Gehypofractioneerde beeldgeleide radiotherapie (IGRT) met beperking van orgaanbewegingen en urethrasparing voor prostaatkanker
De huidige studie evalueert de klinische resultaten na een definitieve ultrahoge dosis per fractie externe bestralingstherapie die wordt toegediend bij patiënten met adenocarcinoom van de prostaat met orgaanbeperking.
Patiënten die deelnemen aan het onderzoek ondergaan beeldgeleide, volumetrische intensiteitsgemoduleerde boogradiotherapie (IGRT-VMAT) met ultramoderne procedures voor behandelingsplanning en kwaliteitsborging, waarbij de nadruk ligt op normaal weefselsparen en accurate toediening via het gebruik van apparaten die zorgen voor stabiliteit en reproduceerbaarheid van de straallocatie. Een rectale ballon met luchtvulling zal worden gebruikt voor immobilisatie van het prostaatdoel en anatomische reproduceerbaarheid, terwijl een urethrale katheter geladen met bakentransponders zal worden gebruikt om reproduceerbaarheid van de set-up en online doeltracering te garanderen. Niet eerder behandelde patiënten met prostaatkanker worden gedurende 5 opeenvolgende dagen behandeld met 45 Gy in vijf fracties van 9 Gy.
Patiënten zullen één maand na de behandeling worden gevolgd en elke 3 maanden gedurende maximaal 12 maanden (+/- 4 weken) en daarna elke 6 maanden (+/- 6 weken). Evaluaties van acute en late toxiciteit zullen zich, maar niet uitsluitend, richten op urinaire, rectale en seksuele functies en zullen worden beoordeeld door middel van gevalideerde Expanded Prostate Cancer Index Composite (EPIC), International prostate symptom score (IPSS) en International index of erectile function (IIEF). vragenlijsten. Serum Prostate Specific Antigen (PSA)-waarden zullen volgens hetzelfde schema worden opgesteld als de klinische follow-up. Patiënten worden gedurende minimaal 5 jaar continu gecontroleerd.

This is a prospective, single-arm phase II study of definitive ultra-hypofractionated image-guided radiotherapy for localized prostate adenocarcinoma. The study is designed to evaluate the acute and late safety profile, feasibility, patient-reported quality-of-life outcomes, PSA kinetics, biochemical relapse-free survival, and clinical outcomes of dose-escalated IGRT-VMAT using organ-motion mitigation and urethral sparing.
Patients will receive 45 Gy in 5 fractions of 9 Gy each. Treatment planning will use CT/MRI-based target and organ-at-risk delineation. The protocol incorporates a rectal balloon for prostate stabilization and anatomical reproducibility, a urethral catheter loaded with beacon transponders for urethral localization and online motion tracking, and inverse dose-painting to reduce urethral dose when compatible with target coverage and disease anatomy. Erectile-related structures, including the neurovascular bundles, urogenital diaphragm, and penile bulb, may be contoured and recorded for quality-of-life and dosimetric analyses.
The primary safety assessments are treatment-related Grade 3 or higher gastrointestinal or genitourinary toxicity occurring from the first protocol fraction through 90 days after treatment completion, and treatment-related late Grade 2 or higher gastrointestinal or genitourinary toxicity occurring more than 90 days through 5 years after completion of radiotherapy. Late Grade 3 or higher gastrointestinal or genitourinary toxicity will be summarized separately. Clinically meaningful deterioration from baseline in urinary, bowel, and sexual patient-reported outcomes will be evaluated as a key safety and quality-of-life assessment.
Secondary assessments include PSA kinetics, biochemical relapse-free survival using the Phoenix definition, quality-of-life changes measured by EPIC, IPSS, and IIEF instruments, erectile and sexual function preservation, associations between clinical or dosimetric factors and outcomes, hormonal therapy exposure, and patterns of recurrence on clinically indicated imaging.

Inclusiecriteria:
- Ondertekend onderzoeksspecifiek toestemmingsformulier;
- Histologische bevestiging van adenocarcinoom van de prostaat door biopsie;
- Eerdere hormoontherapie is toegestaan (maar niet vereist);
- Geen direct bewijs van regionale of verre metastasen na geschikte stadiëringsonderzoeken;
- Leeftijd ≥ 40;
- Prestatiestatus 0-2;
- De score van de American Urological Association (AUA) moet ≤ 20 zijn (alfablokkers zijn toegestaan);
- Computertomografie (CT) of op echografie gebaseerde volumeschatting van de prostaatklier ≤ 150 gram.
Uitsluitingscriteria:
- Gemetastaseerde ziekte van prostaatkanker op beeldvormingsonderzoeken MRI-bewijs van radiografische T3, T4;
- Eerdere bekkenbestraling;
- Eerdere operatie voor prostaatkanker;
- Eerdere hormonale therapie gegeven gedurende meer dan 6 maanden voorafgaand aan de therapie;
- Geschiedenis van de ziekte van Crohn of colitis ulcerosa;
- Eerdere significante obstructieve symptomen;
- Aanzienlijke psychiatrische ziekte;
- Ernstige, actieve comorbiditeit zoals gedefinieerd in rubriek 3.

Participants will receive definitive dose-escalated image-guided volumetric modulated arc radiotherapy for localized prostate adenocarcinoma.
Radiotherapy will be delivered to 45 Gy in 5 fractions of 9 Gy each over 5 consecutive days, using organ-motion mitigation, urethral localization, online target tracking, urethral sparing, rectal balloon stabilization, and protocol-specified treatment planning and quality assurance procedures.
