Ez egy javaslat a melatonin (MT) és az autizmus spektrumzavarban (ASD) szenvedő gyermekek alvási problémái közötti kapcsolat tanulmányozására, az Autizmus Kezelési Hálózat (ATN) együttműködési kutatási struktúrájának részeként. Az ATN fő célja olyan klinikai kutatások elvégzése, amelyek jelentős hatással lesznek az ASD-vel élő egyének mindennapi életére és működésére, valamint a szülők azonnali aggályaira. Az ASD-vel küzdő gyermekek magas arányban tapasztalnak alvászavarokat, amelyek valószínűleg hozzájárulnak nappali kognitív és viselkedési zavaraik súlyosságához, valamint a számukra és családjaik életminőségének romlásához.
Az ASD alvásproblémák kezelése felé tett lépésként javasoljuk annak a hipotézisnek a tesztelését, miszerint az ASD-vel és alvásproblémákkal küzdő gyermekeknél késik az MT megjelenése és/vagy 24 órán keresztül csökkent MT-szekréció a normál kontrollokhoz képest.

Azt jósoljuk, hogy ennek a tanulmánynak az eredményei a metabolizált MT alacsonyabb szintjét fogják feltárni az ASD-ben szenvedő gyermekeknél, mint a normál gyermekeknél. Ezen túlmenően arra számítunk, hogy az ASD-ben szenvedő gyermekeknél késleltetett MT kezdődik vagy megváltozik a cirkadián fázis.
A tanulmányból származó adatok fontos információkkal szolgálnak az ASD cirkadián ritmus-rendellenességével kapcsolatban, és támogatják a jövőbeni tanulmányok kidolgozását, amelyek MT-t használnak a kóros cirkadián ritmusok módosítására és korrigálására.

Bevételi kritériumok:
- Autizmus spektrum zavar diagnózisa; támogatja az ADOS és az ADI vagy SCQ (tárgyak).
- 4 éves vagy annál nagyobb életkor, vagy 9 év vagy annál kisebb;
- A szülők tájékozott beleegyezést adtak.
- Szülő/gondozó folyékonyan beszél angolul írásban és szóban.
- Csak kontrollok: A normál gyermekek meghatározásához 10-nél kisebb SCQ-pontszámot használnak, anélkül, hogy a szülő vagy az orvos aggódna egy másik idegrendszeri fejlődési rendellenesség miatt.
Kizárási kritériumok:
- A pszichoaktív gyógyszerek jelenlegi használata (pl. fluoxetin, metilfenidát, riszperidon, lítium stb.)*;
- béta-blokkolók vagy melatonin jelenlegi vagy az elmúlt 1 hónapban történő használata;
- Alvássegítők jelenlegi használata;
- Kezeletlen egészségügyi problémák jelenléte, amelyek egyébként megmagyarázhatnák az alvási problémákat (pl. obstruktív alvási apnoe, gastrooesophagealis reflux betegség – GERD);
- Vakság.
Csak ellenőrzések: ADHD, depresszió, szorongás vagy bármilyen más pszichiátriai állapot jelenlegi vagy korábbi diagnózisa. (7) Csak kontrollok: A testvérnél autista spektrum zavart diagnosztizáltak.
- A pszichoaktív gyógyszereket meg lehet szakítani, de a szülőknek meg kell beszélniük a gyógyszer abbahagyását felíró orvosukkal, mielőtt csökkentenék vagy abbahagynák.
