L'EFFETTO DELL'ALLENAMENTO CON ESERCIZI PROPRIOCETTIVI SUI LIVELLI DI PROPRIOCEZIONE CERVICALE, CHINESIOFOBIA E CATASTROFIZZAZIONE DEL DOLORE IN INDIVIDUI GERIATRICI SARCOPENICI CHE RISIEDONO IN CASE DI CURA
L'EFFETTO DELL'ALLENAMENTO CON ESERCIZI PROPRIOCETTIVI SUI LIVELLI DI PROPRIOCEZIONE CERVICALE, CHINESIOFOBIA E CATASTROFISMO DEL DOLORE NEI SOGGETTI GERIATRICI SARCOPENICI CHE RISIEDONO IN CASE DI RIPOSO
Questo studio ha indagato gli effetti dell'allenamento con esercizi propriocettivi cervicali sulla propriocezione cervicale, la chinesiofobia e la catastrofizzazione del dolore in residenti di case di cura sarcopenici.
In questo studio randomizzato controllato, 63 residenti di case di cura sono stati sottoposti a screening per la sarcopenia secondo i criteri EWGSOP2, che includono valutazioni della forza di presa (dinamometro Jamar), della performance fisica (test del cammino di 4 metri) e della massa muscolare valutata tramite Analisi di Impedenza Bioelettrica. Trentuno residenti diagnosticati con sarcopenia sono stati assegnati casualmente a un gruppo di controllo (n = 17), che ha completato 12 sessioni supervisionate di esercizi convenzionali (esercizi posturali, di escursione articolare cervicale ed esercizi isometrici) in sei settimane, o a un gruppo sperimentale (n = 14), che ha ricevuto ulteriori 12 sessioni di esercizi propriocettivi cervicali. Entrambi i gruppi sono stati valutati prima e dopo l'intervento. Il senso di posizione articolare cervicale, la chinesiofobia, la catastrofizzazione del dolore, sono stati valutati utilizzando il Test dell'Errore di Posizione Articolare; la Scala di Chinesiofobia di Tampa; la Scala di Catastrofizzazione del Dolore.

Descrizione
Criteri di inclusione;
- Avere 65 anni di età o più,
- Essere diagnosticati con sarcopenia,
- Avere almeno 91 punti o più secondo l'indice delle attività quotidiane di Barthel (essere leggermente dipendenti o completamente indipendenti),
- Avere 24 punti o più secondo la mini valutazione mentale, che è considerata mentalmente sana.
Criteri di esclusione;
- Avere una disabilità (come arti, vista, perdita dell'udito),
- Avere un grave disturbo neurologico, ortopedico o reumatologico che può influenzare la propriocezione,
- Avere ipertensione/diabete non controllati,
- Avere dolore alla colonna vertebrale e/o agli arti,
- Avere una diagnosi di malattia psichiatrica.

Valutazione della propriocezione cervicale

La propriocezione cervicale è stata valutata utilizzando il test dell'errore di posizione articolare.
Da seduto, il partecipante era rivolto verso una tavola posizionata a 90 cm di distanza all'altezza della testa, costituita da cerchi concentrici (diametro 40 cm, incrementi di 1 cm), e indossava un berretto con un puntatore laser montato. Il partecipante prima allineava il laser con il punto centrale per stabilire la posizione neutra, poi - con gli occhi aperti - veniva guidato attraverso flessione, estensione, rotazione destra/sinistra e flessione laterale destra/sinistra, ritornando alla posizione neutra dopo ogni movimento.
Gli stessi sei movimenti venivano poi eseguiti attivamente con gli occhi chiusi.
Sono state completate sei ripetizioni per ogni direzione, e il punto in cui il laser si fermava veniva segnato sulla tavola dopo ogni prova.
La media di questi punti è stata calcolata, e la deviazione angolare dal bersaglio è stata determinata utilizzando il metodo dell'arcotangente basato sulla distanza fissa dalla tavola.
Una deviazione superiore a 4 gradi è stata considerata indicativa di una compromissione della propriocezione.

La chinesiofobia è stata valutata utilizzando la Scala delle Cause della Chinesiofobia (KCS).
Sviluppata da Kocjan e Knapik (2014), la scala identifica le fonti biologiche e psicologiche della paura del movimento e contribuisce a comprendere le cause sottostanti della ridotta attività motoria.
Comprende due domini: biologico e psicologico.
Il dominio biologico include fattori morfologici, requisiti di movimento, risorse energetiche, pulsioni biologiche, esperienze negative, competenza motoria e convinzioni relative alla cura del corpo, mentre il dominio psicologico valuta fattori come la fiducia, l'autoefficacia e l'interazione sociale che contribuiscono al comportamento di evitamento.
Lo studio di validità e affidabilità turco è stato condotto da Çayır et al. (2020).
La scala è composta da 20 elementi valutati su una scala Likert a 5 punti (1 = fortemente in disaccordo, 5 = fortemente d'accordo).
Punteggi più alti indicano una maggiore paura del movimento.

La catastrofizzazione del dolore è stata valutata utilizzando la Pain Catastrophizing Scale (PCS), una misura di 13 item che valuta pensieri e sentimenti disadattivi relativi al dolore.
La scala include tre sottodimensioni: impotenza, amplificazione e ruminazione.
Ogni item è valutato su una scala da 0 a 4 e i punteggi delle sottoscale si ottengono sommando gli item corrispondenti; il punteggio totale della PCS è calcolato come la somma di tutti gli item.
Lo studio di validità e affidabilità turco è stato condotto da Uğurlu et al. (2017).
I punteggi totali variano da 0 a 52, con punteggi più alti che indicano una maggiore catastrofizzazione del dolore. Punteggi superiori a 30 riflettono livelli clinicamente significativi di catastrofizzazione.
