Utilisation d'un BPA pour promouvoir des soins de grande valeur dans la bronchiolite: une étude contrôlée randomisée
L'objectif de cette étude expérimentale est de savoir si différents types de conseils de meilleures pratiques (BPAS) qui dirigent les cliniciens pour référencer les directives cliniques intégrées dans le dossier de santé électronique (DSE) augmentent la prestation de soins fondés sur des preuves chez les enfants se présentant à l'hôpital atteint de bronchiolite. Les principales questions auxquelles il vise à répondre est:
- Les BPA améliorent-ils la prestation des cliniciens des soins condamnés par les directives dans la bronchiolite?
- Les BPA interrupteurs améliorent-ils plus les soins condamnés aux directives de la bronchiolite que les BPAS non interruptifs?
Les chercheurs compareront le traitement et les résultats des patients dont les cliniciens n'ont pas reçu de BPA, à ceux dont les cliniciens ont reçu un BPA non interrompu, à ceux dont les cliniciens ont reçu un BPA interrupteur.
Les patients continueront de recevoir des soins hospitaliers standard pour la bronchiolite.
Les cliniciens seront:
- conserver l'accès à une directive clinique endettée du DSE pour les soins de bronchiolite
- être exposé à aucun BPA, à un BPA non interrompu ou à un BPA interruptif favorisant la directive clinique endettée du DSE (Randomizme par patient rencontre)

Description détaillée
La bronchiolite, une maladie respiratoire virale affectant les nourrissons et les tout-petits, est la raison la plus courante pour laquelle les enfants de moins de 2 ans sont hospitalisés. Cependant, de nombreux enfants admis avec une bronchiolite recevaient des traitements inutiles qui ne font pas la récupération et peuvent même nuire. Malgré la preuve que le meilleur traitement pour la bronchiolite est des soins favorables (c'est-à-dire. Aide à l'oxygène et à l'hydratation), les cliniciens continuent de surutiliser certaines thérapies, ce qui entraîne des séjours à l'hôpital plus longs, des coûts plus élevés et un stress accru pour les familles.
Les chercheurs étudieront les effets des BPAS, qui sont des alertes en temps réel au sein du DSE. Les BPAS de cette étude favorisent l'utilisation d'une directive de soins fondée sur des preuves pour la bronchiolite qui est ancrée dans le DSE. Il existe plusieurs conceptions de BPA couramment utilisées dans les DSE: 1) BPAS interrupteurs, qui obligent les cliniciens à interagir avec l'alerte pour continuer le flux de travail du clinicien; et 2) des BPAS non interruptifs, qui apparaissent comme des indices visuels mais ne nécessitent pas que les cliniciens modifient les flux de travail.

La description
Critères d'inclusion des patients:
- Des enfants <= 24 mois se présentant aux urgences et / ou hospitalisés (sous observation ou statut d'hospitalisation) atteints de bronchiolite sur l'un des trois sites d'étude.
Critères d'exclusion des patients:
- La rencontre actuelle est la naissance
- Actuellement hospitalisé dans une USI
- Hospitalisé avec durée de séjour> 14 jours
Critères d'inclusion des cliniciens:
- Médecins et prestataires de pratique avancée entrant dans le graphique du patient ou les commandes d'activité d'entrée pour le service d'urgence (ed) et les rencontres pour patients hospitalisés
Critères d'exclusion des cliniciens:
- Médecins et prestataires de pratiques avancées qui n'entrent pas dans le tableau du patient ou l'activité d'entrée des commandes

Aucune intervention: No EHR-Alert
Clinicians will not be exposed to a any EHR-alert about the care of bronchiolitis.
The participants will still have access to the EHR-embedded, evidence-based clinical guideline.

Expérimental: Interruptive EHR-Alert
Clinicians will be exposed to an interruptive EHR-alert directing the clinician to reference the EHR-embedded, evidence-based clinical guideline for the care of bronchiolitis.

The interruptive EHR alert requires clinicians to interact with the alert to continue the workflow.
The alert directs clinicians to reference the EHR-embedded, evidence-based clinical guideline for the care of bronchiolitis.

Expérimental: Non-interruptive EHR-Alert
Clinicians will be exposed to an non-interruptive EHR-Alert directing the clinician to reference the EHR-embedded, evidence-based clinical guideline for the care of bronchiolitis.

The non-interruptive EHR alert appears as a visual cue but does not require clinicians to alter workflows.
The alert directs clinicians to reference the EHR-embedded, evidence-based clinical guideline for the care of bronchiolitis.
