Gebruikmakend van elektronische hulpmiddelen voor gezondheidsdossiers om de evidence-based behandeling van kinderen in het ziekenhuis met bronchiolitis te verbeteren
Gebruik van een BPA om hoge waardezorg bij bronchiolitis te bevorderen: een gerandomiseerde gecontroleerde studie
Het doel van dit experimentele onderzoek is om te leren of verschillende soorten best practice-adviezen (BPA's) die clinici leiden om te verwijzen naar klinische richtlijnen ingebed in het Electronic Health Decord (EPD) de levering van evidence-based zorg bij kinderen die zich presenteren aan het ziekenhuis met bronchiolitis. De belangrijkste vragen die het wil beantwoorden zijn:
- Verbeteren BPA's de levering door clinici van richtlijnconcordante zorg bij bronchiolitis?
- Verbetert interruptieve BPA's de richtlijn-conforderende zorg voor bronchiolitis meer dan niet-onderbroken BPA's?
Onderzoekers zullen de behandeling en resultaten vergelijken van patiënten wier clinici geen BPA hebben ontvangen, met degenen wier clinici een niet-onderbroken BPA ontvingen, met degenen wier clinici een interruptieve BPA ontvingen.
Patiënten blijven standaard ziekenhuiszorg voor bronchiolitis ontvangen.
Artsen zullen:
- Toegang behouden tot een EHR-ingebedde klinische richtlijn voor bronchiolitiszorg
- worden blootgesteld aan geen BPA, een niet-onderbroken BPA of een interruptieve BPA die de EHR-ingebedde klinische richtlijn bevordert (gerandomiseerd per ontmoeting per patiënt)

Bronchiolitis, een virale ademhalingsziekte die baby's en peuters treft, is de meest voorkomende reden dat kinderen van minder dan 2 jaar in het ziekenhuis worden opgenomen. Veel kinderen die zijn opgenomen met bronchiolitis ontvangen echter onnodige behandelingen die herstel niet bespoedigen en zelfs schade kunnen veroorzaken. Ondanks bewijs dat de beste behandeling voor bronchiolitis ondersteunende zorg is (d.w.z. Zuurstof- en hydratatieondersteuning), blijven clinici bepaalde therapieën overmatig gebruiken, wat leidt tot langere ziekenhuisopnames, hogere kosten en verhoogde stress voor gezinnen.
De onderzoekers zullen de effecten van BPA's bestuderen, die realtime meldingen zijn binnen de EPD. De BPA's in deze studie bevorderen het gebruik van een evidence-based zorgrichtlijn voor bronchiolitis die is ingebed in de EPD. Er zijn meerdere BPA -ontwerpen die gewoonlijk worden gebruikt in EHR's: 1) onderbrekingsbpas, waarbij clinici moeten communiceren met de waarschuwing om de workflow van de arts voort te zetten; en 2) niet-onderbroken BPA's, die verschijnen als visuele aanwijzingen maar dat clinici geen workflows veranderen.

Beschrijving
Criteria voor de inclusie van de patiënt:
- Kinderen <= 24 maanden oud presenteren zich aan de eerste hulp en/of in het ziekenhuis (onder observatie of intramurale status) met bronchiolitis op een van de drie onderzoekslocaties.
Criteria van de uitsluiting van de patiënt:
- De huidige ontmoeting is geboorte
- Momenteel in het ziekenhuis opgenomen in een ICU
- In het ziekenhuis opgenomen met verblijfsduur> 14 dagen
Inclusiecriteria voor arts:
- Artsen en geavanceerde praktijkaanbieders die de grafiek van de patiënt betreden of de ingang van de invoeractiviteiten voor spoedeisende hulp (ED) en intramurale ontmoetingen
Criteria voor clinicus uitsluiting:
- Artsen en geavanceerde praktijkaanbieders die niet in de grafiek van de patiënt komen of de ingangsbedrijven bestellingen

Clinicians will be exposed to an interruptive EHR-alert directing the clinician to reference the EHR-embedded, evidence-based clinical guideline for the care of bronchiolitis.

The interruptive EHR alert requires clinicians to interact with the alert to continue the workflow.
The alert directs clinicians to reference the EHR-embedded, evidence-based clinical guideline for the care of bronchiolitis.

Experimenteel: Non-interruptive EHR-Alert
Clinicians will be exposed to an non-interruptive EHR-Alert directing the clinician to reference the EHR-embedded, evidence-based clinical guideline for the care of bronchiolitis.

The non-interruptive EHR alert appears as a visual cue but does not require clinicians to alter workflows.
