包含基準:
以下の症状の急性発症:
- PaO2/FiO2が300以下。
- 胸部正面 X 線写真では両側の浸潤が肺水腫と一致しています。
- 気管内チューブまたは気管切開による陽圧換気の要件。
- 肺浸潤を説明できるような左心房高血圧症の臨床的証拠はありません。 測定した場合、肺動脈楔入圧は 18 mmHg 以下。
除外基準:
- 年齢は18歳未満。
- すべての包含基準が最初に満たされてから 24 時間を超えている。
- C5 以上の脊髄損傷、筋萎縮性側索硬化症、ギラン・バレー症候群、重症筋無力症、脊柱側弯症など、補助なしで換気する能力を損なう神経筋疾患(付録 I.A を参照)。
- 妊娠(妊娠の可能性がある女性には妊娠検査薬が陰性であることが必要)。
- 重度の慢性呼吸器疾患(付録 I.C を参照)。
- 重度の慢性肝疾患 (Child-Pugh 11 ~ 15、付録 I.E を参照)
- 体重 > 160 kg。
- 体表面積の 70% 以上を火傷します。
- 6 か月死亡率が 50 % を超えると推定される悪性腫瘍またはその他の不可逆的な疾患または症状 (付録 I.A を参照)。
- 既知の心臓または血管動脈瘤。
- 大腿動脈穿刺に対する禁忌 - 血小板 < 30、両側大腿動脈移植、INR > 3.0。
- 完全なサポートを約束するものではありません。
- 30日以内に他の治験薬の治験に参加している。
- 静脈内ラシックスまたはそのキャリアの成分に対するアレルギー。
- -重度のCHFの病歴 - NYHAクラス≧III、以前に記録されたEF < 30%。
- びまん性肺胞出血。
- 反応性気道疾患の存在(反応性気道疾患は、疾患を制御するためのMDIおよびステロイドの最近の使用頻度と量に基づいて定義されます)。

アクティブコンパレータ:修正された FACTT (コントロール)
研究者の対照群は、FACTT 試験で使用されたプロトコルに基づいて、ARDSnet グループによって公開される、ALI 患者の輸液の保存的管理のための簡略化されたアルゴリズムで構成されています。
このプロトコールでは、ARDSnet 換気、ウィニングプロトコール、および選択された血管作動性ベータアドレナリン作動薬のみの使用を厳守することが求められています。これらの治療法にばらつきがあると、結果が大きく混乱する可能性があると考えられているからです。
イプラトロピウムに反応しない生命を脅かす気管支けいれんを除いて、アルブテロール投与はどちらの腕でも許可されません。
イプラトロピウムは、気管支けいれんに対して治療医師の裁量で投与される場合があります。
PiCCO を各対照患者に配置し、データを 1 日 2 回記録します。
治療を担当する医師はこのデータを知らされません。

CVPが測定された患者に使用される
10 ml/kg PBW クリスタロイド (70 ml に四捨五入) または 25 グラムのアルブミンをできるだけ早く投与します。 各ボーラス投与の直前と直後、および 60 分後に熱希釈を実行します。 ボーラス投与後 60 分以内に EVLW が 2ml/kg PBW を超えて増加した場合は、次回の定期測定までボーラス投与を行わないでください。 この治療法は、マップが 60 未満の患者、または測定された GEDI が目標を下回っている昇圧剤を使用している患者に利用可能です (いずれかを単独で使用することも、組み合わせて使用することもできます)

実験的:EVLW
EVLW が 9 ml/kg PBW を超えると、アルゴリズムによる治療が開始され、EVLW ≤9 ml/kg PBW または抜管のいずれか許容できる方になるまで継続されます (図 6 を参照)。 フロセミドと体積収縮は、付随する低灌流を引き起こすことなく測定されたEVLWを減少させる手段として体積収縮を実行するのに十分な体積前負荷(GEDI)が利用できる場合に開始されます。 輸液の投与も EVLW の変化によって決まります。 輸液投与の結果として EVLW > 2ml/kg PBW が増加すると、次に予定されている測定まで、それ以上の輸液投与は削減されます。 私たちの最終的な治療目標は、血行動態の低下や末端器官損傷を併発することなく、EVLW を正常範囲に向けて最大限に低下させ、それによって肺の仕組みとガス交換を改善することです。 そうすることで、このアルゴリズムに基づいた目標指向の治療アプローチにより、結果が改善されるはずだと私たちは感じています。 |
