これは、潜在的または境界切除可能転移性結腸直腸腺癌(RASおよびB-RAF WT腫瘍)と確定診断され、転移性疾患に対してこれまでに化学療法を受けていない患者を対象とした第II相非盲検ランダム化研究である。
この研究は、術前化学療法セツキシマブ後に観察された病理学的反応をFOLFOXまたはFOLFIRIと比較することを目的としています。

- インフォームドコンセントに署名した時点で18歳以上の女性または男性の患者
- ECOG パフォーマンス ステータス 0 または 1
- 上皮内原発腫瘍の有無にかかわらず、結腸または直腸の腺癌の組織学的または細胞学的確定診断。 野生型 RAS および B-RAF 腫瘍の状態。
- 診断時に切除可能な転移性疾患を有しており、治癒目的で最初に化学療法が推奨される患者。 必要に応じて、切除可能性を 1 段階または複数段階で計画することができます。 一般に認められているように、切除可能とは、辺縁に腫瘍がなく、十分な肝予備能を備えた、検出可能なすべての(肝臓)病変の外科的除去(+/- 高周波アブレーション)を意味します。 実際には、両側の腫瘍の位置、病変の数と位置、および不十分な肝予備能が依然として主な決定要因となります。
- 患者が mCRC に対する化学療法を受けたことがない場合、転移性疾患の部分的および軽度の切除は、包含前 3 か月以内に許可されます。
- 追加の肝転移部位は 1 部位に限定されます。
- 患者は、最後の化学療法が組み入れの少なくとも6か月前(オキサリプラチンの場合は12か月)前に投与された場合に限り、骨盤への補助化学療法または(新規)補助化学放射線療法を受けている可能性があります。 骨盤への以前の放射線治療は除外基準ではありません。
以下のような血液、腎臓、肝臓の機能が適切であること。
血液学的:
ヘモグロビン >9g/dl 好中球 > 1.5 x 109/L 血小板 > 100 x 109/L
腎臓:
クレアチニン < 1.5 x ULN (正常の上限)
肝臓:
ビリルビン 1.5 X ULN 以下、AST (アスパラギン酸アミノトランスフェラーゼ)、および ALT (アラニン アミノトランスフェラーゼ) 5 x ULN 以下、Phos Alc 5 x ULN 以下
- 女性患者は、閉経後、無菌(外科的、放射線照射、または化学的誘発)であるか、または許容される避妊方法を使用して性的に活動的である必要があります。
- 男性患者は外科的に不妊であるか、性的に活動的で閉経前のパートナーがいる場合は許容可能な避妊方法を使用している必要があります。
- 積極的な治療を行わない場合、余命は少なくとも3か月です。
除外基準:
- 1.診断時に切除不能なmCRCである
- 2.mCRCに対する以前の化学療法または全身療法。 結腸直腸がんに対する補助化学療法は、対象となる前に 6 か月以上前に完了していれば除外基準にはなりません。 オキサリプラチンベースの化学療法は、参加の 1 年以上前に完了する必要があります。
- 3.セツキシマブ、パニツムマブ(または他の抗EGFR(上皮成長因子受容体)療法)の以前の利用。
- 4.上腹部に照射された以前の放射線治療。
- 5 測定不可能な疾患(RECIST 1.1基準)
- 6.臨床的または放射線学的評価によって決定される、腹水、肝硬変、門脈圧亢進症、主要な門脈腫瘍の関与または血栓症の証拠。
- 7.以前の大規模な肝臓切除術: 残存肝臓が最初の肝臓容積の 50% 未満。
- 8.患者をこのプロトコールによる治療に不適当にする非悪性疾患。
- 9.中枢神経系転移の併発
- 10.グレード2以上の末梢神経障害。
- 11.間質性肺疾患
- 12.妊娠中または授乳中。
- 13.患者は、以下を除く悪性腫瘍を以前または併発している:5年以上寛解している浸潤性悪性腫瘍、および非黒色腫皮膚癌または子宮頸部上皮内癌。

|
主要病理学的応答率 (MPRR) は、主要な病理学的応答を示した患者の割合として定義されます。
病理学的反応は、Rubbia-Brandt 腫瘍回帰グレード分類に従って評価されます。多発性結腸直腸転移を有する患者の場合、全体的な病理学的反応は、すべての転移の平均 TRG に基づいて分類されます。主要な反応は、平均 TRG < 3 として定義されます。部分奏効は、平均 TRG ≧ 3 および <4 を示す患者に対して定義され、平均 TRG ≧ 4 を示す患者に対しては無反応と定義されます。
