœur (scan MUGA) ou échocardiogramme
- Les patients peuvent ne pas recevoir d'autres agents expérimentaux.
- Antécédents de réactions allergiques aux anilinoquinazolines comme le gefitinib, l'erlotinib ou le BIBW 2992
- Maladie intercurrente non contrôlée qui empêcherait un patient de subir une intervention chirurgicale
- Maladie psychiatrique / situations sociales qui limiteraient le respect des exigences de l'étude.
- Enceinte (test de grossesse positif) ou allaitante
- Incapacité à se conformer aux procédures d'étude et/ou de suivi
- Patients non candidats à la chirurgie ou refusant la chirurgie

Expérimental: Étude à un seul bras
Essai à un seul bras où tous les patients seront traités avec l'afatinib jusqu'au jour de la chirurgie et pendant au moins deux semaines.
Les patients recevront un traitement avec 40 mg d'afatinib par voie orale par jour.

Faisabilité
Délai: 30 jours de soins post opératoires

Le critère d'évaluation principal démontrera la faisabilité du traitement néoadjuvant du BIBW 2992 chez les patients atteints d'un CPNPC de stade précoce (IA à IIIA) en attente d'une résection chirurgicale.
La faisabilité sera évaluée en fonction de la capacité à terminer le traitement pour chaque patient.
Pour notre critère de faisabilité, nous déclarerons le traitement "terminé" si un patient termine au moins 14 jours de traitement, a subi une thoracotomie pour la résection chirurgicale prévue et 30 jours de soins postopératoires.

L'objectif secondaire est de déterminer si le traitement préopératoire au BIBW 2992 affecte le marquage métabolique de la tumeur, tel que mesuré par PET-CT.
Nous mesurerons la variation absolue et en pourcentage du SUV entre le départ et le suivi.
