この研究の目的は、TACEと組換えヒトアデノウイルス5型注射が、手術や局所切除療法ができない進行性肝細胞癌(HCC)患者の転帰を改善するかどうかを判断することです。

経動脈化学塞栓術(TACE)は現在、世界中で切除不能な肝細胞癌(HCC)患者に対する緩和治療の主流の 1 つとなっています。しかしながら、TACE で治療された HCC 患者の長期転帰は一般に不良でした。
E1B遺伝子欠失アデノウイルスである組換えヒトアデノウイルス5型は、顕著な抗腫瘍活性を有することが知られている。
さらに、組換えヒトアデノウイルス 5 型を局所注射すると、抗腫瘍療法 (化学療法および放射線療法) の効果を高めることができます。
仮説は、切除不能な HCC 患者は TACE と組み合わせた組換えヒト アデノウイルス 5 型から恩恵を受ける可能性があるというものです。

- 欧州肝臓研究協会の基準に従って新たに HCC と診断された患者。
- BCLC ステージ A または B
- Child-Pugh クラス A または B (Child-Pugh スコア 7)
- ECOG パフォーマンス ステータス 0
患者は、以下の基準の両方を満たす腫瘍病変を少なくとも 1 つ持っている必要があります。
- RECIST 基準に従って、少なくとも 1 つの次元で病変を正確に測定できます。
- この病変は、これまでに手術、放射線療法、高周波アブレーション、エタノールまたは酢酸の経皮注射、または冷凍アブレーションによる治療を受けていません。
- 以前に非標的病変に対する局所療法(RFA、PEI、冷凍アブレーション、手術、切除)を受けた患者が対象となります。
- 局所治療はベースラインスキャンの少なくとも 4 週間前に完了している必要があります。
- 血液学:好中球絶対数 (ANC) > 1 x 109/L、血小板数 > 40 x 109/L、ヘモグロビン > 9 g/dL (このレベルを維持または超えるために輸血される場合があります) プロトロンビン時間の国際正規化比 < 1.5
- 生化学:総ビリルビン < 2 mg/dL 血清クレアチニン < 1.5 x 正常値の上限
- プロトコールを理解し、書面によるインフォームドコンセント文書に同意して署名する能力
除外基準:
腫瘍因子
- 肝外転移の存在
- 主に浸潤性病変
- 両葉に及ぶ広範な病変を伴うびまん性の腫瘍形態。
血管合併症
- 肝動脈血栓症、または
- 主要な門脈の部分的または完全な血栓症、または
- 対側葉の門脈枝への腫瘍浸潤、または
- 肝静脈腫瘍血栓、または
- シャント閉塞の影響を受けにくい重大な動脈門脈シャント
肝機能
- 進行性肝疾患:腹水、肝性脳症
- -研究登録前の30日以内に臨床的に重大な胃腸出血を起こした患者。
その他
- 血液透析または腹膜透析を必要とする腎不全
- 妊娠中または授乳中の女性。
- 活動性敗血症または出血。
- 静脈内造影剤に対する過敏症。
- 患者は、HCC標的病変に対して以前に治療を受けている。
心臓病の病歴
- うっ血性心不全 > NYHA クラス 2。活動性冠動脈疾患
- β遮断薬やジゴキシン以外の抗不整脈療法を必要とする不整脈。
- 高血圧は、最適な医学的管理にもかかわらず、収縮期血圧 > 150 mmHg または拡張期血圧 > 90 mmHg として定義されます。
- 重篤な治癒していない創傷(二次的意図による創傷治癒を含む)、急性または治癒していない潰瘍、または3か月以内の骨折。
- 研究者の意見では、患者は治療および研究の指示に従うことができない、および/または従いたがらない。
- 患者の研究への参加または研究結果の評価を妨げる可能性のある薬物乱用(現在の)、心理的または社会的状態。
- 進行中の臨床的に重篤な感染症(グレード2以上のNCI-CTCAE ver 3.0)
