Radiothérapie guidée par l'image hypofractionnée (IGRT) avec atténuation des mouvements d'organes et épargne urétrale pour le cancer de la prostate
La présente étude évalue les résultats cliniques après une radiothérapie externe définitive à ultra-haute dose par fraction administrée à des patients atteints d'un adénocarcinome confiné de la prostate.
Les patients inscrits à l'étude subiront une radiothérapie à l'arc volumétrique à modulation d'intensité guidée par l'image (IGRT-VMAT) avec des procédures de planification de traitement et d'assurance qualité à la pointe de la technologie, en mettant l'accent sur la préservation des tissus normaux et la précision de l'administration via l'utilisation de dispositifs assurant la stabilité et la reproductibilité de la localisation du faisceau. Un ballonnet rectal rempli d'air sera utilisé pour l'immobilisation de la cible prostatique et la reproductibilité anatomique, tandis qu'un cathéter urétral chargé de transpondeurs beacon sera utilisé pour assurer la reproductibilité de la configuration et le suivi de la cible en ligne. Les patients atteints d'un cancer de la prostate n'ayant jamais été traités seront traités avec 45 Gy en cinq fractions de 9 Gy sur 5 jours consécutifs.
Les patients seront suivis un mois après le traitement et tous les 3 mois jusqu'à 12 mois (+/- 4 semaines) et tous les 6 mois (+/- 6 semaines) par la suite. Les évaluations de la toxicité aiguë et tardive se concentreront, mais pas exclusivement, sur les fonctions urinaires, rectales et sexuelles et seront évaluées à l'aide de l'indice composite élargi du cancer de la prostate (EPIC), du score international des symptômes de la prostate (IPSS) et de l'indice international de la fonction érectile (IIEF). questionnaires. Les valeurs d'antigène spécifique de la prostate (PSA) sériques seront tirées selon le même calendrier que le suivi clinique. Les patients seront surveillés en continu pendant au moins 5 ans.

This is a prospective, single-arm phase II study of definitive ultra-hypofractionated image-guided radiotherapy for localized prostate adenocarcinoma. The study is designed to evaluate the acute and late safety profile, feasibility, patient-reported quality-of-life outcomes, PSA kinetics, biochemical relapse-free survival, and clinical outcomes of dose-escalated IGRT-VMAT using organ-motion mitigation and urethral sparing.
Patients will receive 45 Gy in 5 fractions of 9 Gy each. Treatment planning will use CT/MRI-based target and organ-at-risk delineation. The protocol incorporates a rectal balloon for prostate stabilization and anatomical reproducibility, a urethral catheter loaded with beacon transponders for urethral localization and online motion tracking, and inverse dose-painting to reduce urethral dose when compatible with target coverage and disease anatomy. Erectile-related structures, including the neurovascular bundles, urogenital diaphragm, and penile bulb, may be contoured and recorded for quality-of-life and dosimetric analyses.
The primary safety assessments are treatment-related Grade 3 or higher gastrointestinal or genitourinary toxicity occurring from the first protocol fraction through 90 days after treatment completion, and treatment-related late Grade 2 or higher gastrointestinal or genitourinary toxicity occurring more than 90 days through 5 years after completion of radiotherapy. Late Grade 3 or higher gastrointestinal or genitourinary toxicity will be summarized separately. Clinically meaningful deterioration from baseline in urinary, bowel, and sexual patient-reported outcomes will be evaluated as a key safety and quality-of-life assessment.
Secondary assessments include PSA kinetics, biochemical relapse-free survival using the Phoenix definition, quality-of-life changes measured by EPIC, IPSS, and IIEF instruments, erectile and sexual function preservation, associations between clinical or dosimetric factors and outcomes, hormonal therapy exposure, and patterns of recurrence on clinically indicated imaging.

Critère d'intégration:
- Formulaire de consentement éclairé spécifique à l'étude signé ;
- Confirmation histologique de l'adénocarcinome de la prostate par biopsie ;
- Une hormonothérapie antérieure est autorisée (mais pas obligatoire);
- Aucune preuve directe de métastases régionales ou à distance après des études de stadification appropriées ;
- Âge ≥ 40 ;
- État des performances 0-2 ;
- Le score de l'American Urological Association (AUA) doit être ≤ 20 (alpha-bloquants autorisés) ;
- Tomographie informatisée (CT) ou estimation du volume par ultrasons de la prostate ≤ 150 grammes.
Critère d'exclusion:
- Maladie métastatique du cancer de la prostate sur les études d'imagerie Preuve IRM de T3, T4 radiographiques ;
- Radiothérapie pelvienne antérieure ;
- Chirurgie antérieure pour le cancer de la prostate ;
- Traitement hormonal antérieur administré pendant plus de 6 mois avant le traitement ;

- Maladie psychiatrique grave ;

Expérimental: Experimental: Ultra-hypofractionated IGRT-VMAT 45 Gy in 5 fractions of 9 Gy
Participants will receive definitive dose-escalated image-guided volumetric modulated arc radiotherapy for localized prostate adenocarcinoma.
Radiotherapy will be delivered to 45 Gy in 5 fractions of 9 Gy each over 5 consecutive days, using organ-motion mitigation, urethral localization, online target tracking, urethral sparing, rectal balloon stabilization, and protocol-specified treatment planning and quality assurance procedures.
