Una sperimentazione clinica sulla somministrazione della terapia della luce per prevenire e curare le ulcere della bocca nei bambini affetti da cancro (PBM)
Terapia di fotobiomodulazione per la prevenzione e il trattamento della mucosite nei pazienti oncologici pediatrici
L'obiettivo di questo studio clinico è scoprire se la terapia della luce può prevenire e/o curare le afte nella bocca nei bambini malati di cancro. Le principali domande a cui si propone di rispondere sono:
È ragionevole e accettabile fornire la terapia della luce ai bambini malati di cancro?
La terapia della luce previene e cura le ulcere della bocca legate alle cure mediche?
I ricercatori confronteranno i bambini che non hanno ricevuto la terapia della luce prima della sperimentazione clinica con i bambini che la ricevono durante la sperimentazione clinica per vedere se la terapia della luce aiuta a prevenire e curare la mucosite.
I partecipanti:
- Comunica all'infermiera il punteggio del dolore, correlato alle ulcere alla bocca, prima di ricevere la terapia della luce.
- Fatti fotografare la bocca per cercare eventuali ulcere alla bocca.
- Ricevi la terapia della luce a giorni alterni durante il ricovero in ospedale nell'unità oncologica o durante il ricovero in un'unità alternativa.

Descrizione
Criteri di inclusione:
- Ricoverato in unità di oncologia, PICU o in un'unità alternativa di terapia intensiva.
- Diagnosi:
- Dopo la diagnosi fino al recupero della conta (ANC≥500), il paziente non presenta segni clinici di mucosite o viene dimesso. TUTTI Metotrexato ad alta dose di mantenimento ad interim ii. TUTTI con trisomia 21 trattati con metotrexato ad alte dosi iii. antiriciclaggio iv. Neuroblastoma (esclusi ricoveri per terapia anticorpale) v. Linfoma di Burkitt vi. Osteosarcoma trattato con metotrexato ad alte dosi vii. Tumori a cellule germinali trattati con Etoposide viii. Rabdomiosarcoma ix. Sarcoma di Ewing
- Tumori della testa/collo sottoposti a radiazioni
- In grado di parlare e comprendere l'inglese o lo spagnolo

|
Light therapy will be administered using the cluster probe on the external cheeks bilaterally and intraorally using the lollipop probe for patients ≥7 years of age. Patients <7 years of age will not receive intraoral treatment with the lollipop. Instead, they will be asked to open their mouth while the covered cluster probe is held externally to treat their intraoral membranes. Each treatment (both external cheeks and intraoral buccal region) will be administered over 1 min with a Modulation Frequency 2.5Hz, Skin Conduction nS 001, and Beam Power mW 000, for a total treatment time of 3 minutes. Patients will continue to receive light therapy in the home setting through day +8 of their chemotherapy cycle using the Luminance Device.

Feasibility of PBM will be assessed by the RN completing a survey using a 5-point Likert scale (5 being strongly agree and 1 being strongly disagree) to evaluate the ease of delivery of PBM therapy.
A score of ≥3 indicates that the PBM therapy was feasible.

Evaluate the acceptability of using PBM in pediatric oncology patients.

Acceptability of PBM will be assessed by the patient/caregiver completing a survey using a 5-point Likert scale (5 being strongly agree and 1 being strongly disagree) to evaluate the patient's tolerance of the treatment.
A score of ≥3 indicates acceptability of PBM therapy.

Evaluate the effectiveness of using PBM in reducing the incidence and severity of mucositis in pediatric oncology patients receiving chemotherapy.
Lasso di tempo: From date of enrollment until date of last PBM treatment, assessed up to 2 years.

This will be assessed by the study team through analysis of retrospective and prospective data entered into the electronic health record based on the number of unique patients with the value of "mucositis/stomatitis" in the oral mucosa flowsheet row.
Additionally, mucositis onset, severity, and grade will be measured using the CTCAE grading scale and the ChIMES patient/caregiver survey
