The Women In Steady Exercise Research (WISER) Survivor Trial & 病気の費用と代替管理戦略の費用対効果
WISER Survivor は、過体重または肥満で乳がん関連のリンパ浮腫を患っている座りがちな乳がんサバイバーを対象とした、1 年間の減量と運動に関する研究です。
この試験には、運動のみ、減量のみ、運動と減量の組み合わせ、対照群の 4 つのグループがあります。
この研究の主な目的は、リンパ浮腫の臨床転帰に対するこれらの介入の効果をテストすることです。
副次的な結果には、体重減少、乳がん関連のバイオマーカー、および生活の質が含まれます。
資金が利用できる限り、参加者は再発と死亡率への影響を調べるために長期的に追跡されます。

- 乳がんサバイバー
- 過体重または肥満 (BMI 25 以上)
- 乳がんに関連するリンパ浮腫を持っている必要があります
- 治療後少なくとも 6 か月 (例: 手術、化学療法、または放射線療法)が、診断からの経過時間に上限はありません
- 対象年齢の下限はありません。
- 現在、がんのない
除外基準:
- 運動プログラムへの参加を禁止する、または私たちの主な目的をテストする能力に悪影響を与える病状または投薬
- 補助なしで6分間歩くことができない
- 極度の肥満(体格指数 50 kg/m2 以上)
- 追加の計画 (例: -研究期間中の治癒または再建)手術
- 過去 1 年間のウェイトリフティングの自己申告
- 中程度の強度の有酸素運動を週に 3 回以上行っている
- 来年中にその地域から引っ越す予定
- 減量薬の現在の使用 (OTC または処方箋)

NO_INTERVENTION:リンパ浮腫ケアのみ
4つのグループすべてが、次のようにリンパ浮腫ケアを受けます。

実験的:運動のみ
エクササイズ インターベンションは、60 ~ 90 分の週 2 回の監視付きウェイト リフティング セッションと、週 180 分の有酸素運動を組み合わせたものです。
女性は、認定されたフィットネスの専門家によって、ウェイトリフティングの介入と、有酸素運動活動を安全に増加させる方法の両方について、週 6 回のセッションでトレーニングを受け、その後、電話での連絡と月 1 回の対面セッションを通じて監視されます。
すべてのエクササイズ参加者には、1 ~ 21 ポンドから 1 ~ 2 ポンド単位で抵抗を増やすことができる調節可能なダンベルである「パワー ブロック」が提供されます。
最初の6週間のセッションと毎月のチェックインセッションを除いて、すべてのウェイトトレーニングは自宅で行われます.

減量介入は、24 週間の集中段階から始まります。これには、毎週のミーティングと、商用メーカー (NutriSystem®, Inc., Fort Washington, PA) からのすべての食事とスナックの提供が含まれます。
この最初の 24 週間は、毎日のカロリー摂取量を 1 日あたり 1200 ~ 1500 カロリーに厳密に管理します。
参加者は、目標体重に達するまで 1 日あたりのカロリー数を維持し、その後、残りの介入を通じて体重を維持するために、カロリー摂取量を徐々に増やしていきます (約 500 カロリー/日)。
