Test da sforzo sull'angiotensina II. Cinetica della renina durante il trattamento dello shock vasoplegico con angiotensina II in relazione alla risposta emodinamica al trattamento con angiotensina II.
Lo shock è una condizione pericolosa per la vita che può causare insufficienza multiorgano e persino la morte.
Una caratteristica dello shock è la bassa pressione sanguigna che viene gestita con farmaci chiamati vasopressori.
I vasopressori più frequentemente utilizzati sono la noradrenalina, la vasopressina e recentemente anche l'angiotensina II.
L'angiotensina II è presente nell'organismo e ha un ruolo fisiologico nel mantenimento della pressione sanguigna nelle persone sane.
La renina è un enzima e un fattore chiave nella produzione di angiotensina II nel corpo.
Nei pazienti con shock si verifica una carenza di angiotensina II e un eccesso di renina nel corpo.
In base alla letteratura, la renina può potenzialmente essere un indicatore della gravità dello shock.
L'angiotensina II sintetica viene utilizzata nei pazienti con shock nei quali non è possibile normalizzare la pressione sanguigna con noradrenalina e vasopressina.
Per quanto riguarda i dati scientifici, l'uso dell'angiotensina II sintetica riduce la dose di noradrenalina e vasopressina e l'incidenza del danno renale acuto.
Lo scopo del nostro studio è scoprire quale sia la relazione tra la concentrazione di renina prima e 6 ore dopo l'inizio dell'uso dell'angiotensina II in pazienti con shock e il loro esito clinico.
Poiché non tutti i pazienti con shock rispondono all'angiotensina II, lo scopo del nostro studio è anche quello di scoprire quali pazienti potrebbero trarre maggiori benefici dall'angiotensina II sintetica.

Descrizione dettagliata
Nei pazienti con shock distributivo (≥18 anni) con noradrenalina >0,3 mcg/kg/min e vasopressina >0,03 IE/min che non raggiungono una pressione arteriosa media adeguata (65-85 mmHg) verrà iniziata un'infusione di angiotensina II a 20 ng/kg/min e successivamente aggiustato a una dose massima di 40 ng/kg/min, se necessario.
Prima dell'infusione e 6 ore dopo l'inizio dell'infusione di angiotensina II verrà prelevato un campione di sangue per determinare la concentrazione di renina.
L'esito primario saranno i giorni liberi da insufficienza d'organo e i giorni liberi in terapia intensiva.
L'esito secondario sarà la necessità di vasopressori, dialisi, ventilazione meccanica, andamento della concentrazione di renina.

Pazienti (≥ 18 anni) con shock distributivo (durata <72 ore) indipendentemente dall'eziologia, senza limitazioni di trattamento e con una sopravvivenza prevista >24 ore.
Descrizione
Criterio di inclusione:
- pazienti con shock distributivo della durata < 72 ore
- un obiettivo di pressione arteriosa media (65-85 mmHg) non raggiunto nonostante un'infusione di almeno noradrenalina 0,3 mcg/kg/min e vasopressina 0,03 IE/min
- il paziente non aveva ricevuto angiotensina II in precedenza
- la sopravvivenza prevista è >24 ore
- nessuna limitazione per il trattamento attivo
Criteri di esclusione:
- brucia >20% dell'area corporea
- sindrome coronarica acuta
- broncospasmo
- malattia epatica (MELD ≥30)
- sanguinamento acuto grave (necessità di 4 o più unità di eritrociti concentrati)
- ischemia mesenterica acuta
- dissezione aortica
- leucopenia <1000/mm3
- gravidanza
- Malattia di Raynaud, sclerosi sistemica, malattia vasospastica
- la necessità di una dose giornaliera di idrocortisone pari o superiore a 500 mg
- ECMO
