Description détaillée
Cette étude observationnelle de patients présentant des symptômes de gastroparésie (Gp) et de dyspepsie fonctionnelle (FD) avec une vidange gastrique retardée ou normale vise à évaluer les similitudes et les différences cliniques, physiopathologiques et psychologiques entre les patients atteints de Gp, FD.
Le résultat principal sera la mesure de la gravité des symptômes de la gastroparésie et de la dyspepsie fonctionnelle à l'aide de la variation du score total de l'évaluation des symptômes des troubles gastro-intestinaux supérieurs par le patient (PAGI SYM) entre le départ et 48 semaines.

Critère d'intégration:
- Avoir au moins 18 ans lors de la première visite de dépistage
- Symptômes de Gp et/ou FD d'une durée d'au moins 12 semaines avec divers degrés de nausées, vomissements, satiété précoce, plénitude postprandiale, douleurs abdominales, brûlures abdominales. Ainsi, les patients peuvent entrer dans le registre GpR4 principalement avec des douleurs abdominales suggérant un syndrome douloureux épigastrique FD.
- Réussite de la scintigraphie de vidange gastrique des solides en utilisant le protocole Egg Beaters® de 4 heures (ou un repas de blanc d'œuf liquide générique équivalent) au cours des 6 derniers mois
- Endoscopie haute négative ou séries radiographiques gastro-intestinales hautes négatives dans les 2 ans suivant l'enregistrement
Critère d'exclusion:
- Utilisation d'analgésiques narcotiques plus de trois jours par semaine
- Présence d'autres conditions pouvant expliquer les symptômes du patient tels que :
- Obstruction pylorique ou intestinale mise en évidence par l'EGD, l'UGI ou la TDM abdominale
- Maladie intestinale inflammatoire active
- Gastro-entérite à éosinophiles ou œsophagite à éosinophiles connue
- Affections neurologiques primaires pouvant provoquer des nausées et des vomissements, telles qu'une augmentation de la pression intracrânienne, une occupation de l'espace ou des lésions inflammatoires/infectieuses
- Insuffisance rénale aiguë ou chronique (créatinine sérique > 3 mg/dL) et/ou sous hémodialyse ou dialyse péritonéale
- Insuffisance hépatique aiguë ou maladie hépatique avancée (Child's B ou C ; un score de Child-Pugh-Turcotte (CPT) ≥ 7)
- Trouble pancréatique si présent sur l'imagerie pancréatique ou les tests de la fonction pancréatique
- Chirurgie gastrique antérieure, y compris résection gastrique totale ou sous-totale (presque complète), œsophagectomie, gastrojéjunostomie, pontage gastrique, manchon gastrique, pyloroplastie, pyloromyotomie ou toute fundoplication (Nissen, Tor)
- Toute autre condition qui, de l'avis de l'investigateur, pourrait expliquer les symptômes ou interférer avec les exigences de l'étude
- Les femmes qui sont enceintes. Un test de grossesse urinaire est systématiquement obtenu sur toutes les femmes immédiatement avant les procédures de vidange gastrique.
- Incapacité à se conformer ou à terminer le test de scintigraphie de vidange gastrique (y compris allergie aux œufs)
- Incapacité à obtenir un consentement éclairé

Mesure de la gravité des symptômes de la gastroparésie et de la dyspepsie fonctionnelle à l'aide de la variation du score total de l'évaluation par le patient des symptômes des troubles gastro-intestinaux supérieurs (PAGI SYM) entre le départ et 48 semaines.

Changement du score total PAGI-SYM de la ligne de base à 48 semaines. Le PAGI-SYM final contient 20 items regroupés en 6 sous-échelles couvrant : brûlures d'estomac/régurgitations (7 items), nausées/vomissements (3 items), sensation de satiété post-prandiale/satiété précoce (4 items), ballonnements (2 items), douleur abdominale haute (2 items) et douleur abdominale basse (2 items) . Une échelle de réponse de Likert en 6 points, allant de 0 (aucun) à 5 (très sévère), est utilisée pour évaluer la sévérité de chaque élément de symptôme sur une période de rappel de 2 semaines.

Modifications de la vidange gastrique, mesurées par scintigraphie de vidange gastrique (GES) au départ et à 48 semaines. Des valeurs normales pour la scintigraphie de vidange gastrique ont été établies dans des études antérieures utilisant des volontaires sains. La rétention gastrique de 99mTc > 60 % à 2 heures et/ou > 10 % à 4 heures est considérée comme la preuve d'un retard de la vidange gastrique des solides. |

Présence ou absence du signe de Carnett pour la douleur de la paroi abdominale au départ. Le signe de Carnett est une constatation à l'examen clinique dans laquelle la douleur abdominale (aiguë) reste inchangée ou augmente lorsque les muscles de la paroi abdominale sont tendus. Dans le cadre de l'examen abdominal, le patient est invité à lever la tête et les épaules de la table d'examen pour tendre les muscles abdominaux. Une alternative consiste à demander au patient de lever les deux jambes avec les genoux tendus. Un test positif indique la probabilité accrue que la paroi abdominale et non la cavité abdominale soit la source de la douleur (par exemple, en raison d'un hématome de la gaine du rectus au lieu d'une appendicite). On dit qu'un signe de Carnett négatif se produit lorsque la douleur abdominale diminue lorsqu'on demande au patient de lever la tête; cela indique une cause intra-abdominale de la douleur |
