反復経頭蓋磁気刺激 (rTMS) は、脳の興奮性に変化をもたらすことができる脳刺激の非侵襲的手段です。 一連の毎日のrTMSセッションの後、この神経回路の変調および他の遠隔効果は、PDの運動症状および神経精神症状の一部を一度に数か月間助ける可能性があります. 最近、FDA は、偽対照試験とその安全性プロファイルで有効性が実証された後、投薬抵抗性うつ病の治療として、前頭前野での毎日の rTMS を承認しました。 PD 患者における rTMS のいくつかの小規模なパイロット研究の中で、運動皮質または前頭前皮質のいずれかに対する rTMS は、重大な有害事象なしで、運動および気分 (うつ病) 症状に有益な効果を示すことが報告されています。 しかし、PD 患者において、気分および運動症状の両方に対する運動、前頭前野、またはその両方に対する rTMS の相対的有効性はまだ確立されていません。

包含基準:
- 運動障害の専門知識を持つ神経科医によって確認された、英国ブレインバンク基準によるPDの診断。
- PD の正式な診断から 3 年以上経過し、ドーパミン作動性療法が必要である (少なくとも、レボドパおよび/またはドーパミン アゴニスト療法)。
- 15 ポイント以上のモーター UPDRS の最小ベースライン OFF スコア。
- 初期の顕著な小脳、錐体、または自律神経の機能障害など、非定型パーキンソニズムを示唆する特徴の欠如;核上凝視麻痺;症状の最初の 1 年以内に該当します。ドーパミン作動薬を開始する前の幻覚。

- 頭蓋内圧上昇の徴候または症状。

- 過去6か月以内にうつ病で入院が必要な患者は、研究に参加できません。 研究中に参加被験者のうつ病が悪化し、入院が必要とみなされる程度になった場合、または被験者が重大な自殺念慮を発症した場合、被験者は研究から脱落し、治療のために精神科医に紹介されます。
- 双極性感情障害の患者、および精神病的特徴を特徴とするうつ病の患者。
- 自発的な幻覚または妄想の既往歴のある患者、および他の潜在的な精神病性障害(統合失調症、統合失調感情障害、妄想性障害など)の患者。 登録医師が抗パーキンソン病薬に明らかに関連しているとみなす視覚的錯覚または幻覚の存在は許可されますが、登録医師がそれらが安定しており、投薬の変更を必要としそうにないと信じている場合に限ります(つまり、抗精神病薬の追加または抗パーキンソン薬の投与量)。 妄想のある患者は除外されます。
