足関節固定術と足関節形成術の比較
末期の足首関節炎 (ESAA) は、重度の痛み、機能障害、および生活の質の低下を伴う衰弱状態です。 多くの ESAA 患者は、100 フィート (30 m) も歩くことも、階段を 1 段上るのも困難です。 ESAA の手術を希望する患者には、足首関節固定術 (足首の固定術) と足首関節形成術 (足首の置換術) の 2 つの主要な治療オプションがあります。 これら 2 つの手順の有効性を直接比較した研究はほとんどなく、ランダム化比較試験 (RCT) は実施されていません。
治験責任医師は、足首関節固定術および足首関節形成術を受ける被験者について、手術前、手術後 3 か月および 6 か月、および 10 年までの年次ベースで以下を比較します。

研究開始から 2014 年 3 月まで、研究者らは 2 年間の追跡期間にわたって足関節固定術と足関節形成術の有効性を比較する多施設ランダム化比較試験 (RCT) を実施しました。 この研究では、修正された包括的コホート デザインが使用されました。 手術に無作為化することを望まない被験者は、優先コホートに入り、外科医と相談して手術を選択することにより、研究に参加することができます。
2014 年 3 月までに、ランダム化されたコホートに参加した被験者はいませんでした。 データおよび安全性監視委員会 (DSMB) は、無作為化された被験者が手術に無作為化されることを望んでいなかったため、研究の無作為化されたアームでの募集を中止することを推奨しました. すべての患者は優先コホートに登録され、関節固定術または関節形成術が選択されました。 研究デザインの変更にもかかわらず、目的は変わりません。

説明
包含基準:
- 足首関節炎の診断
- 保存的治療に失敗し、手術候補と見なされた
- 21歳から89歳までの成人患者
- 歩行可能だが、痛みのない歩行への主な障害が足首関節炎である
除外基準:
- -最近の外科的、神経学的、代謝的、または下肢の筋骨格系の問題で、重度の膝や股関節の変形性関節症など、研究における歩行手段を損なう可能性があります
- 複数回の修正が必要な煩雑な手続き
- 多巣性疾患を伴う炎症性関節炎(すなわち、 体の複数の部分に影響を与える関節炎)
- -参加に同意するための不十分な認知機能または言語機能
- -手術後24か月間、クリニックのフォローアップおよび/またはリモートフォローアップに参加できない、または参加したくない

好み、足首関節固定術
|
すべての外科医は、堅固な内部固定の十分に確立された技術を採用します。
この技術では、表面から軟骨を取り除いて関節を準備し、骨を希望どおりに配置し、骨を一緒に圧縮して動きを防ぐためにネジやプレートを取り付けます。
この技術により、ギプスや外部サポートが不要になります。
患者は、すぐに体重を支える補助具を使って歩くことができます。
体重負荷状態を判断するために、6週目にX線写真を撮ります。
手術後 6 週間から 12 週間かけて徐々に肢に体重をかけていきます。

Ankle specific function was assessed using the Foot and Ankle Ability Measure (FAAM) Activities of Daily Living (ADL) subscale, a validated, self-reported questionnaire which measures functional ability during activities of daily living.

FAAM ADL subscale sums and coverts responses to a standardized score ranging from 0 to 100%.
A score of 0% represents complete disability, and a score of 100% represents no functional limitations.
Higher scores indicate better ankle function.

Ankle specific function was assessed using the Short Form-36 (SF-36) Physical Component Summary (PCS) score, a validated, generic measure of health-related quality of life that evaluates physical health status.
The PCS score is derived from responses to the SF-36 questionnaire and is norm-based, with scores ranging from 0-100, where higher scores indicate better physical health and lower scores indicate poorer physical health.
