口腔がんの化学予防のための生体接着ゲルの臨床評価

顕微鏡で確認された前癌性口腔上皮疾患(上皮異形成)を有する患者41人が、オハイオ州立大学、ノースカロライナ大学チャペルヒル校、ルイビル大学の3つの臨床センターでこの試験に登録される。 3 つの施設すべてで、参加者の半数が 10% FBR ジェル (0.5 gm を 1 日 4 回、3 か月間) にランダムに割り当てられ、半数がプラセボ対照群に入ります。
確立された標準治療に従って、すべての参加者は診断を確立するために疑わしい口腔病変の生検を受ける必要があります(研究ではありません)。 治験参加者は合計 3 回の生検を受けます。 治療前の生検には以下が含まれます: 1) FBR 代謝プロファイリング研究のための病変周囲組織および単一の唾液サンプル (代謝プロファイリングのために 10% FBR ゲルを 0.5 グラム 1 回塗布した 15 分後に組織と唾液を採取します。 ゲルの塗布および非病変生検は、病変組織の切開生検の前に行われます)、および 2)診断を確立し、実験パラメータの治療前ベースラインを提供するために病変組織の半切片が得られます。 治療前の生検には病変周囲組織と病変組織の両方の除去が含まれますが、病変周囲組織は病変組織と隣接しているため、手術創は 1 つだけになります。 残りの病変組織を含む治療部位の最終切除生検(口腔異形成病変の切除は現在の標準治療と一致します)は、3か月の治療後に行われます。 実験計画では、各患者を独自の内部対照として機能させることができます。 簡単に言うと、以下のパラメータがすべての参加者でモニタリングされます(患者に適合する治療前から治療後の生検と比較して行われます):1)光学顕微鏡診断、2)臨床的外観および病変サイズ、3)マイクロアレイ遺伝子発現解析、4)微小血管密度表層結合組織のデータ、5) 腫瘍抑制遺伝子に関連する遺伝子座での LOH インデックス、6) COX-2 および iNOS タンパク質の上皮内レベル (画像解析による免疫組織化学の定量化)、7) 注目された化学予防効果の程度に対する FBR 代謝プロファイルの比較。

- 年齢:21歳から80歳まで
- 顕微鏡で確認された前癌性口腔上皮疾患
- がんの既往歴がないこと(皮膚の基底細胞がんを除く)
- 治験に参加する前の少なくとも6週間は禁煙しており、治験の3か月間は禁煙を継続する
- 必要な研究フォローアップ評価を利用可能(試験期間中は10~14日ごと)
- インフォームド・コンセントを提供できる。
除外基準:
- がんの既往歴(皮膚の基底細胞がんを除く)
- 現在タバコ製品を使用している、または3か月の研究期間中禁煙を拒否している
- 顕微鏡的に確認された前癌性の口腔上皮変化の欠如
- 口腔扁平上皮癌の顕微鏡診断
- 頭頸部領域の同じ側に放射線療法を受けたことのある患者
- あらゆる種類のベリーに対するアレルギー歴
- 治験中に妊娠していると判断された、または妊娠を計画している女性
- 授乳中の女性たち。

光学顕微鏡による組織学的スコア化診断により、治療前から治療後まで診断可能
時間枠:3ヶ月の治療前と治療後です。

診断を確立し、実験パラメーターの治療前のベースラインを提供するために、3 か月の治療前に病変組織の半切片が行われます。
残存病変組織を含む治療部位の肛門切除生検(口腔異形成病変の切除は現在の標準治療と一致する)を治療後診断を行うために3か月の治療後に取得します。
0 ~ 8 の組織学的スケールは次のとおりです。0 = 正常、過角化症の有無にかかわらず最良の結果、1 = 異型、2 = 軽度の異形成、3 = 軽度〜中等度の異形成、4 = 中等度の異形成、5 = 中等度〜重度の異形成、6 = 重度の異形成、7 = 上皮内癌、8 = 浸潤性口腔扁平上皮癌 (最悪の結果)。

病変の大きさの変化

残りの口腔形成異常病変は、フォローアップのたびに (10 ~ 14 日ごと) 検査されます。
生検は、硬くなった、うねった境界、治癒していない潰瘍などの悪性変化の兆候がある患者に対して直ちに実施されます。したがって、これらの患者は治験から撤退します。
参加者は、接触粘膜炎と一致する変化がないかどうかも監視されます。
適用部位の痛みと紅斑。
患者の記録用に臨床写真が撮影されました。
定規を置いた治療前および治療後の写真を使用して、治療前および治療後のサイズを正確に測定しました。
注: 治療が有益な場合、病変サイズは減少し、負の数として反映されます。
