포함 기준:
다음의 급성 발병:
- PaO2/FiO2 300 이하.
- 정면 흉부 방사선 사진에서 폐부종과 일치하는 양측 침윤.
- 기관내관 또는 기관절개술을 통한 양압 환기에 대한 요구 사항.
- 폐 침윤을 설명할 수 있는 좌심방 고혈압의 임상적 증거는 없습니다. 측정된 경우 폐동맥 쐐기 압력이 18mmHg 이하입니다.
제외 기준:
- 만 18세 미만.
- 모든 포함 기준이 처음 충족된 후 24시간 이상 경과했습니다.
- C5 이상의 척수 손상, 근위축성 측삭 경화증, 길랭-바레 증후군, 중증 근무력증 또는 척추측만증과 같이 도움 없이 환기하는 능력을 손상시키는 신경근 질환(부록 I.A 참조).
- 임신(임신 가능성이 있는 여성에게 필요한 음성 임신 검사).
- 중증 만성 호흡기 질환(부록 I.C 참조).
- 중증 만성 간 질환(Child-Pugh 11 - 15, 부록 I.E 참조)
- 무게 > 160kg.
- 전체 체표면적의 70% 이상 화상.
- 6개월 사망률이 50% 이상으로 추정되는 악성 종양 또는 기타 돌이킬 수 없는 질병 또는 상태(부록 I.A 참조).

활성 비교기: 수정된 FACTT(대조군)
조사자의 컨트롤 암은 FACTT 시험에서 사용된 프로토콜을 기반으로 ARDSnet 그룹에서 발표한 ALI 환자의 체액의 보존적 관리를 위한 단순화된 알고리즘으로 구성됩니다.
이 프로토콜은 ARDSnet 환기에 대한 엄격한 준수, 우리의 이유 프로토콜 및 이러한 치료의 변화가 우리의 결과를 심각하게 혼란스럽게 할 수 있다고 느끼기 때문에 선택적인 혈관 활성, 베타-아드레날린성 약물의 사용을 요구합니다.
알부테롤 투여는 이프라트로피움에 반응하지 않는 생명을 위협하는 기관지 경련을 제외하고 어느 쪽 팔에서도 허용되지 않습니다.
이프라트로피움은 기관지경련에 대한 치료 의사의 재량에 따라 투여될 수 있습니다.

실험적: EVLW
EVLW가 9ml/kg PBW를 초과하면 알고리즘 치료가 시작되고 EVLW ≤9ml/kg PBW 또는 발관 중 먼저 오는 것이 허용될 때까지 계속됩니다(그림 6 참조). 푸로세마이드와 체적 수축은 관류저하를 수반하지 않고 측정된 EVLW를 감소시키는 수단으로 체적 수축을 시행할 수 있는 충분한 체적 예압(GEDI)이 제공될 때 시작됩니다. 유체 관리는 또한 EVLW의 변경 사항에 따라 안내됩니다. 수액 투여의 결과로 EVLW > 2ml/kg PBW의 증가는 다음 예정된 측정까지 추가 수액 투여를 줄입니다. 우리의 궁극적인 치료 목표는 EVLW를 정상 범위로 최대한 낮추어 폐 역학 및 가스 교환을 개선하는 것입니다. 수반되는 혈역학적 손상 및 말단 기관 손상을 유발하지 않습니다. 그렇게 함으로써 우리는 이 알고리즘적이고 목표 지향적인 치료적 접근이 결과를 개선해야 한다고 생각합니다. |
