OBJECTIFS PRINCIPAUX:
I. Évaluer les scores cosmétiques photographiques mammaires de 24 mois avec un rayonnement à 9 fractions par rapport à l'hypofractionnement standard.
OBJECTIFS SECONDAIRES :
I. Évaluer les résultats rapportés par les patientes (PRO) de Breast-Q par rapport au contrôle historique dans la conservation du sein.
II. Évaluer l'incidence des complications aiguës et tardives de l'irradiation, sur la base des critères de terminologie communs pour les événements indésirables (CTCAE) 4.0 toxicité.
III. Évaluer le taux de récidive local et local régional. IV. Comparer les coûts directs et indirects pour le patient de la radiothérapie par rapport aux témoins historiques ainsi que les années de vie ajustées sur la qualité, en utilisant les résultats rapportés par les patients (PRO) de Breast-Q et l'EQ-5D.
PRÉSENTER:
Dans les 12 semaines suivant la chirurgie conservatrice du sein, les patientes subissent une radiothérapie hypofractionnée pendant 9 fractions sur 2 semaines.
Après la fin du traitement à l'étude, les patients sont suivis à 2-8 semaines, 6 mois et annuellement pendant 3 ans.

Critère d'intégration:
- Carcinome invasif confirmé histologiquement et/ou carcinome canalaire in situ (CCIS) du sein
- Tis pathologique final, T1-T3, tous doivent être de statut N0 et M0.
- Marges histologiques encrées négatives de la tumorectomie, à l'exception d'un foyer de marge positive au fascia pectoral
- Le radio-oncologue ne prévoit pas de traiter les ganglions lymphatiques régionaux au-delà des champs tangents standards du sein entier
- Tumorectomie avec ganglion lymphatique négatif lors de l'évaluation chirurgicale (les cellules tumorales isolées dans les ganglions lymphatiques seront autorisées). Les patients atteints d'un carcinome invasif ≥ 70 ans et avec une tumeur ER+ positive ≤ 2,0 cm peuvent s'inscrire sans évaluation chirurgicale des ganglions lymphatiques, conformément à la section 5.1. Les patientes atteintes d'un carcinome canalaire in situ (CCIS) du sein uniquement peuvent s'inscrire sans évaluation chirurgicale des ganglions lymphatiques.
Bêta-gonadotrophine chorionique humaine (HCG) sérique ou urinaire négative chez les femmes en âge de procréer = < 7 jours avant l'enregistrement
- Une femme en âge de procréer est une femme sexuellement mature qui n'a pas subi d'hystérectomie ou d'ovariectomie bilatérale et qui n'a pas été naturellement ménopausée pendant au moins 12 mois consécutifs
- Les femmes en âge de procréer doivent accepter d'utiliser une forme de contraception ou accepter de subir une abstinence sexuelle pendant la radiothérapie
- Statut de performance du Eastern Cooperative Oncology Group (ECOG) (Zubrod) 0-1
- Capable de fournir un consentement éclairé et disposé à signer un formulaire de consentement approuvé conforme aux directives fédérales et institutionnelles
Critère d'exclusion:
- Radiothérapie antérieure au thorax, au cou ou aux aisselles
- Antécédents de cancer du sein homolatéral (maladie invasive ou CCIS) ; le carcinome lobulaire in situ (CLIS) et la maladie bénigne du sein sont autorisés
- Antécédents de cancer du sein invasif controlatéral antérieur ou concomitant ; une maladie bénigne du sein, un CLIS ou un CCIS du sein controlatéral est autorisé
- Maladies vasculaires actives du collagène, telles que : lupus érythémateux disséminé, sclérodermie ou dermatomyosite
- Complications post-tumorectomie importantes nécessitant une ré-opération non planifiée ou une admission pour des antibiotiques intraveineux (IV) ; une nouvelle opération pour l'évaluation des marges ou l'évaluation nodale est acceptable
- Conditions médicales coexistantes avec une espérance de vie < 5 ans
- Autre tumeur maligne dans les 5 ans suivant l'enregistrement, à l'exception du carcinome basocellulaire ou épidermoïde de la peau traité par résection locale uniquement ou du carcinome in situ du col de l'utérus
- Chimiothérapie néoadjuvante ou chimiothérapie adjuvante administrée avant la radiothérapie
- Histologie du carcinome ou du sarcome neuroendocrinien
- Médicaments sensibilisants aux rayonnements en même temps que les rayonnements, par médecin traitant

Le système d'évaluation cosmétique du sein de l'EORTC (Organisation européenne pour la recherche et le traitement du cancer) est une méthode photographique numérique en aveugle qui compare le sein irradié avec le côté controlatéral non traité et évalue : la taille, la forme, l'emplacement de l'aréole/du mamelon, l'apparence du site chirurgical. cicatrice, modifications de la pigmentation cutanée, présence de télangiectasie et un score cosmétique global basé sur l'ensemble des facteurs.
Les caractéristiques sont notées sur une échelle de quatre points : 0, excellent ou aucune différence ; 1, bonne ou petite différence ; 2, différence juste ou modérée ; et 3, différence faible ou importante.
Les images prises à 24 mois ont été évaluées par 3 examinateurs impartiaux, dont un radio-oncologue, un chirurgien et une infirmière.
Les données sont présentées sous forme de nombre de participants répondant aux critères d'évaluation globaux évalués par ces évaluateurs.
