Critère d'intégration:
- A terminé 6 semaines de traitement post-randomisation dans l'essai CVL-231-2001 (NCT05227690) ou CVL-231-2002 (NCT05227703) et qui, de l'avis de l'investigateur, pourrait potentiellement bénéficier d'un traitement par l'emraclidine pour la schizophrénie.
- Diagnostic primaire de schizophrénie selon le Manuel diagnostique et statistique des troubles mentaux, Cinquième édition (DSM-5), tel que confirmé par le Mini International Neuropsychiatric Interview (MINI) for Psychotic Disorders.
- Participants stables sous antipsychotiques pendant au moins 3 mois au cours de l'année précédant le dépistage.
- Statut ambulatoire au moment de la signature du formulaire de consentement éclairé formulaire de consentement éclairé (ICF).
- Disposé à interrompre tous les médicaments interdits pour respecter les éliminations requises par le protocole avant et pendant la période d'essai.
- Capacité, de l'avis de l'investigateur, à comprendre la nature de l'essai, à participer aux visites d'essai et à se conformer aux exigences du protocole.
Critère d'exclusion:
Diagnostic actuel du DSM-5 autre que la schizophrénie (remarque : les symptômes d'anxiété secondaires à la schizophrénie sont autorisés.
- Les symptômes dépressifs aigus dans les 30 jours précédant la signature de l'ICF qui nécessitent un traitement avec un antidépresseur sont exclusifs.
- Les symptômes maniaques aigus dans les 30 jours précédant la signature de l'ICF qui nécessitent un traitement avec un stabilisateur de l'humeur sont exclusifs.
L'un des éléments suivants :
- La schizophrénie est considérée comme résistante/réfractaire au traitement antipsychotique par l'anamnèse (absence de réponse à 2 cycles ou plus de traitement pharmacologique adéquat défini comme une dose adéquate par étiquette et une durée de traitement d'au moins 4 semaines).
- Antécédents de réponse au traitement par la clozapine seule ou absence de réponse au traitement par la clozapine.
- Antécédents actuels ou passés de tumeurs malignes cardiovasculaires, pulmonaires, gastro-intestinales, rénales, hépatiques, métaboliques, génito-urinaires, endocriniennes (y compris le diabète sucré) (à l'exception du carcinome basocellulaire de la peau et du carcinome cervical in situ, à la discrétion de l'investigateur) , hématologique, immunologique, neurologique ou psychiatrique qui, de l'avis de l'investigateur ou du moniteur médical, pourrait compromettre la sécurité des participants ou les résultats de l'essai.
- Infection active du système nerveux central, maladie démyélinisante, maladie neurologique dégénérative, tumeur cérébrale, hospitalisation antérieure pour un traumatisme crânien grave, convulsions (à l'exclusion des convulsions fébriles de l'enfance) ou toute maladie du système nerveux central jugée évolutive au cours de l'essai pouvant confondre l'interprétation des résultats de l'essai
- Diagnostic d'un trouble modéré à grave lié à la consommation de substances ou d'alcool (à l'exclusion de la nicotine ou de la caféine) selon les critères du DSM-5 dans les 12 mois précédant la signature de l'ICF.
- Risque de comportement suicidaire tel qu'évalué par le C-SSRS et l'évaluation clinique de l'investigateur.
- Toute affection susceptible d'affecter l'absorption des médicaments, y compris, mais sans s'y limiter, les résections intestinales, la chirurgie de perte de poids bariatrique, l'anneau gastrique et la gastrectomie
- Utilisation de médicaments interdits avant la randomisation pendant la période de sevrage requise ou susceptibles de nécessiter un traitement concomitant interdit pendant l'essai.
- Résultats anormaux cliniquement significatifs à l'examen physique, à l'examen des antécédents médicaux, à l'ECG ou aux résultats de laboratoire clinique lors du dépistage.
- Résultat positif du test de grossesse avant de recevoir le médicament expérimental (IMP).
Remarque : les participantes qui sont enceintes, qui allaitent ou qui envisagent de devenir enceintes pendant le traitement IMP ou dans les 7 jours suivant la dernière dose d'IMP sont également exclues.

Participants who completed treatment with emraclidine in one of the double-blind, placebo-controlled Phase 2 efficacy trials (CVL-231-2001; NCT05227690 or CVL-231-2002; NCT05227703) were denoted as "active rollover" participants.
They received emraclidine 30 mg tablets orally once daily (QD) for 52 weeks in this study.

An adverse event (AE) is defined as any untoward medical occurrence in a patient or clinical investigation participant administered a pharmaceutical product which does not necessarily have a causal relationship with this treatment.
The investigator assesses the relationship of each event to the use of study drug.
A serious adverse event (SAE) is an event that results in death, is life-threatening, requires or prolongs hospitalization, results in a congenital anomaly, persistent or significant disability/incapacity or is an important medical event that, based on medical judgment, may jeopardize the participant and may require medical or surgical intervention to prevent any of the outcomes listed above.
Treatment-emergent adverse events/treatment-emergent serious adverse events (TEAEs/TESAEs) are defined as any event that began or worsened in severity on or after the first dose of study drug.

C-SSRS SI severity subscale ranges from 0 (no SI) to 5 (active SI with plan and intent).

Change from baseline for a given endpoint was defined as the value on a given Study Day (Time Point) minus the Baseline Value.
Negative changes from Baseline indicate an improvement in symptoms.

Change From Baseline in Abnormal Involuntary Movement Scale (AIMS) Movement Rating Score

The Abnormal Involuntary Movement Scale assessment consists of 10 items describing symptoms of dyskinesia. Facial and oral movements (items 1-4), extremity movements (items 5 and 6), and trunk movements (item 7) are observed unobtrusively while the participant is at rest, and the investigator also makes global judgments on the participant's dyskinesias (items 8-10). Each item is rated on a 5-point scale, with a score of 0 representing absence of symptoms (for item 10, no awareness), and a score of 4 indicating a severe condition (for item 10, awareness, severe distress). In addition, the AIMS includes 2 yes/no questions that address the participant's dental status. The AIMS Movement Rating Score is defined as the sum of individual scores from items 1-7, ranging from 0 to 28. A lower score indicates less severe or absent abnormal movements. A negative change in the mean from Baseline indicates improvement in the severity of abnormal involuntary movements. |

The BARS consists of 4 items related to akathisia.
The fourth item, reported here, is the Global Clinical Evaluation Score.
The Global Clinical Evaluation Score is evaluated using a 6-point scale, ranging from 0 to 5, with 0 representing absence of symptoms and a score of 5 representing severe akathisia.
A negative change from baseline indicates an improvement in symptoms.
