纳入标准:
- 签署知情同意书时年满 18 岁的女性或男性患者
- ECOG 体能状态 0 或 1
- 结肠或直肠腺癌的组织学或细胞学确诊,伴有或不伴有原发性肿瘤。 野生型 RAS 和 B-RAF 肿瘤状态。
- 诊断时有可能可切除的转移性疾病的患者,建议首先以治愈为目的进行化疗。 如果需要,可以在一个或多个阶段计划可切除性。 正如普遍承认的那样,可切除性意味着所有可检测(肝脏)病灶的手术清除(+/-射频消融),边缘无肿瘤,并与足够的肝脏储备相容。 实际上,双侧肿瘤位置、病变的数量和位置以及肝脏储备不足仍然是主要的决定因素。
- 如果患者从未接受过 mCRC 化疗,则允许在纳入前 3 个月内对转移性疾病进行部分和小切除术。
- 肝外转移部位仅限于 1 个部位。
- 患者可能已经接受了盆腔辅助化疗或(新)辅助化放疗,前提是最后一次化疗是在纳入前至少 6 个月(奥沙利铂为 12 个月)进行的。 以前对骨盆进行过放射治疗不是排除标准。

- 女性患者必须是绝经后、不育(手术或辐射或化学诱导),或者使用可接受的避孕方法性活跃。
- 男性患者必须经过手术绝育,或者如果性活跃并且有绝经前伴侣,则必须使用可接受的避孕方法。
- 在没有任何积极治疗的情况下,预期寿命至少为 3 个月。
排除标准:
- 1.诊断时明确不可切除的 mCRC
- 2.mCRC的既往化疗或全身治疗。 结直肠癌的辅助化疗不是排除标准,前提是它在纳入前 6 个月以上完成。 基于奥沙利铂的化疗必须在纳入前 1 年以上完成。
- 3.先前使用西妥昔单抗、帕尼单抗(或其他抗EGFR(表皮生长因子受体)疗法)。
- 4.以前的放射治疗传递到上腹部。
- 5 不可测量疾病(RECIST 1.1 标准)
- 6.根据临床或放射学评估确定的腹水、肝硬化、门静脉高压症、主要门静脉肿瘤受累或血栓形成的证据。
- 7.既往大肝切除术:残肝<初始肝体积的50%。
- 8. 使患者不适合根据本方案进行治疗的非恶性疾病。
- 9.并发中枢神经系统转移
- 10.周围神经病变≥2级。
- 11.间质性肺病
- 12.怀孕或哺乳期。
- 13.患者既往或合并恶性肿瘤,但:缓解期5年以上的侵袭性恶性肿瘤和非黑色素瘤皮肤癌或宫颈原位癌除外。
