脊髄性筋萎縮症 (SMA) は、重度の筋力低下を引き起こす遺伝性疾患です。 これは、子供の筋力低下を引き起こす最も一般的な状態の 1 つです。 SMA の患者は、乳幼児期に衰弱を発症することが最も多い。 SMAのほとんどの人は、時間の経過とともに筋力と能力を徐々に失います. SMA の重度の乳児型である SMA タイプ I の赤ちゃんは、通常、数週間または数か月かけて能力と体力を急速に失います。
バルプロ酸 (VPA) は、てんかん患者の治療に長年使用されてきた薬です。 最近の研究では、VPA が SMA 患者細胞株における SMN 遺伝子のバックアップ コピーの発現をアップレギュレートできる可能性があることが示唆されています。 さらに、いくつかの予備データは、SMA の動物モデルで生存を延長する可能性があることを示唆しています。 VPAはSMAタイプIの子供のカルニチンを枯渇させる可能性があるため、毒性の可能性を防ぐためにカルニチンが追加されています.

- SMAの遺伝子診断と一致するSMN変異/欠失の検査記録
- SMA type Iの臨床診断
- 生後2週間から12ヶ月
- 両親/保護者の書面によるインフォームドコンセント
除外基準:
- -肝臓または膵臓の機能不全の臨床的または検査的証拠。
- -治験薬の開始前14日以内に行われた検査結果は、以下を示しています。

- -研究に参加する前の2週間以内に全身治療および/または入院を必要とする深刻な病気。
- -VPAまたはカルニチン代謝を妨げる研究登録から30日以内の薬物またはサプリメントの使用; VPAまたはカルニチンの潜在的なリスクを高める;または、リルゾール、バルプロ酸、ヒドロキシ尿素、アルブテロールの経口使用、フェニル酪酸ナトリウム、酪酸誘導体、クレアチニン、成長ホルモン、アナボリックステロイド、プロベネシド、経口または非経口など、SMA動物モデルまたはヒト神経筋障害に有益な効果があると仮定されている入口でのコルチコステロイドの使用、または筋力を増減させると予想される薬剤、またはヒストン脱アセチル化酵素(HDAC)阻害が推定される薬剤。
- -研究登録から30日以内にSMAの治療試験に参加した乳児、またはこの研究の過程で他の治療試験に登録する予定の乳児。
- 研究評価のための旅行への不本意。
- -VPA /カルニチンの使用に矛盾する併存する病状、または研究サイトへのおよびからの旅行。
