Uso de un BPA para promover una atención de alto valor en la bronquiolitis: un estudio controlado aleatorio
El objetivo de este estudio experimental es saber si los diferentes tipos de avisos de mejores prácticas (BPA) que dirigen a los médicos a hacer referencia a las pautas clínicas integradas en el registro electrónico de salud (EHR) aumentan la entrega de atención basada en evidencia en niños que se presentan al hospital con bronquiolitis. Las principales preguntas que pretende responder son:
- ¿Los BPA mejoran la entrega de la atención de la guía-concordante de los médicos en la bronquiolitis?
- ¿Los BPA interruptivos mejoran la atención de bronquiolitis de guía más que los BPA no interruptivos?
Los investigadores compararán el tratamiento y los resultados de los pacientes cuyos médicos no recibieron un BPA, con aquellos cuyos médicos recibieron un BPA no interruptivo, a aquellos cuyos médicos recibieron un BPA interrumpido.
Los pacientes continuarán recibiendo atención hospitalaria estándar por bronquiolitis.
Los médicos lo harán:
- Retener el acceso a una guía clínica embebida en EHR para la atención de bronquiolitis
- estar expuesto a no BPA, un BPA no interruptivo o un BPA interrumpido que promueva la guía clínica embebida con EHR (aleatorizado por encuentro con el paciente)

La bronquiolitis, una enfermedad respiratoria viral que afecta a los bebés y niños pequeños, es la razón más común por la que los niños de menos de 2 años son hospitalizados. Sin embargo, muchos niños ingresados con bronquiolitis reciben tratamientos innecesarios que no aceleran la recuperación e incluso pueden causar daño. A pesar de la evidencia de que el mejor tratamiento para la bronquiolitis es cuidados de apoyo (es decir, Soporte de oxígeno e hidratación), los médicos continúan utilizando en exceso ciertas terapias, lo que lleva a estadías hospitalarias más largas, mayores costos y mayor estrés para las familias.
Los investigadores estudiarán los efectos de los BPA, que son alertas en tiempo real dentro del EHR. Los BPA en este estudio promueven el uso de una guía de atención basada en evidencia para la bronquiolitis que está integrada dentro del EHR. Hay múltiples diseños de BPA comúnmente utilizados en EHR: 1) BPA interruptivos, que requieren que los médicos interactúen con la alerta para continuar con el flujo de trabajo del clínico; y 2) BPA no interruptivos, que parecen señales visuales pero no requieren que los médicos alteren los flujos de trabajo.

Descripción
Criterios de inclusión del paciente:
- Niños <= 24 meses de edad que se presentan en la sala de emergencias y/o hospitalizados (bajo observación o estado hospitalario) con bronquiolitis en uno de los tres sitios de estudio.
Criterios de exclusión del paciente:
- El encuentro actual es una apariencia de nacimiento
- Actualmente hospitalizado en una UCI
- Hospitalizado con duración de estadía> 14 días
Criterios de inclusión clínica:
- Los médicos y los proveedores de práctica avanzada que ingresan a la tabla del paciente o la actividad de entrada de órdenes para el departamento de emergencias (DE) y los encuentros hospitalarios

Clinicians will not be exposed to a any EHR-alert about the care of bronchiolitis.
The participants will still have access to the EHR-embedded, evidence-based clinical guideline.

Experimental: Interruptive EHR-Alert
Clinicians will be exposed to an interruptive EHR-alert directing the clinician to reference the EHR-embedded, evidence-based clinical guideline for the care of bronchiolitis.

The interruptive EHR alert requires clinicians to interact with the alert to continue the workflow.
The alert directs clinicians to reference the EHR-embedded, evidence-based clinical guideline for the care of bronchiolitis.

Experimental: Non-interruptive EHR-Alert
Clinicians will be exposed to an non-interruptive EHR-Alert directing the clinician to reference the EHR-embedded, evidence-based clinical guideline for the care of bronchiolitis.

The non-interruptive EHR alert appears as a visual cue but does not require clinicians to alter workflows.
The alert directs clinicians to reference the EHR-embedded, evidence-based clinical guideline for the care of bronchiolitis.
