The Women In Steady Exercise Research (WISER) Survivor Trial & kosten van ziekte en kosteneffectiviteit van alternatieve managementstrategieën
WISER Survivor is een eenjarig onderzoek naar gewichtsverlies en lichaamsbeweging voor overlevenden van sedentaire borstkanker die overgewicht of obesitas hebben met lymfoedeem gerelateerd aan borstkanker.
Er zullen vier groepen in deze proef zijn: alleen sporten, alleen afvallen, sporten en afvallen gecombineerd, en een controlegroep.
Het primaire doel van deze studie is om de effecten van deze interventies op klinische lymfoedeemuitkomsten te testen.
Secundaire uitkomsten zijn onder meer gewichtsverlies, biomarkers die verband houden met borstkanker en kwaliteit van leven.
Voor zover er financiering beschikbaar is, zullen deelnemers langdurig worden gevolgd om effecten op recidief en mortaliteit te onderzoeken.

Deelname Criteria
Onderzoekers zoeken naar mensen die aan een bepaalde beschrijving voldoen, de zogenaamde geschiktheidscriteria. Enkele voorbeelden van deze criteria zijn iemands algemene gezondheidstoestand of eerdere behandelingen.

Beschrijving
Inclusiecriteria:
- overlevende van borstkanker
- overgewicht of obesitas (BMI van 25 of hoger)
- moet aan borstkanker gerelateerd lymfoedeem hebben
- minimaal 6 maanden na de behandeling (bijv. operatie, chemotherapie of radiotherapie), maar er is geen bovengrens voor de tijd die verstrijkt vanaf de diagnose
- de in aanmerking komende leeftijdscategorie heeft geen ondergrens.
- momenteel vrij van kanker
Uitsluitingscriteria:
- medische aandoeningen of medicijnen die deelname aan een oefenprogramma zouden verbieden of een negatieve invloed zouden hebben op ons vermogen om onze primaire doelen te testen
- onvermogen om 6 minuten zonder hulp te lopen
- extreem zwaarlijvig (body mass index groter dan of gelijk aan 50 kg/m2)
- plannen voor aanvullende (bijv. curatieve of reconstructieve) chirurgie tijdens de studieperiode
- zelfrapportage van gewichtheffen in het afgelopen jaar
- al bezig met 3 of meer keer per week aërobe activiteit van matige intensiteit
- van plan om het komende jaar uit het gebied te verhuizen
- huidig gebruik van medicijnen voor gewichtsverlies (OTC of recept)
- zelfrapportage van alcohol- of middelenmisbruik in de afgelopen 12 maanden, inclusief risicodrinken (huidige consumptie van meer dan 14 alcoholische dranken per week)
- gewichtsverlies van meer dan 10% in de afgelopen 3 maanden
- geschiedenis van bariatrische chirurgie
- vrouwen die zwanger zijn

GEEN_INTERVENTIE: Alleen zorg voor lymfoedeem
Alle 4 de groepen krijgen als volgt lymfoedeemzorg:

EXPERIMENTEEL: Alleen trainen
De Exercise Intervention combineert tweemaal per week 60-90 minuten gewichtheffen onder toezicht met 180 minuten wekelijkse aerobe training.
Vrouwen worden getraind door gecertificeerde fitnessprofessionals in zowel de gewichtheffen-interventie als in het veilig verhogen van hun aerobe trainingsactiviteit gedurende 6 wekelijkse sessies, en worden vervolgens gevolgd via telefonisch contact en maandelijkse persoonlijke sessies.
Alle deelnemers aan de oefening krijgen 'Power Blocks', verstelbare dumbbells waarmee ze de weerstand kunnen verhogen in stappen van 1-2 pond van 1-21 pond.
Alle gewichtstrainingen worden bij hen thuis gedaan, behalve de eerste 6 wekelijkse sessies en maandelijkse check-in-sessies.
Groepsleden die alleen trainen, ontvingen ook de hierboven beschreven zorginterventie voor lymfoedeem.

De Exercise Intervention combineert de tweewekelijkse gewichtheffen-interventie van PAL met de 180 minuten wekelijkse aerobe training van de onlangs voltooide WISER-proef en de lopende WISER Sister Trial.
Vrouwen zullen worden getraind door gecertificeerde fitnessprofessionals in zowel de PAL-interventie voor gewichtheffen als in het veilig verhogen van hun aerobe trainingsactiviteit gedurende 6 wekelijkse sessies, en vervolgens worden gevolgd via telefonisch contact en maandelijkse persoonlijke sessies.

EXPERIMENTEEL: Alleen gewichtsverlies
De gewichtsverliesinterventie begint met een intensieve fase van 24 weken die wekelijkse bijeenkomsten omvat en alle maaltijden en snacks verstrekt door een commerciële fabrikant (NutriSystem®, Inc., Fort Washington, PA).
De dagelijkse calorie-inname wordt gedurende deze eerste 24 weken strikt gecontroleerd op 1200-1500 calorieën per dag.
Deelnemers worden begeleid om op hetzelfde aantal calorieën per dag te blijven totdat ze het streefgewicht bereiken, gevolgd door een geleidelijke toename van de calorie-inname (van ongeveer 500 calorieën/d) om hun gewicht te behouden gedurende de rest van de interventie.
De behandelgroepen staan onder leiding van geregistreerde diëtisten en krijgen doorlopend toezicht via telefoon- en e-mailcontact.
Groepsleden die alleen gewicht verloren, ontvingen ook de hierboven beschreven lymfoedeemzorginterventie.

Gecombineerde groepsleden ontvingen ook de hierboven beschreven zorginterventie voor lymfoedeem.
