Description détaillée
Il s'agit d'une étude de phase II, ouverte, randomisée chez des patients avec un diagnostic confirmé d'adénocarcinome colorectal métastatique potentiellement ou limite résécable (tumeurs RAS et B-RAF WT), qui n'ont pas reçu de chimiothérapie antérieure pour leur maladie métastatique.
L'étude vise à comparer les réponses pathologiques observées après la chimiothérapie préopératoire cetuximab avec FOLFOX ou FOLFIRI.

Critère d'intégration:
- Patients de sexe féminin ou masculin âgés d'au moins 18 ans au moment de la signature du consentement éclairé
- Statut de performance ECOG 0 ou 1
- Diagnostic histologique ou cytologique confirmé d'adénocarcinome du côlon ou du rectum, avec ou sans tumeur primitive in situ. Statut tumoral RAS de type sauvage et B-RAF.
- Patients porteurs d'une maladie métastatique potentiellement résécable au diagnostic et pour lesquels une chimiothérapie préalable à visée curative est recommandée. La résécabilité peut être planifiée en un ou plusieurs temps si indiqué. Comme communément admis, la résécabilité signifie la clairance chirurgicale (+/- ablation par radiofréquence) de toutes les lésions détectables (foie) avec des marges sans tumeur et compatibles avec une réserve hépatique adéquate. Pratiquement, la localisation tumorale bilatérale, le nombre et la localisation des lésions, et une réserve hépatique insuffisante restent les principaux facteurs décisionnels.
- La résection partielle et mineure de la maladie métastatique est autorisée dans les 3 mois précédant l'inclusion si le patient n'a jamais reçu de chimiothérapie pour le mCRC.
- La localisation métastatique extra hépatique est limitée à 1 site.
- Les patients peuvent avoir reçu une chimiothérapie adjuvante ou une chimio-radiothérapie (néo-)adjuvante du bassin, à condition que la dernière dose de chimiothérapie ait été administrée au moins 6 mois avant l'inclusion (12 mois pour l'oxaliplatine). Une radiothérapie antérieure du bassin n'est pas un critère d'exclusion.
Fonction hématologique, rénale et hépatique adéquate comme suit :
Hématologique:
hémoglobine >9g/dl Neutrophiles > 1,5 x 109/L Plaquettes > 100 x 109/L
Rénal:
Créatinine< 1,5 x LSN (limite supérieure de la normale)
Hépatique:
Bilirubine < ou égal à 1,5 X LSN AST (Aspartate Aminotransférase) et ALT (Alanine Aminotransférase) < ou égal à 5 x LSN, Phos Alc < ou égal à 5 x LSN
- Les patientes doivent être soit ménopausées, stériles (induites par chirurgie, radiothérapie ou chimique) ou sexuellement actives en utilisant une méthode de contraception acceptable.
- Les patients de sexe masculin doivent être chirurgicalement stériles ou s'ils sont sexuellement actifs et avoir un partenaire pré-ménopausique doivent utiliser une méthode de contraception acceptable.
- Espérance de vie d'au moins 3 mois sans aucun traitement actif.
Critère d'exclusion:
- 1. CCRm définitivement non résécable au moment du diagnostic
- 2. Chimiothérapie antérieure ou thérapie systémique pour le mCRC. La chimiothérapie adjuvante du cancer colorectal n'est pas un critère d'exclusion à condition qu'elle ait été réalisée plus de 6 mois avant l'inclusion. La chimiothérapie à base d'oxaliplatine doit être terminée depuis plus d'un an avant l'inclusion.
- 3. Utilisation antérieure du cétuximab, du panitumumab (ou d'un autre traitement anti-EGFR (récepteur du facteur de croissance épidermique)).
- 4. Radiothérapie antérieure administrée au haut de l'abdomen.
- 5 Maladie non mesurable (critères RECIST 1.1)
- 6. Preuve d'ascite, de cirrhose, d'hypertension portale, d'atteinte tumorale de la veine porte principale ou de thrombose, déterminée par une évaluation clinique ou radiologique.
- 7.Résection hépatique majeure antérieure : foie résiduel < 50 % du volume initial du foie.
- 8. Maladie non maligne qui rendrait le patient inapte au traitement selon ce protocole.
- 9.Métastases concomitantes du système nerveux central
- 10. Neuropathie périphérique ≥ grade 2.
- 11.Pneumopathie interstitielle
- 12. Enceinte ou allaitante.
- 13.La patiente a des affections malignes antérieures ou concomitantes, à l'exception : des affections malignes invasives en rémission depuis plus de 5 ans et d'un cancer de la peau non mélanique ou d'un carcinome in situ du col de l'utérus.

La résection des métastases se fera par voie chirurgicale, après une chimiothérapie randomisée (FOLFOX ou FOLFIRI) + Thérapie ciblée (Cetuximab).
La chirurgie permettra de comparer la réponse pathologique sur les métastases réséquées par le type chimiothérapie + thérapie ciblée.
Bolus de 5-FU 400 mg/m2, bolus IV toutes les 2 semaines Perfusion continue de 5-FU 2400 mg/m2, 46 h cont.

Le taux de réponse pathologique majeure (MPRR) est défini comme la proportion de patients présentant une réponse pathologique majeure.
La réponse pathologique sera évaluée selon la classification Rubbia-Brandt Tumor Regression Grade. une réponse partielle est définie pour un patient présentant un TRG moyen ≥3 et <4, et une non-réponse pour un patient avec un TRG moyen ≥4.

-La survie sans progression (PFS) est définie comme le temps entre la randomisation et le moment du premier événement (rechute du mCRC initial, développement d'un nouveau cancer colorectal ou décès quelle qu'en soit la cause).
Les patients sans événement de ce type au moment de l'analyse des données seront censurés à la dernière date à laquelle ils étaient connus pour être sans événement.
L'analyse de la SSP sera basée sur les évaluations des tumeurs et les évaluations de suivi de la survie.

La chimiothérapie néo-adjuvante systémique dans le mCRC provoque fréquemment des lésions morphologiques impliquant la microvascularisation hépatique.
L'obstruction sinusoïdale, compliquée de fibrose périsinusoïdale et de lésion veino-occlusive du foie non tumoral, doit figurer dans la liste des effets secondaires indésirables de la chimiothérapie systémique colorectale, notamment liée à l'utilisation de l'oxaliplatine.
