Detaljeret beskrivelse
Denne observationsundersøgelse af patienter med symptomer på gastroparese (Gp) og funktionel dyspepsi (FD) med enten forsinket eller normal gastrisk tømning har til formål at vurdere de kliniske, patofysiologiske og psykologiske ligheder og forskelle mellem patienter med Gp, FD.
Det primære resultat vil være målingen af symptomsværhedsgraden af gastroparese og funktionel dyspepsi ved hjælp af ændringen i totalscore fra patientvurderingen af øvre gastrointestinale lidelser (PAGI SYM) fra baseline til 48 uger.

Beskrivelse
Inklusionskriterier:
- Alder mindst 18 år ved første screeningsbesøg
- Symptomer på Gp og/eller FD af mindst 12 ugers varighed med varierende grader af kvalme, opkastning, tidlig mæthed, postprandial fylde, mavesmerter, mavebrænding. Patienter kan således komme ind i GpR4-registret primært med mavesmerter, der tyder på FD-epigastrisk smertesyndrom.
- Vellykket afslutning af gastrisk tømningsscintigrafi af faste stoffer ved hjælp af 4-timers Egg Beaters®-protokollen (eller tilsvarende generisk flydende æggehvidemel) inden for de sidste 6 måneder
- Negativ øvre endoskopi eller øvre radiografisk GI-serie inden for 2 år efter registrering
Ekskluderingskriterier:
- Brug af narkotiske analgetika mere end tre dage om ugen
- Tilstedeværelse af andre tilstande, der kan forklare patientens symptomer såsom:
- Pylorisk eller intestinal obstruktion som påvist af EGD, UGI eller abdominal CT
- Aktiv inflammatorisk tarmsygdom
- Kendt eosinofil gastroenteritis eller eosinofil esophagitis
- Primære neurologiske tilstande, der kan forårsage kvalme og opkastning, såsom øget intrakranielt tryk, pladsoptagelse eller inflammatoriske/infektiøse læsioner
- Akut eller kronisk nyresvigt (serumkreatinin >3 mg/dL) og/eller i hæmodialyse eller peritonealdialyse
- Akut leversvigt eller fremskreden leversygdom (Child's B eller C; en Child-Pugh-Turcotte (CPT) score på ≥7)
- Bugspytkirtellidelse, hvis den er til stede ved billeddannelse af bugspytkirtlen eller bugspytkirtelfunktionstest
- Tidligere gastrisk kirurgi inklusive total eller subtotal (næsten fuldstændig) gastrisk resektion, esophagectomy, gastrojejunostomi, gastrisk bypass, gastrisk sleeve, pyloroplastik, pyloromyotomi eller enhver fundoplikation (Nissen, Tor)
- Enhver anden tilstand, som efter investigatorens mening kunne forklare symptomerne eller forstyrre undersøgelseskravene
- Kvinder, der er gravide. En uringraviditetstest udføres rutinemæssigt på alle kvinder umiddelbart før mavetømningsprocedurer.
- Manglende evne til at overholde eller fuldføre gastrisk tømningsscintigrafitest (inklusive allergi over for æg)
- Manglende evne til at indhente informeret samtykke

Ændring i PAGI-SYM totalscore fra baseline til 48 uger. Den endelige PAGI-SYM indeholder 20 emner grupperet i 6 underskalaer, der dækker: halsbrand/regurgitation (7 genstande), kvalme/opkastning (3 genstande), mæthed efter måltid/tidlig mæthed (4 genstande), oppustethed (2 genstande), smerter i øvre del af maven (2 genstande) og smerter i nedre mave (2 genstande) . Ændringer i gastrisk tømning, målt ved gastrisk tømningsscintigrafi (GES) ved baseline og 48 uger. Normale værdier for gastrisk tømningsscintigrafi er blevet fastlagt i tidligere undersøgelser med raske frivillige. Gastrisk retention på 99mTc > 60 % efter 2 timer og/eller > 10 % efter 4 timer anses for at være tegn på forsinket gastrisk tømning af faste stoffer. |

Tilstedeværelse eller fravær Carnetts tegn på mavevægssmerter ved baseline. Carnetts tegn er et fund ved klinisk undersøgelse, hvor (akutte) mavesmerter forbliver uændrede eller tiltager, når musklerne i bugvæggen spændes. Som en del af maveundersøgelsen bliver patienten bedt om at løfte hoved og skuldre fra undersøgelsesbordet for at spænde mavemusklerne. Et alternativ er at bede patienten hæve begge ben med lige knæ. En positiv test indikerer den øgede sandsynlighed for, at bugvæggen og ikke bughulen er kilden til smerten (for eksempel på grund af rectus sheath hæmatom i stedet for blindtarmsbetændelse). Et negativt Carnetts tegn siges at opstå, når mavesmerterne aftager, når patienten bliver bedt om at løfte hovedet; dette peger på en intraabdominal årsag til smerten |
