Een Window of Opportunity-onderzoek naar Afatinib bij niet-kleincellige longkanker in een vroeg stadium (NSCLC)
Een therapeutische proef met Afatinib in de neoadjuvante setting. Moleculaire effecten van Afatinib: een kansstudie in een vroeg stadium van NSCLC
Deze studie is voor patiënten met niet-kleincellige longkanker (NSCLC) en hij of zij zal een operatie ondergaan om de tumor te verwijderen. Het belangrijkste doel van deze studie is om te onderzoeken hoe veilig en geschikt de behandeling met Afatinib is bij deelnemers vóór de operatie om de tumor te verwijderen. Een ander doel van de studie is om te zien of behandeling met Afatinib de groei en activiteit van de tumor beïnvloedt. Afatinib is niet goedgekeurd voor gebruik door de FDA (Food and Drug Administration) en is experimenteel.
Bij sommige personen wordt aangenomen dat de groei van kankercellen wordt gestimuleerd door epidermale groeifactor (EGF). Dit is een eiwit dat in het lichaam wordt aangetroffen en dat zich aan de celwand bindt op plaatsen die receptoren worden genoemd en celgroei stimuleert. Sommige cellen hebben te veel receptoren en ongecontroleerde groei en ontwikkelen zich daardoor tot een kankergezwel. Er zijn verschillende andere geneesmiddelen tegen kanker die zich aan deze receptoren binden om de groei van kanker te vertragen of te stoppen, zoals Gefitinib en erlotinib, die door de FDA zijn goedgekeurd en worden gebruikt voor de behandeling van NSCLC.
Van het medicijn Afatinib is aangetoond dat het gemuteerde EGF-receptoren in klinische onderzoeken remt en mogelijk een kandidaat is voor de behandeling van NSCLC en een verscheidenheid aan kankers in een vroeg stadium. Deelnemers aan deze studie zullen Afatinib via de mond innemen, in pilvorm, gedurende ten minste 14 dagen voordat ze hun geplande operatie ondergaan om hun kankertumor te verwijderen. Deelnemers worden geobserveerd op bijwerkingen en gedurende 30 dagen na de operatie gevolgd om de resultaten na de operatie te beoordelen.

Beschrijving
Inclusiecriteria:
- Histologisch bevestigde NSCLC, die volgens standaardcriteria als chirurgische kandidaten worden beschouwd. Patiënten met alle histologieën mogen zich inschrijven.
Patiënten met ziekte van stadium IA tot IIIA
- ECOG-prestatiestatus 0-1
- Meetbare ziekte volgens RECIST 1.1-criteria
- Mediastinoscopie en/of endoscopische bronchiale echografie (EBUS) en/of endoscopische echografie (EUS) voor volledige chirurgische stadiëring indien klinisch geïndiceerd
- Totaal bilirubine ≤1,5 mg/dl, SGOT (AST) en SGPT (ALAT) ≥ 3 x ULN
- Serumcreatinine ≤ 1,5 mg/dl
- Ernstige, actieve infecties moeten onder controle worden gehouden. Patiënten kunnen worden ingeschreven terwijl ze nog antibiotica gebruiken, zolang de klinische tekenen van actieve infectie zijn verdwenen.
- Een ondertekend geïnformeerd toestemmingsdocument (ICD)
- Patiënten van 18 jaar of ouder
- In staat en bereid om orale medicatie in te nemen
Uitsluitingscriteria:
- Bekende reeds bestaande longziekte.
- Geschiedenis of aanwezigheid van klinisch relevante cardiovasculaire afwijkingen zoals ongecontroleerde hypertensie, congestief hartfalen NYHA-classificatie van 3, onstabiele angina pectoris of slecht gecontroleerde aritmie. Myocardinfarct binnen 6 maanden voorafgaand aan randomisatie.
- Significante of recente acute gastro-intestinale stoornissen met diarree als een belangrijk symptoom, b.v. Ziekte van Crohn, malabsorptie of CTC-graad ≥2 diarree van welke etiologie dan ook.
- Basislijn (<1 maand voor de behandeling) linker ventrikelfunctie van het hart met een ejectiefractie in rust van minder dan 50%, gemeten door multigated blood pool imaging van het hart (MUGA-scan) of echocardiogram
- Het is mogelijk dat patiënten geen andere onderzoeksagentia krijgen.
- Geschiedenis van allergische reacties op anilinoquinazolinen zoals gefitinib, erlotinib of BIBW 2992
- Ongecontroleerde bijkomende ziekte die een patiënt zou beletten een operatie te ondergaan
- Psychiatrische ziekte/sociale situaties die de naleving van de studievereisten zouden beperken.
- Zwanger (positieve zwangerschapstest) of borstvoeding gevend
- Het niet kunnen volgen van studie- en/of vervolgprocedures
- Patiënten die geen chirurgische kandidaat zijn of een operatie weigeren

Experimenteel: Eenarmige studie
Eenarmige studie waarbij alle patiënten tot de dag van de operatie en gedurende minimaal twee weken met afatinib worden behandeld.
Patiënten zullen dagelijks worden behandeld met afatinib 40 mg oraal.

Het primaire eindpunt zal de haalbaarheid aantonen van neoadjuvante behandeling van BIBW 2992 bij patiënten met NSCLC in een vroeg stadium (IA tot IIIA) die wachten op chirurgische resectie.
De haalbaarheid zal worden beoordeeld op basis van het vermogen om de behandeling voor elke patiënt te voltooien.
Voor ons haalbaarheidseindpunt verklaren we de behandeling als "voltooid" als een patiënt ten minste 14 dagen behandeling heeft voltooid, een thoracotomie heeft ondergaan voor de geplande chirurgische resectie en 30 dagen postoperatieve zorg.

Het secundaire doel is om te bepalen of preoperatieve BIBW 2992-behandeling invloed heeft op de metabole tumorlabeling, zoals gemeten met PET-CT-scanning.
We meten de absolute en de procentuele verandering in SUV vanaf baseline tot follow-up.
