Gedetailleerde beschrijving
Deze observationele studie van patiënten met symptomen van gastroparese (Gp) en functionele dyspepsie (FD) met vertraagde of normale maagontlediging heeft tot doel de klinische, pathofysiologische en psychologische overeenkomsten en verschillen tussen patiënten met Gp, FD te beoordelen.
Het primaire resultaat is de maat voor de ernst van de symptomen van gastroparese en functionele dyspepsie met behulp van de verandering in de totale score van de Patient Assessment of Upper Gastrointestinal Disorders Symptomen (PAGI SYM) vanaf baseline tot 48 weken.

Beschrijving
Inclusiecriteria:
- Leeftijd minimaal 18 jaar bij het eerste screeningsbezoek
- Symptomen van Gp en/of FD die ten minste 12 weken aanhouden met variërende gradaties van misselijkheid, braken, vroege verzadiging, postprandiale volheid, buikpijn, brandend gevoel in de buik. Patiënten kunnen dus voornamelijk het GpR4-register binnenkomen met buikpijn die wijst op FD-Epigastrisch pijnsyndroom.
- Succesvolle afronding van de maagontledigingsscintigrafie van vaste stoffen met behulp van het 4-uurs Egg Beaters®-protocol (of gelijkwaardig generiek vloeibaar eiwitmeel) in de afgelopen 6 maanden
- Negatieve bovenste endoscopie of bovenste radiografische GI-serie binnen 2 jaar na registratie
Uitsluitingscriteria:
- Gebruik van narcotische analgetica meer dan drie dagen per week
- Aanwezigheid van andere aandoeningen die de symptomen van de patiënt kunnen verklaren, zoals:
- Pylorische of intestinale obstructie zoals blijkt uit EGD, UGI of Abdominale CT
- Actieve inflammatoire darmziekte
- Bekende eosinofiele gastro-enteritis of eosinofiele oesofagitis
- Primaire neurologische aandoeningen die misselijkheid en braken kunnen veroorzaken, zoals verhoogde intracraniale druk, ruimtebeslag of inflammatoire/infectieuze laesies
- Acuut of chronisch nierfalen (serumcreatinine >3 mg/dl) en/of hemodialyse of peritoneaaldialyse ondergaan
- Acuut leverfalen of gevorderde leverziekte (Child's B of C; een Child-Pugh-Turcotte (CPT)-score van ≥7)
- Pancreasaandoening indien aanwezig op pancreasbeeldvorming of pancreasfunctietesten
- Eerdere maagchirurgie, inclusief totale of subtotale (bijna volledige) maagresectie, slokdarmresectie, gastrojejunostomie, gastric bypass, gastric sleeve, pyloroplastiek, pyloromyotomie of enige fundoplicatie (Nissen, Tor)
- Elke andere aandoening die naar de mening van de onderzoeker de symptomen zou kunnen verklaren of de studievereisten zou kunnen verstoren
- Vrouwtjes die zwanger zijn. Een urine-zwangerschapstest wordt routinematig verkregen bij alle vrouwtjes onmiddellijk voorafgaand aan maagledigingsprocedures.
- Onvermogen om de maagledigingsscintigrafietest uit te voeren of te voltooien (inclusief allergie voor eieren)
- Onvermogen om geïnformeerde toestemming te verkrijgen

Maatstaf voor de ernst van de symptomen van gastroparese en functionele dyspepsie met behulp van de verandering in de totale score van de Patient Assessment of Upper Gastrointestinal Disorders Symptomen (PAGI SYM) vanaf baseline tot 48 weken.

Verandering in PAGI-SYM-totaalscore vanaf baseline tot 48 weken. De uiteindelijke PAGI-SYM bevat 20 items gegroepeerd in 6 subschalen die betrekking hebben op: brandend maagzuur/oprispingen (7 items), misselijkheid/braken (3 items), postprandiale volheid/vroege verzadiging (4 items), opgeblazen gevoel (2 items), pijn in de bovenbuik (2 items) en pijn in de onderbuik (2 items) . Een 6-punts Likert-responsschaal, variërend van 0 (geen) tot 5 (zeer ernstig), wordt gebruikt om de ernst van elk symptoomitem te beoordelen gedurende een terugroepperiode van 2 weken.

Veranderingen in maaglediging, zoals gemeten met maagledigingsscintigrafie (GES) bij aanvang en na 48 weken. Normale waarden voor maagledigingsscintigrafie zijn vastgesteld in eerdere studies met gezonde vrijwilligers. Maagretentie van 99mTc > 60% na 2 uur en/of > 10% na 4 uur wordt beschouwd als bewijs van vertraagde maaglediging van vaste stoffen. |

Aanwezigheid of afwezigheid Carnett-teken voor buikwandpijn bij aanvang. Het teken van Carnett is een bevinding bij klinisch onderzoek waarbij (acute) buikpijn onveranderd blijft of toeneemt wanneer de spieren van de buikwand aangespannen zijn. Als onderdeel van het abdominaal onderzoek wordt de patiënt gevraagd het hoofd en de schouders van de onderzoekstafel te tillen om de buikspieren te spannen. Een alternatief is om de patiënt te vragen beide benen met gestrekte knieën op te heffen. Een positieve test geeft de verhoogde kans aan dat de buikwand en niet de buikholte de bron van de pijn is (bijvoorbeeld door hematoom van de rectusschede in plaats van blindedarmontsteking). Er wordt gezegd dat een negatief Carnett-teken optreedt wanneer de buikpijn afneemt wanneer de patiënt wordt gevraagd het hoofd op te tillen; dit wijst op een intra-abdominale oorzaak van de pijn |
